The Blackheath Brain Injury Rehabilitation Centre comprises two units. The Thames Brain Injury Rehabilitation Unit (21 beds), is an NHSE-funded Level 1C neuro-behavioural unit for patients with complex cognitive and/or behavioural rehabilitation needs due to acquired brain injury or neurological disease. The Heathside Neuro-disability Unit (18 beds), is an NHSE-funded Level 2A unit for patients requiring complex physical neuro-rehabilitation.

The site is part of the wider Active Care Group Neuro Network (consisting of 25+ sites) nationally. Blackheath was recently rated as GOOD by the CQC.

This is a unique opportunity for a motivated and enthusiastic individual to work in a Level 1c specialist neuro-rehabilitation service. The post-holder would be based mainly on the Thames Brain Injury Rehabilitation Unit, alongside the Consultant Neuropsychiatrist and another experienced Speciality Doctor. There is cross-cover with the Specialty Doctor on HNDU. Patients are referred mainly by London Major Trauma centres and central teaching hospitals, which affords the opportunity to gain invaluable experience in the assessment and management of complex and uncommon neuropsychiatric conditions. We have links with UCLH and Kings College London and involvement in clinical research is encouraged. There are no on-call commitments.

Job summary

The Frailty and Complex Care Nurse will work within their scope of practice as part of the Lincoln Healthcare Partnership (PCN) Frailty and Complex Care Team, providing proactive, person-centred care to individuals living with frailty, multiple long-term conditions, and complex health needs across both care home and community settings.

The post holder will support the delivery of the Enhanced Health in Care Homes (EHCH) service, providing clinical assessment, care planning, proactive management, and multidisciplinary support to residents within care homes. They will work closely with care home staff, GP practices, community services, and wider partners to prevent unnecessary hospital admissions and improve resident outcomes.

In addition, the post holder will support the Community Frailty and Complex Care Service, delivering comprehensive assessment, case management, care coordination, and ongoing clinical support to patients living in their own homes who have frailty, advanced illness, or complex healthcare needs.

The role requires the ability to work autonomously, undertaking comprehensive history taking, holistic assessment, clinical examination, and clinical decision-making. The post holder will identify individuals at risk of deterioration, contribute to anticipatory and personalised care planning, coordinate multidisciplinary interventions, and support patients to maintain independence and achieve the best possible quality of life.

A key requirement of this role is demonstrable experience in frailty assessment and management, together with excellent history-taking and clinical assessment skills, enabling the practitioner to formulate appropriate management plans and coordinate timely interventions.

Working under the leadership of the Frailty and Complex Care Clinical Lead and Lead GP, the post holder will work collaboratively with Frailty Care Coordinators, Clinical Pharmacists, General Practice teams, community services, and other healthcare professionals to deliver high-quality, integrated care.

The overarching aim of the role is to improve patient outcomes, reduce health inequalities, support people to remain safely in their usual place of residence, and reduce avoidable hospital admissions through proactive, coordinated care.

• To deliver high quality clinical care to the patients registered and attending the GP surgery and to work closely with the clinical leads and local practice team to support the delivery of the surgeries operational plan.

• To provide clinical care to patients as described in the job plan and in line with the total triage model. This will include consultation sessions (both in person and by video), telephone triage, correspondence, results, medication reviews and prescription signing and other clinical administration,safeguarding lead, medicals, reports, home visits, surgery meetings and on-call (excluding out of hours).

• The post holder will work as part of a team to deliver high quality palliative care in the in-patient unit at either Ashford/Canterbury/Thanet but may be required to work at any of the other sites upon request.
• The post holder will support the operational management by take responsibility for the ward in the absence of the Ward Manager and Ward Sister, ensuring patient care is maintained.
• Although not responsible for a budget, the post holder will be expected to be aware of cost implications and make the best use of valuable resources.
• The post holder will contribute to the learning environment, acting as practice supervisor/assessor to qualified/unqualified colleagues and students, to facilitate and assist in achieving learning objectives.

• The post holder will be a registered nurse accountable for the direct delivery of patient care within an in-patient unit caring for patients with palliative needs.
• Work will be undertaken in line with Pilgrims Hospice, local and national policy and guidance and the NMC professional codes of conduct.
• The post holder will work within the multidisciplinary team and will influence the decisions of patient care from a nursing perspective. On a day to day basis the post holder will work under indirect supervision making decisions based on best practice and evidence based research.
• Working within a team structure the registered nurse will ensure all incidents, complaints, difficulties and complex situations are documented and any remedial actions taken by the multi-disciplinary. In addition the post holder will ensure the Ward Sister or Ward Manager are notified and aware of such occurrences.

Principle accountabilities and responsibilities

Clinical and professional

• Maintain a high level of nursing care through direct delivery of personal, emotional and psychological care for patients. Through effective assessment of each patients needs and preferences the post holder will plan evidence based nursing care, implement and continuously evaluate the care given and ensure this meets the needs of individualized patient care.
• Undertake patient care as directed in the patients care plan in order to meet each patients care needs whilst maintaining independence as much as possible and maintaining privacy and dignity at all times. This will include for example hygiene and elimination needs, assisting eating and drinking and ensuring nutrition is maintained.
• Successfully manage clinical emergencies specifically related to palliative care through swift recognition, instigating and carrying out any immediate actions and reporting and sharing information quickly.
• Whilst responsible to the ward sister/team leader, the post holder will work competently and confidently without direct supervision to complete delegated work load, including maintaining skill mix and staff planning.
• To maintain effective operation, the post holder will take responsibility for the ward, delegate the workload and supervise trained and untrained staff as needed.
• Maintain continuity of patient care through accurate verbal reports and electronic and written documentation including the use of electronic patient management systems ensuring patient confidentiality at all times.
• Ensure all patient information is electronically recorded in a safe, accurate and contemporaneous manner through familiarity with and daily use of EMIS and Ashtons electronic patient management system and adherence to Hospice IT protocols.
• Expedite successful discharge where appropriate with good quality planning for patients needs.

• Maintain adequate levels of drugs, dressings and general stores by ordering according to hospice practice.
