Main duties of the job
This is a varied, non-patient-facing administration role providing essential back-office support across the division and Modality Practices. You will deliver an efficient administrative service, managing workloads, processing documentation accurately within agreed timescales, and communicating professionally with clinical and non-clinical colleagues.
The role includes supporting meetings across Practices, preparing and managing medical and insurance reports, tracking progress and payments, and coordinating records and documentation. You will also support Practices with complaints administration, patient registrations including GP2GP workflows, and call and recall processes such as test results, long-term condition management, childhood immunisations, and cytology.
The role requires flexibility, initiative, and commitment. You will work resourcefully as part of a wider team to ensure tasks are completed to a high standard in line with procedures, protocols, and service requirements.

Job summary
In this role, youll be trusted to support the safe movement and management of IT equipment, ensuring colleagues have the technology they need to deliver effective services.Youll be part of the IT Operations team, helping to keep devices and equipment organised, tracked and available across a range of sites. The Infection Prevention & Control Nursing Service is seeking to appoint a Band 6 Infection Prevention & Control Nurse to join our experienced and friendly team. This is an exciting opportunity for the right candidate who wishes to expand their knowledge, experience and learn new skills within a dynamic team.
The successful candidate will be expected to have a sound knowledge of clinical nursing, excellent clinical skills and experience working in the field of infection prevention & control. Job summary
We work with general practices and other healthcare providers in Hillingdon to deliver high quality services for patients. In addition, we aim to improve care for patients by facilitating collaboration across general practice, releasing clinical time and developing the primary care workforce.

We are looking for an experienced Admin Assistant to come and join our Warwickshire Cluster for 20 hours per week over Monday to Saturday. The role covers our units in Coventry, Nuneaton and Stratford. More details can be found in the further information section below.
You'll work as part of the team in carrying out administration tasks in an efficient and friendly manner ensuring that clients feel accepted, supported, and confident about BPAS services.
About You
You will have proven experience of working within an office environment, where you've carried out administration duties. You will be an excellent communicator both internally and externally and will demonstrate a friendly and positive attitude when interacting with clients, colleagues, and external parties.
Main duties of the job
- Answer and direct telephone queries, liaising with both internal and external colleagues, as necessary.
- Accurately check or complete BPAS and legal paperwork, ensuring all relevant documentation is available for the clients episode of care.
- Monitor, edit and amend appointment list, using as appropriate BPAS booking system, informing clients if changes to their appointment become necessary.
- Assist in the monitoring, control and ordering of stock, sort and distribute incoming mail and prepare and despatch statistical information and reports.

Job summary
We are looking for a caring and person-centred Healthcare Support Worker to join our Community Assessment Unit in Petersfield/Gosport.
You will be working within the multi-disciplinary Integrated Care Team that delivers community and clinic-based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.
The job will include home visiting and lone-working without direct supervision and a driving licence and access to a vehicle is required for this role.

Job summary
We are seeking a compassionate and motivated Healthcare Assistant to join our Primary Care Network team.Working as part of an integrated multidisciplinary team, the HCA will deliver high-quality, person-centred care to adults living with frailty, multiple long-term conditions and complex health needs across community and primary care settings.
This is an exciting opportunity to play a key role in delivering proactive, person-centred care to some of our most vulnerable patients. Working under the supervision of registered clinicians, you will support routine health monitoring, undertake clinical observations and contribute to the early identification of patient deterioration. You will work closely with GPs, nurses and the wider multidisciplinary team to improve health outcomes, promote independence and reduce health inequalities.

Main duties of the job
The Healthcare Assistant provides clinical support to older adults and adults with long-term conditions, undertaking phlebotomy, clinical observations, routine health monitoring and supporting long-term condition reviews in both primary care and community settings.
Working under the supervision of registered clinicians, the postholder plays a key role in identifying early signs of deterioration, supporting preventative healthcare and contributing to coordinated care for vulnerable patients.
- The Healthcare Assistant undertakes planned home visits to housebound patients, managing their own workload within agreed competencies while recognising limitations and escalating concerns appropriately.
- The postholder contributes to high-quality, person-centred multidisciplinary care, working collaboratively with GPs, nurses, therapists, social prescribers and other community services to improve health outcomes and reduce health inequalities.

Main duties of the job
The successful post-holder will become part of the PCNs clinical leadership team, joining an ambitious locality network that is committed to developing a strong, caring and compassionate multidisciplinary workforce.

Job summary
Coastal Medical Partnership is an innovative and forward-thinking Partnership that also represents a single Primary Care Network (PCN). We are in the unusual position of being able to work seamlessly across the network with no inter Partnership/Practice boundaries.
Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe, well and out of hospital as well as assisting to reduce the demands on general practice.
Main duties of the job
The purpose of the role is to support the GPs, Clinical Pharmacists and the wider MDT across the PCN, to maximise safe, cost effective prescribing to improve the quality of patient care. The post holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines related objectives. Under the supervision of a wider team the Pharmacy Technicians will use their expertise to ensure effective and efficient use of medicines, including running their own clinics within their area of expertise.

MATERNITY COVER-FTC-12 MONTHS
This fixed-term maternity cover offers an exciting opportunity for a registered Dietitian to work within the Medicines Optimisation Delivery Team in South West London (SWL). The role will focus on the delivery of strategies and actionable plans in relation to nutrition to deliver the best possible health outcomes for the SWL local population, with a focus on prevention, personalised care and health inequalities.
Main duties of the job
The role will support and at times lead primary care and neighbourhood NHS models of care at Place in enhancing outcomes from the prescribing of nutritional products, including ensuring value, safety, and overall quality improvement. The post holder will work collaboratively across boroughs within the Medicines Optimisation Delivery Team to facilitate the efficient implementation of 'do once and share' functions.

Job summary
The appointment is to commence ASAP for a fixed term period of 12 months. The role is full time, with a standard of 40 hours per week. There are no out of hours or weekend on call commitments.
MRCP is essential. The post does not award a National Training Number. Main duties of the job
The appointee will be part of a team of rheumatology clinicians, including consultants, resident doctors at various stages of training, and clinical nurse specialists who are responsible for providing outpatient services across 3 sites, inpatient care at the Royal Hallamshire Hospital, and review of ward referrals across the STH sites. Clinical supervision will be provided by the consultant team, and the appointee will have a named clinical supervisor for the duration of the post.
The successful candidate will work flexibly and co-operatively with other members of the team in order to provide a high quality, comprehensive service within this teaching hospital environment. To minimise travel between sites the job plan has been designed to ensure that wherever possible the successful candidate will be based at one site each day.

Rheumatology has 13 NHS consultants with a wide range of special interests. We have 4 specialist rheumatology registrars, 2 IMT3 trainees (Rheumatology/ GIM) and 2 junior trainee (one GP trainee and one core medical trainee). We have a Matron, clinical nurse specialists, a senior sister, a pharmacist and an extended scope physiotherapist supporting us with independently run out-patient clinics.
Rheumatology therapy services provide further specialist support to our patients - based within MSK outpatient facility. There are 3 specialist physiotherapists, additional junior physiotherapy staff on rotation, 2 specialist occupational therapists & in-department hydrotherapy.

Main duties of the job
To practice as an emergency care practitioner and demonstrate advanced clinical competence and knowledge above those associated with conventional nursing roles.
Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgery or the community as an autonomous practitioner.
Using own clinical judgement to diagnose and treat patients either face to face consultation, clinical telephone triage and other mediums.
Support patients to adopt health promotion strategies to improve healthy lifestyles, and apply principles of self-care.
Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.
Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, Patient Group Directions (PGDs), NMC standards or referring to registered GP as necessary.
Provide information and advice on prescribed or over the counter medication, medication regiments, side effects and interactions.
To recognise the need for and initiate where appropriate relevant diagnostic investigations, using their skills in this area to take the correct course of action.
Interpret test and investigation results as appropriate and advise patients of next steps.
Understanding local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults.
About us
Sandy Health centre is located in Sandy Bedfordshire is a purpose built building looking after patients in the town and surrounding villages. We have in circa 10.100 patients.
Our clinical team consists of 5 GP partners, 2 salaried GPs, physician associate, advanced nurse practitioner, nurse, HCA and phlebotomist. The clinical team is supported by a good admin team and dispensary staff. We now have a PCN pharmacist and pharmacy technician assigned to our practice to help with medication reviews. We are part of Sandhills PCN which covers two practices two in Sandy and one in Biggleswade and work closely with each other to improve patient care.
We work well as a team to offer excellent medical care to our patients and to support each during the working day as we all know how busy it can get.
