To support the South & East Leeds General Practice Group Practices with all aspects of medicines optimisation.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:

Management of long-term conditions

Minor illness

Management of medicines on transfer of care

Reviewing systems for safer prescribing

Contributing to repeat prescription authorisations and reauthorisation

Actioning acute prescription requests

Addressing both the public health and social care needs of patients

Contributing to achievement of QOF and locally commissioned quality improvement schemes

Undertaking clinical audit

The specifics of the role in each PCN will be worked up between the post-holder, the PCN and SEL GP Group.The post holder will be supported by existing practice employed pharmacists within the PCN (where these exist), a GP clinical mentor and the head of clinical pharmacy for SEL GP Group. They will also engage with pharmacy colleagues across Leeds for peer support.

The post holder will be supported to develop their role to become a non-medical prescriber, if that qualification is not already held, and will be required to enrol on the General Practice Pharmacist Training Pathway from CPPE unless an equivalent qualification is already held or exemption is agreed by CPPE.

Key duties and responsibilities

(Please note this is a list of options, it is not exhaustive, which options are deployed will be decided by the individual PCN, in conjunction with the clinical pharmacist and SEL GP Group). It is however mandated by NHS England that this role is patient-facing in nature.

1. Patient facing Clinical Medication Review

Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring. These reviews could be cohort based, in care homes, polypharmacy or any other area required by the PCN, within the pharmacists competence. Home visits may be required.

2. Medicines quality improvement

Undertake clinical audits of prescribing in areas directed by the PCN, feedback the results and implement changes in conjunction with the relevant practice team.

Identify cohorts of patients at high risk of harm from medicines through computer searches. This might include risks that are patient related, medicine related, or both. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups

3. Medicines safety

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

4. Management of common/minor/self-limiting ailments

Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring

to GPs or other healthcare professionals where appropriate.

5. Patient facing medicines support

Provide patient facing clinics for those with questions, queries and concerns about their

medicines in the practice.

6. Telephone medicines support

Provide a telephone help line for patients with questions, queries and concerns about their

medicines.

7. Management of medicines at change of care setting

Reconcile medicines following discharge from hospital or admission to intermediate care or care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in Care Homes).

8. Medicine information to practice staff and patients

Answer relevant medicine-related enquiries from GPs, other network staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggest and recommend solutions. Providing follow up for patients to monitor the effect of any changes.

9. Drug monitoring

Ensure robust systems are in place for drug monitoring at each practice across the PCN, streamlining these where possible. Understand and apply the traffic light classifications for prescribing in the Leeds Health Economy.

10. Signposting

Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.

11. Repeat prescribing

Ensure each practice in the PCN has a robust repeat prescribing policy, and streamline these across the PCN where possible. You may be asked to contribute to the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates. Ensure patients have appropriate monitoring in place when required.

12. Service development

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).

MAIN RESPONSIBILTITES

• To manage designated clinics in line with agreed clinic profiles, reducing overbooking and ensuring that patient numbers are adjusted to clinician availability.
• To liaise with the responsible clinician to provide regular updates regarding the status of their clinics profiles, in an agreed format.To use a variety of systems to monitor clinics and clinicians availability e.g PIMs
• To provide a high standard reception service and general administrative support to designated outpatient clinics, being a first point of contact to patients as required.To identify all patients AIS requirements and record the data accurately on the Trusts electronic system.
• Ensure clinic digital screens and whiteboards are updated with the correct and most recent information as required.
• Accurately record the patients attendance in clinic and time of departure.
• Record outpatients outcomes, ensuring that the each outpatient procedure is coded promptly for every patient as required.
• To book and reschedule appointments as required, including the booking of appointments for diagnostic purpose in liaison with the appropriate departments, including the booking centre and the call centre.
• Ensure follow up appointments are booked according to agreed Key Performance Indicators and in line with treatment protocols.
• To provide patients with up to date information relating to waiting times in clinicas required.
• Ensure all appointment cancellations and alternations are correctly recorded and highlight any issues identified with appointment bookings to clinicians working in accordance with the Trusts Access Policy.
• Ensure all clinics are cashed accurately and promptly on electronic administration systems within agreed Key Performance Indictor standards as required.
• Respond to appointment requests and other queries via email from clinicians promptly.
• Respond to appointment requests and other queries via email from patients promptly on a daily basis as per the rota.
• Ensure all referrals are scanned onto the system promptly and on a daily basis.
• Ensure waiting list is added and appointment booked through the waiting list.
• To liaise with the clinician and nursing teams working in clinics to keep them informed of any problems that may impact on the running of the clinic and ensure clinic capacity and resources are optimised.
• Ensure patient information is obtained and recorded on Trust electronic systems and that this checked regularly and updated electronically.
• Maintain a good knowledge of RTT and apply this to appointment bookings where appropriate

CLERICAL DUTIES

• To adhere to all agreed key Performance Indicators for data quality, providing exception reports where these are not met.
• Respond to telephone enquiries from patients and staff promptly and in a courteous and professional way, adhering to service guidelines andstandards. It is expected to deal with concerns as appropriate in order to maintain the efficiency of the service
• Respond accurately to patient queries regarding their treatment pathway, explaining in simple terms where required, referring to relevant colleagues where required.
• To book translators for patients where required and provide correspondence to patients in the most appropriate format in line with Accessible Information Standards (AIS).
• Provide administrative support for clinicians where required.
• Ensure letters, reports and results are scanned onto the system
• Assess patient eligibility for NHS treatment in accordance with Department of Health and Trust policies and procedures.
• Participate in the induction and training of new members of staff within the team, sharing knowledge and expertise of the administrative processes, functions and specialty specific knowledge.
• Participate in a rota system to provide cross-cover for other members of staff within the team to ensure consistent and effective service is maintained during periods of annual leave/sickness.
• Attend mandatory training required for administrative staff to ensure compliance with the Trust Mandatory Training Framework including participating in annual performance development reviews.
• Adhere to the Trusts values; the Moorfields Way at all times.
• To undertake any other duties designated by the Outpatient Supervisor or Assistant Clinic Manager which are commensurate with the grade.
• To actively participate in any training development identified.
