Main duties of the job
Permanent vacancy for 20 hours per week Working week is based on a two week rota:

Job summary

Care coordinators play a varied and important role within our practices. Their primary function is to work with along side and with our clinical triage team to coordinate the care for our practice patient population and ensure the right care at the right time.
Care coordinators review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.
This role is intended to become an integral part of the practices multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN. There may be a need to work remotely depending on the requirements of the role.
Please note that the role of a care coordinator is not a clinical role.
Main duties of the job
Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
Work collaboratively with GPs and other primary care professionals within the practice to proactively identify and manage a caseload, which may include patients with long-term health conditions, and where appropriate, refer back to other health professionals at our PCN or within our community service.
Work with people, their families, carers and healthcare team members to encourage effective help-seeking behaviours.
Support our digital team in developing communication channels between GPs, people and their families and carers and other agencies.
Maintain records of referrals and interventions to enable monitoring and evaluation of the service. Conduct follow-ups on communications from out of hospital and in-patient services.

We currently have an exciting opportunity for a Lead Biomedical Scientist to join our Microbiology team, supporting and serving our clinical partners throughout the South West Peninsula.

Within the General Surgical Unit (GSU) we are looking for an enthusiastic, motivated and experienced Senior Staff Nurse with a passion to provide excellent evidenced-based care.
The General Surgical Unit was the first ward within the trust to achieve the award of ward accreditation, which is a standard for consistently providing excellent an excellent standard of patient care and staff support.
The unit has 40-6 beds and cares for emergency General Surgical and now planned elective care. We also now have an enhanced care unit consisting of 4 beds for more complex surgical patients.
The Surgical Assessment Unit (SAU) is also part of GSU and provides emergency access for patients to the general surgical MDT team.
The unit takes referrals from GP surgeries, the ED department, outpatient clinics and direct from post-discharge patients.

Main duties of the job
We are looking for a Registered Nurse with vast surgical experience to for the post for senior staff nurse. Your leadership will contribute to a team mission to deliver person-centered care with kindness and respect, in line with our hospital values.

Main duties of the job

We are looking for highly motivated doctors to join our rapidly expanding medical team. Our big practice with a small friendly team approach is situated in purpose build premises in the pleasant residential areas of Guiseley and Yeadon in Leeds.
Guiseley and Yeadon Medical Practice has been working very closely with Aire Valley Surgery and this is a temporary collaborative arrangement whilst the GYMP practice continues to develop and continues to grow.
We are a training practice and participate in a range of enhanced services. Guiseley and Yeadon Medical Practice has a dedicated large team which consists of GPs including Registrars, A Physician Associate, Practice Pharmacist. We have a new Nursing Team and our attached staff in house include the Midwifes, First Contact Practitioners, Care Coordinator and we have two local care homes.

Tiptree Medical Centre are looking for an organised, confident and proactive Operations Supervisor to take responsibility for the smooth day-to-day running of our reception and administrative functions.
This is a varied, hands-on supervisory role at the heart of the practice. You will lead our reception and administration teams, coordinate clinical and non-clinical rotas, support HR processes and ensure that important operational systems and workflows run effectively.

Applications are invitied for the post of Locally Employed Doctor to work at Higher level in Thoracic Surgery. This post is fixed term until 3rd August 2027, to commence as soon as possible.
The LED higher is one of a team of Tier 2 doctors and Advanced Clinical Practitioners (ACPs) who deliver care to Thoracic Surgical patients. Although this Job does not have the deanery educational approval it will provide training opportunities at the same level as a registrar (StR3+) from the training programme sharing the same rota and the training opportunities. It will be suitable for a candidate with a dedicated interest in Thoracic Surgery or Cardiothoracic Trainees wishing to develop subspecialty skills in Thoracic Surgery or research.
Main duties of the job
The duties and responsibilities of the LED Higher can be balanced between service and training to support the successful applicant. All doctors will take part in a EWTD compliant non-resident on-call Rota (with prospective cover). The post can provide a period of dedicated training in Thoracic Surgery. A large number of Fellows who have previously taken up this post have managed to successfully apply for NTN Specialty Training and other pursued their career and successfully obtained consultant posts in the NHS including the thoracic surgery unit at UHB.

This is an exciting opportunity to join the Ear, Nose and Throat (ENT) team at Lincoln County Hospital. We are seeking a highly motivated and enthusiastic individual to contribute to the delivery and continued development of our ENT service.
We are looking to recruit an experienced Resident Doctor to join the ENT Department. The successful candidate will be expected to work at a level equivalent to a first-year ENT Registrar; therefore, previous experience within ENT is essential. While this post is aligned to an IMT3-level position, applications are welcomed from candidates with previous ENT Specialty Registrar (SpR) experience.
Although this is not a recognised training post, the successful applicant will be assigned an Educational Supervisor and supported in completing workplace-based assessments aligned to the ENT curriculum competencies. The post holder will be encouraged to participate in the Trust's educational and teaching programmes and will have opportunities for exposure to a range of ENT subspecialties, supporting professional development, career progression, and future applications to specialty training programmes.

Job summary
Custom House Surgery is seeking an enthusiastic and committed Salaried GP to join our friendly, supportive and forward-thinking team for 6 sessions per week. This is an opportunity to work within a well-established practice serving a diverse population in East London.

Job responsibilities
The Salaried GP will provide high-quality, patient-centred care to the practice population, delivering safe and effective clinical services across a range of settings. The post holder will work as part of a multidisciplinary team to manage acute and chronic medical conditions, promote health and wellbeing, contribute to service development and support the delivery of contractual requirements. The role includes routine and urgent clinical care, clinical administration, participation in the Duty GP rota and involvement in quality improvement activities.
Main Duties and Responsibilities:
Clinical Duties
- Deliver high-quality, patient-centred care through face-to-face, telephone and online consultations.
- Assess, diagnose and manage a wide range of acute, chronic and complex medical conditions.
- Develop and implement appropriate management and treatment plans.
- Prescribe safely and effectively in accordance with local and national prescribing guidance.
- Manage patients with long-term conditions in collaboration with the wider clinical team.
- Provide preventative healthcare advice and support health promotion initiatives.
- Undertake home visits where clinically appropriate.
- Provide care in accordance with GMC standards and evidence-based clinical guidelines.
- Maintain awareness of safeguarding responsibilities and act in accordance with local policies.
- Review and action pathology results in a timely manner.
- Process incoming clinical correspondence, hospital letters and discharge summaries.
- Review and authorise repeat medication requests where appropriate.
