This role is fixed term for 12 months (Maternity cover)
Windsor House Group Practice is a large GP Practice with 3 x surgeries based in Leeds (Morley, Churwell and Drighlington) with a list size of over 20,000 patients.
We have an exciting opportunity for an enthusiastic person to join our clinical team as a Healthcare Assistant.
The role is very hands on and requires a high degree of accuracy, confidentiality, and ability to perform all the task require, which includes bending, lifting and moving.
Please see the job role for details on what the role of HealthCare Assistant within a GP Surgery is expected to do.
Hours of work required are approximately 20 hours over 3 days (Wednesday, Thursday and Friday).

About us
We are a very friendly and forward thinking Practice within the Leeds area with a list size of approximately 20000 patients.
Our multi-disciplinary team includes 12 GP's, 10 registrars, 2 Clinical Pharmacists and Pharmacists Technicians, Practice Nurses, HCAs all supported by a strong administration and reception team(s). Our aim is to deliver good quality healthcare to our patients.

Job summary
To provide nursing support to the Practice Nurse and General Practitioners to ensure the efficient and effective provision of the full range of nursing services to the patients of the practices with appropriate training. To work across all sites both in the North and South as required.

Midlands Medical Partnership is a Birmingham super-partnership currently caring for 72,000 Birmingham Cross-City CCG patients across North and South Birmingham operating out of 11 sites. To undertake all duties in line with the training and instruction provided by the Practice Nurse and General Practitioners.
Conduct weight and height measurements, test and record pulse, blood pressure and blood sugar levels.
Test and record results of clinical urine specimens.
Venepuncture and the taking and preparing of pathology samples.
Domiciliary Visits if required for taking blood tests Bps etc;
Carry out routine patient dressings in accordance with practice guidelines after adequate training.
Perform ECGs on patients as requested by GPs.
Carry out various Health Checks to include NHS Health Checks, Pre-Diabetes Checks, Learning Disability Health Checks etc.
Assist with the regular monitoring of Diabetes patients, to include foot checks
Offer lifestyle advice to patients when advisable such as diet, exercise and alcohol advice with a view to health promotion
The collection of clinical waste ensuring appropriate storage prior to disposal.

We are looking to recruit a motivated, compassionate and enthusiastic Healthcare Assistant / Care Co-ordinator to join our friendly, award-winning multi-site GP practice team.

The LLS has been successful in becoming a National Wound Care Strategy Programme (NWCSP) Implementation Site 1st tranche for the NWCSP Lower Limb Recommendations. The service has also recently been shortlisted for a HSJ award.
You will need to demonstrate a strong interest/experience in Tissue Viability & lower limb management in order to provide specialist care, in all aspects of lower limb health & skin care. Working clinically under the guidance of Registered Nurses and Nurse Associates. Effective communication & teamwork are key. Health promotion/patient education is an essential part of this role, as is collaboration with other allied healthcare professionals as Marjon Universitys Sports Therapists deliver our Wellbeing programme.
The DN Treatment Clinics provide care to non-housebound patients that is unavailable at their GP surgeries: , urinary catheter management, nephrostomy care & Topical Negative Pressure Therapy wound dressings.
Daily duties require periods of prolonged bandaging & the work is physically demanding. You will be required to work in a variety of treatment clinic settings across the Livewell footprint,
Main duties of the job
As a member of the DN Treatment Clinics/Wellbeing Lower Limb Service you may be required to work independently during your working day.
The post holder will demonstrate a history of interest in lower limb management and Tissue Viability.
The post holder is supported by the Community Team Manager and Specialist Service Lead and is responsible for supporting and collecting information for the holistic assessment and management of Lower Limb disease. To be highly skilled in compression bandage application techniques and wound management. Underpinning aetiology and management of patients with leg ulcers. The post holder will be required to make contemporaneous recommendations for the management of lower limb conditions and implementation of an appropriate care plans, know when to esclate to a senior member of the team. Demonstrating the principles of wound healing and management, the aim of which is to enable patients to enjoy maximum independence and optimum quality of life.

Main duties of the job
This is a hands-on, patient-facing clinical support role working closely with the nursing and wider multidisciplinary team to promote health, prevent illness, and support the ongoing management of patients across the practice.

The Healthcare Assistant post is an integral part of the Multi Disciplinary Team on the Ward that provides both physical, social and psychological care to the patients that are admitted to Harford Ward. We are looking for the right person with an aspiration to provide compassionate care and support to people admitted to the Unit who are acutely mentally unwell and in crisis. The Glenbourne Unit is an acute mental health inpatient service provided by Livewell Southwest. The Unit consist of one female 19 bedded acute ward which is Bridford Ward and a male 19 bedded acute ward which is Harford Ward. We are currently rated outstanding by the CQC with the ward providing a wide range of support and treatment available to those experiencing deteriorating mental health; to support and enable their recovery. The Unit is an integral element of our Acute Care Pathway with links with the citywide Home Treatment Team and Community Mental Health Services.
Main duties of the job
The role of the Nursing Assistant is to assist the trained nurse in delivering physical, social and psychological care to the patient / client with mental health problems / illness, admitted to the Unit.
The post holder will be required to work both within Glenbourne and with clients in the community. This will involve contact with relatives, carers and other agencies. They will carry out interventions as part of the care plans and record information using our electronic patients notes.
The post holder will be directed to undertake tasks once adequate instruction has been given by the trained nurse and both parties are confident that the task can be fulfilled competently. However, the trained nurse is required to carry the responsibility for decision making and direction.

We are looking for an enthusiastic ACP to join our friendly, forward thinking team. We are a 6 partner practice with 8 regular GPs supported by our friendly group of Nurses and HCAs.
This position is open to Acute Nurse Practitioners / Paramedic Practitioners, or Advanced Clinical Practitioners.
The non-clinical team at our practice is highly skilled and very efficient, having a fluid working system with our clinicians. We also benefit from having a First Contact Physio, 2 Clinical Pharmacists, 2 Pharmacy Technicians, 3 Social Prescribers, 2 Health and Wellbeing Coaches, a Physician Associate and 2 Mental Health Workers on hand to help with the workload.
Main duties of the job
Beyond the expected assessment, treatment and collaboration as required, you will sometimes need to refer patients for investigations and/or the secondary care input, you will ensure the provision of continuity of care including follow-up if required. You will contribute to the writing of Care Plans for those patients at risk of hospital admission where necessary.

- Ensure the secure handling of drugs and clinical equipment recording usage appropriately.
- Complete vehicle logs, reporting any discrepancies to the lead driver
- Report any vehicle issues to the lead driver for investigation and resolution.
- Support the treating clinician with chaperoning
- Support the treating clinicians if they need support
