Job summary
We are looking for an experienced and compassionate Practice Nurse to join our well-established rural dispensing GP practice for 16 hours per week, working Tuesdays and Fridays.
This is an excellent opportunity for an experienced General Practice Nurse to become part of our close-knit team, where continuity of care, teamwork and building strong relationships with our patients are at the heart of what we do.
The successful candidate will be confident working independently within a General Practice setting and will have previous experience delivering a broad range of Practice Nursing services. The role will include treatment room nursing, Long-Term Condition management, health promotion, preventative care, vaccinations and screening, alongside contributing to QOF and Enhanced Services.
You will work closely with our GPs, HCA and wider practice team to provide safe, effective and evidence-based care to our patients.
We are looking for someone who is caring, motivated and professional, with the clinical experience and confidence to manage a varied Practice Nurse workload.
Previous experience working as a Practice Nurse within General Practice is essential. This role is not suitable for newly qualified nurses or applicants without previous General Practice experience.

Main duties of the job
As our Practice Nurse, you will provide a high standard of clinical care to patients across treatment room and Long-Term Condition services. You will work confidently and independently within your scope of practice, using your previous General Practice experience to deliver safe, effective and patient-centred care.
Your main duties will include:
- Delivering nurse-led clinics for patients with Long-Term Conditions (LTCs), including annual reviews and supporting QOF delivery.
- Providing treatment room services, including wound care, dressings, venepuncture, ECGs, blood pressure monitoring, injections and specimen collection.
- Administering childhood, adult and seasonal vaccinations and immunisations, and undertaking cervical screening where qualified.
- Promoting healthy lifestyles through patient education, NHS Health Checks and preventative care.
- Maintaining accurate clinical records using SystmOne, including appropriate clinical coding.
- Supporting infection prevention and control, maintaining clinical equipment and treatment room stock, and contributing to clinical audit and service improvement.
- Recognising and escalating clinical concerns appropriately and maintaining high standards of patient safety and clinical governance.

Our Team Hibaldstow Medical Practice is a small, friendly rural dispensing practice serving approximately 3,500 patients across a wide geographical area of around 120 square miles. Our close-knit team includes two GPs, two practice nurses, a nurse associate, a healthcare assistant, eight dispensers, four receptionists, two administrators and a practice manager. We work collaboratively to provide high-quality, patient-focused care to our local community.

Cheltenham Peripheral Primary Care Network is a group of 5 Practices situated around the edge of Cheltenham. We collaborate as a Network as we truly believe by working together we can achieve better and greater care for our patients in the communities.
We have an established Frailty Team for the Cheltenham Peripheral PCN which is a multi-disciplinary team dedicated to supporting patients who are frail, elderly or vulnerable and living in the community. The team has been created specifically by the Cheltenham Peripheral Network to recognise the needs of the local population and the scope for transforming services to provide personalised care.
The role of the frailty nurse supports CP PCNs plan to reduce the number of frail older people who have a lack of support, who want to continue living in their own homes and require access to some additional care. Cheltenham Peripheral want to establish a standardised approach to supporting frail patients in the community. The need for this has arisen because routine and unscheduled care has not changed in line with the needs of an ageing population. Older people can live with several health conditions and a gradual decline may be missed until a crisis occurs preventative work can help to avoid the person reaching a crisis.
Main duties of the job
The Frailty Nurse will work alongside our established Frailty team in developing close relationships with our five GP practices, supporting them to:
Establish systems and co-ordinate patient access to services.
Delivering the Gloucestershire Model of Frailty.
Enable smooth and planned transfer between care settings.
Provide advice and information.
Oversee safeguarding administration and communication with partner organisations where required.
Ensure personalised health and care planning is proactive, supportive and patient-centred.

The Leckhampton Surgery, Stoke Road Surgery, Sixways Clinic, Winchcombe Medical Centre and Cleevelands Medical Centre work together as the Cheltenham Peripheral Primary Care Network. Penrhyn Bay & Deganwy Medical Centre is a friendly and highly regarded practice with a list size of 6500 patients, situated on the North Wales coast close to Llandudno.
Due to the additional capacity funds and local housing developments, we are looking for a GP to join our practice as a GP Partner or salaried GP with a view to partnership in the future.

The administrator role is varied and rewarding supporting senior management and educators with projects and programs. In addition there are the more traditional administrative duties such as minute taking and diary management.

Clinical responsibilities

Main duties of the job

Main duties of the job
Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). The clinical team includes GP's, Practice Nurse, Pharmacist, Physician Associate and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.

An exciting opportunity has arisen to join Severnvale Primary Care Network as our PCN Manager on a 13-month maternity cover contract, working 22.5-30 hours per week. We are happy to discuss the exact hours with the successful candidate.

Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team in general practice.
