Main duties of the job
Providing comprehensive clinical assessments and management for patients requiring urgent care outside of normal surgery hours across base working and home visitations
Managing a varied caseload including acute minor illnesses, injuries, and chronic condition exacerbations.
Utilizing the Adastra clinical system to accurately record consultations and clinical notes.
Working collaboratively with healthcare colleagues, including nurses, paramedics, and administrative staff, to ensure continuity and quality of patient care.
Making appropriate referrals, liaising with secondary care services, and ensuring patient safety through effective clinical decision-making.
Participating in clinical audits, governance activities, and continuous professional development.
About us
Fully qualified General Practitioner with GMC registration and relevant UK licence to practice.
Previous experience working in an Out of Hours setting is essential.
Proven experience using the Adastra clinical system for patient record management.
Strong clinical assessment skills and the ability to manage a diverse and often complex caseload.
Excellent communication and interpersonal skills with the ability to work both independently and as part of a team.
Commitment to providing compassionate, patient-centred care aligned with Practice Plus Groups values.
Flexibility with working hours, including evenings, weekends, and bank holidays.

Main duties of the job
Mowbray Square PCN, together with Tees Esk and Wear Valleys NHS Foundation Trust, are working together to transform local community mental health services for people with mental health problems. New funding has enabled the PCN and local GP surgeries, to create a multi-disciplinary team that will work in close collaboration with a wide range of organisations.
The postholder joins the existing First Contact Mental Health Practitioner that has been extremely successful with good outcomes for patients and the PCN. About us
Stratton Medical Centre is a busy town practice on the outskirts of Bude. We have over 10,700 patients who are looked after by an integrated team of 9 GPs, 2 Nurse Practitioners, a Paramedic, 2 Nurses, 3 Health Care Assistants and a Phlebotomist.
As part of a wider Primary Care Network (PCN), our GP team also benefit from a clinical pharmacist, a home visiting paramedic, a social prescriber and health and wellbeing coach. You must be able to prioritise workload without supervision and have good organisational and IT skills. Provide secretarial support to GP's and clinicians. You will be required to help out on the telephones as and when required. Answer telephone calls, respond to all enquiries, direct the public as and when appropriate, provide admin support to nursing & clinical staff. Full training will be provided on the use of SystmOne.

Main duties of the job
- Answer telephone calls & respond to all enquiries directing the public as appropriate
- Provide broad range of tasks and general admin and secretarial support to practice staff
- Ensure timely appointments for patients attending the GP service, triaging according to urgency
- Process patient referrals
- Ensure all Emergency Care documentation is coded & filed correctly, and copies are sent to relevant health care professionals
- Receive confidential information by telephone or face to face and input accurately into patient service systems
- Admin coding of all patient records ensuring all medical history is coded accurately and entered into the clinical system
- Contribute to the delivery of a comprehensive primary care medicine management system
- Undertake activities at practice level which contribute to the efficient and effective delivery of medicines to patients
About us
Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 120,000 registered patients in Northumberland and North Tyneside.
Whilst appointees will have a base practice within NPC,they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care.
