Job summary
Band 5 Staff Nurse Short Term Care Beds Working as a Staff Nurse for Leeds Community Healthcare NHS Trust within the Short Term Care Beds service is very different from many traditional nursing roles. We work as a community within a community, providing person-centred care and rehabilitation to support patients on their recovery journey. The Short Term Care Beds service provides care for patients who require a short-term stay following discharge from hospital. While returning home is the preferred outcome for many patients, some require a temporary stay in a supportive, homely environment where 24-hour care is available. The service supports patients to regain independence, achieve their rehabilitation goals, and plan for a safe discharge either to their own home or, where appropriate, to a nursing or residential care setting. Our aim is to work in partnership with patients and their families to help them achieve their recovery goals. This is delivered through a multidisciplinary team approach, with support from Registered Nurses, Health Care Support Workers, Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dietitians, and General Practitioners (GPs). The Short Term Care Beds service operates 24 hours a day, 365 days a year, and you will be required to support patients across a variety of shift patterns to ensure the continuous delivery of high-quality care and rehabilitation services.

This role is suitable for pharmacists whose qualifications, experience and competencies are broadly equivalent to NHS Agenda for Change Band 6 or Band 7 roles. We are a supportive, open-minded practice with experienced GP Partners, Advanced Nurse Practitioners and Practice nurses. We have an open door policy so you are never on your own!
We follow the BMA contract with 15 minute appointments, with an example session of 10 face to face appointments, 2 telephone calls and SDL time inbuilt into the weekly timetable.
We are a training practice so we are able to support a newly qualified GP and also welcome experienced GPs.
We have a stable practice population of circa. 10500 patients looked after by a team of 4 partners, 2 salaried GPs, 3 ANPs, 3 practice nurses and 2 HCAs who are all supported by a hard-working and supportive reception and admin team. We are also very fortunate to work in an Integrated Care Centre housing the Community Nursing team, Same Day Emergency Care, Minor Injuries and Outreach Visiting Practitioners.
We have a total triage system where the on call GP will see all requests to the practice and direct them towards an appointment, redirect to appropriate service, or call the patient directly. We all share the on call which is restricted to morning or afternoon session. You are fully supported while on call.

Job summary
We are currently seeking to recruit a Care Coordinator on a part-time basis, 21.5 hours per week on a permanent basis, to provide a comprehensive reception service to achieve an efficient and effective service in providing the best possible care to patients.
The post holder will be required to work flexibly across all SGPA practices as required. As well as greeting patients and carrying out general reception duties you will also be expected to process prescriptions and fully utilise EMISWeb and other IT systems.
Applicants must have experience of working in a NHS setting and have a working knowledge of medical reception duties.

The SARC Clinical Lead will provide clinical leadership and governance across Sexual Offence Examiner services, ensuring the delivery of safe, high-quality and nationally compliant forensic care.
Acting as Medical Lead Doctor, the post holder will support service development, clinical oversight and governance activity in collaboration with the Clinical Service Manager, contributing to continuous improvement and the professional development of Sexual Offence Examiners and Crisis Support Workers.
The post holder will support the provision of clinical leadership to the SARC team and provide leadership and mentorship to Sexual Offence Examiners and Doctors. This includes delivering induction, training and supervision for trainee Sexual Offence Examiners, including shadowing, competency assessment, case reviews and final sign-off. The SARC Clinical Lead will provide clinical leadership and governance across Sexual Offence Examiner services, ensuring the delivery of safe, high-quality and nationally compliant forensic care.
Acting as Medical Lead Doctor, the post holder will support service development, clinical oversight and governance activity in collaboration with the Clinical Service Manager, contributing to continuous improvement and the professional development of Sexual Offence Examiners and Crisis Support Workers.

Main duties of the job

We are seeking a dedicated and compassionate Salaried GP to join our Practice for a 2 session post on Fridays.
Main duties of the job

We are looking for a 2 Session salaried GP to work Fridays at Homestead.
We are a PMS practice with a list size of 8,900. You will be joining our fantastic clinical team of 3 Partners, 1 Salaried GP, 1 Diabetic 1 Practice Nurse and a Phlebotomist. Main duties of the job
As a Practice Nurse at Grove Medical Centre, you will provide high-quality, patient-centred nursing care across a range of clinical services within a busy and supportive primary care environment. The role offers a varied workload, including the management of long-term conditions, health promotion, cervical screening, immunisations, wound care and preventative healthcare.
You will work closely with GPs and the wider multidisciplinary team to support patients in managing their health, promote wellbeing and deliver evidence-based care.
