Job summary

We have 4-6 clinical sessions available. For the right candidate there would be some flexibility and we would be willing to wait.
Main duties of the job
A trained General Practitioner, willing to use the online consultation software, SystmConnect on a daily basis and SystmOne
As we are a teaching practice, provide as part of a team training, teaching and supervision of other healthcare professionals
Home Visits as part of a rota
Ensure own clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations, arranging referral to other members of the primary and secondary health care team if necessary.
We are not looking to fulfil any areas of specialty. However there will be opportunity in the future to develop.
About us
We are a semi rural practice situated 3 miles north of Lincoln and currently has approximately 12,200 patients. There are two sites, with Nettleham being the main site and a branch surgery at Cherry Willingham.
We have a extensive multi-disciplinary primary care team comprising of GPs, Physician Assistant, Nurses, Healthcare Assistants, Phlebotomists, Clinical Pharmacist, First Contact Physiotherapist, Social Prescribers and Primary Care Chaplains.

Job description
Job responsibilities
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the high quality medical services to all patients in accordance with the practices procedures and protocols.
Will be part of a multidisciplinary team providing a full range of general medical services to patients. In addition to providing primary care services, the post holder will be expected to be involved in liaison with other agencies.
Clinical responsibilities:

- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)

We are looking to recruit a full time Care Navigator/Administrator to join our dedicated team. 37 hours per week, Mon-Fri with a finish time of 18:30 and this is non-negotiable.

DRIVER
To primarily transport clinicians to the homes of patients in order to provide medical assistance. The post holder will also be responsible for the maintenance of the vehicle during each shift. The Community Minor Injury Units are based in Abingdon on Thames, Witney, and Henley on Thames, their is also a First Aid Unit in Bicester. The MIUs see approximately 4000 patients across the service per month, making a real difference, providing care closer to home and avoiding admission to hospital wherever possible and appropriate.
The Minor Injury units are open 7 days a week, 365 days per year. We are happy to consider full or part time hours.
The GP OoH services runs when the GP surgeries are closed and manages referrals from 111. The role includes learning about telephone triage, supporting base visits and home visiting.
The role offers excellent opportunities in career development in injury and illness management at an Advanced level accredited through West London University. Study is based in Oxford.
The ability to travel independently between sites within the Trust is essential for this role.
Due to internal promotion we are recruiting in our Witney base to support and provide care in the community.
The role is based in Witney, however on occasion you may be asked to move to another base to support service delivery.
Main duties of the job

Reporting to the Hospice at Home Manager, you will play a key role in delivering high-quality specialist palliative care within patients' own homes. We are seeking a reliable and detail-oriented Medical Laboratory Assistant to support our UKAS accredited department by carrying out a range of routine pre- and post-analytical tasks across various areas including the urgent bench, referrals, miscellaneous, Histology, and routine blood sciences.Full training will be provided, so no degree is required. The ideal candidate will have demonstrable experience in a customer service environment, be computer literate with good keyboard skills, and have strong attention to detail. Main duties of the job
The post holder will perform a range of routine and non-routine tasks within Pathology including, numbering up clinical samples, liaising with external laboratories to obtain outstanding referral results, entering patient results into the Laboratory system, advising patients and service users on appropriate tests, dealing with private patients invoicing and dealing with the sensitive disposal of foetal remains.

We are a unique dispensing practice in a picturesque Cotswold town with excellent railway connections to both Oxford and London. Attractive Cotswold Stone single site premises with good on-site parking. Easy access to Charlbury Station, which is only a 15 minute journey from Oxford Station. About us
The Charlbury Medical Centre provides GP services to 5,489 patients in a rural area of the Cotswolds, in the small picturesque market town of Charlbury. Job summary
Woodlands Medical Practice are seeking an experienced and highly organised Medical Secretary to join our friendly and busy GP practice.
The successful candidate will work 16 hours per week providing efficient administrative and secretarial support to the clinical team, ensuring that referrals, correspondence and patient information are processed accurately, promptly and confidentially.
