Role Overview
• Position: Family Medicine Physician
• Location: Dubai, United Arab Emirates

Key Responsibilities – Family Medicine
• Provide comprehensive primary healthcare services to patients across all age groups, ensuring continuity of care and long-term patient management.
• Conduct detailed clinical assessments, medical examinations, and diagnostic evaluations to diagnose and treat a wide range of acute and chronic medical conditions.
• Manage chronic diseases such as diabetes, hypertension, cardiovascular conditions, respiratory disorders, thyroid disease, and metabolic disorders in accordance with evidence-based clinical guidelines.
• Deliver preventive healthcare services, including routine health screenings, immunisations, lifestyle counselling, and early disease detection initiatives.
• Develop and implement individualised treatment and care plans, ensuring appropriate follow-up and monitoring of patient outcomes.
• Provide patient education and counselling on health maintenance, disease prevention, nutrition, and lifestyle modification.
• Coordinate referrals to specialist physicians and allied healthcare professionals when required and ensure effective follow-up care.
• Maintain accurate and comprehensive electronic medical records and clinical documentation in compliance with regulatory and organisational standards.

Clinical Responsibilities:
In accordance with the practice timetable, the GP will work autonomously, delivering care within their scope of practice to the patient population. Contributing towards clinical areas such as health promotion, chronic disease management, health prevention, well-women and well man clinics as well as supporting the management team in the reviewing of clinical policy and procedure.
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Provide wound care (ulcer /Doppler etc.) to patients
- Provide specialist clinics such as ear syringing, contraception etc.
- Undertake the collection of pathological specimens
- Provide travel medicine services
- Request pathology services as necessary

Job summary
NWL has the largest homeless population in London. This population faces fundamental inequalities in outcomes, experience, and access - with multiple, long term, under-treated health needs and do not always get the care they need from the health and care system.
People experiencing homelessness have six times more A&E presentations, are admitted to hospital four times more often, and stay three times as long as the general population.
In hospital, they are likely to be poorly identified, experience delays once medically fit for discharge, and are at high risk of discharge to the street, readmission and poor overall health outcomes.
We have received confirmation from the NWL ICB that a business case has been approved to fund a new specialist hospital Homeless Health team up until 31st March 2029.
The Homeless Health team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge - ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.
The GP role works to improve the care for homeless patients in hospital, to drive up the quality of the care they receive during a hospital stay, and to improve their quality-of-life following discharge.
Main duties of the job
Working with the rest of the Homeless Health team, the GP role works to improve the care for homeless patients in hospital, to drive up the quality of the care they receive during a hospital stay, and to improve their quality-of-life following discharge. The key responsibilities of the role are to:
To clinically lead, support and promote the specialist homelessness team in the hospital.
To work collaboratively to improve health and social outcomes for people who are homeless.
To promote and lead weekly multi -disciplinary team meetings for hospital, primary care and voluntary sector staff working with the most challenging patients.
To offer clinical judgment/expertise on how best to address complex issues among this patient group, particularly with reference to the tri -morbidity of substance misuse, mental illness and physical health problems.
The successful post holder will have substantial personal experience of working with people who are homeless and will have a demonstrable special interest in:
Working with people experiencing drug or alcohol dependence or/and domestic violence including substitute prescribing.
Experience of working in or with the wider statutory and charitable homelessness sector.
- Experience of mental illness particularly working with patients with complex psychological trauma/diagnosed with personality disorder
- Sexual health.

Job summary
We at Linden Hall Surgery are looking to recruit a permanent salaried GP for 6 sessions per week.
This is an excellent role for both established and newly qualified GPs in a friendly and diverse practice with a focus on work-life balance.
Our ideal candidate is positive with a good sense of humour, fantastic communication skills and keen to really integrate with the current team. We are also able to sponsor Tier 2 applicants.
Main duties of the job
The practice is split between two surgeries, one in Newport and one in Muxton, which has recently been extended and updated.
The working day is primarily face-to-face consultations, both prebooked and on the day, with clinician autonomy on the way their day is structured. A session consists of 13x 15 minute appointments.
We are a total triage practice, utilising GP triage for all requests on the AccuRx platform, supported by an excellent triage support team within a dedicated triage hub. This has given us fantastic responsiveness to clinical need, with patients seeing the right person at the right time. 60% of patients are seen on the day, and all within 2 weeks.
We utilise EMIS, Docman 10 and AccuRx, with an emphasis on fair and reasonable admin requirements. Admin is supported by a team of GP assistants and an excellent secretarial and admin team.
We have a low burden of home visits and you are not required to see any extras when working a routine session.
The role will consist of one duty triage session per week, with occasional annual leave cover.
The role of GP is well supported with additional healthcare staff including clinical pharmacist, first contact physiotherapist, advanced nurse practitioner, paramedics, social prescribers, mental health practitioner, and a full team of nurses and healthcare assistants.
We are a training practice, with three trainers, one of whom is also a TPD. All GP staff are involved in supervision of trainees with allocated time to do so.
About us
Linden Hall Surgery is based across two sites in the Shropshire market town of Newport, and the smaller suburb of Muxton, both north of Telford. The surgery currently has a a list size of 16500 patients and continues to grow.
The surgery consists of a close-knit team of 4 partners and 6 salaried GPs, alongside an expanding team of PCN ARRS members currently including a First Contact Physiotherapist, Primary Care Paramedics, Clinical Pharmacist, Pharmacy Technicians, Social Prescriber, Mental Health Practitioner, and Age UK Link Worker.
The surgery is a training practice with three trainers and regular GP VTS and FY2 placements and is highly regarded in this capacity. We always welcome new ideas and innovation, and support all doctors to be involved with practice development if they wish.
Whole surgery social activities are regularly arranged in addition to clinical meetings.

Job summary
Release Engagement Practitioner: This role requires supporting people primarily in the area of East Lancashire, though will require cover across other areas of Lancashire as required by the service.
Essential: Diploma in Health and/or Social Care or equivalent experienceLevel 3 NVQ in health related subjectDesirable: Foundation degree within a relevant field e.g. health, psychology, criminology etc (or equivalent qualification/ reflected experience).
The post holder will work within the Reconnect Care after Custody service for Lancashire and Cumbria, covering 6 prisons and the wider community of Lancashire and Cumbria. Reconnect is a triage, assessment, referral and support service and starts working with people before they leave prison, where they will have access to a Reconnect practitioners and support practitioners, helping them to make the transition to community-based services that will provide the health and care support that they need. It is offered to people in prison up to 3 months prior to release, and up to 6 months support post release.
Main duties of the job
Reconnect will create an effective link between prison & community in order to:
- increase access & uptake up of healthcare or relevant support services for vulnerable individuals who would otherwise struggle to engage
- reduce health inequalities for prison leavers
- ensure the health needs of individuals who are leaving prison are met
- ensure a safe transition from prison to community-based healthcare & support services & to provide follow-up to ensure engagement is maintained
The post holder will actively engage and work with a number of individuals who are likely to have a wide range of vulnerabilities and complex needs, including homelessness, substance misuse problems, mental health, significant risk histories and social exclusion, and who at times may present with extremely challenging behaviours. They will support people to access services for the full scope of vulnerabilities as identified
Please note that hybrid working is necessary due to the area of coverage - involving some working from home and utilising other trust sites as a bases as required
Vetting and Clearances:
o LSCFT staff entering the prison and probation estate will be subject to HMPPS security vetting carried out by National Offender Management Service and will be required to undergo Enhanced Level 1 vetting.o The post holder is also expected to be have Enhanced DBS status.

An exciting opportunity has arisen for Band 6 practitioners to work within our established Urgent Community Response Team in West Essex.
The teams assess and manage acutely unwell patients at home using a variety of pathways including respiratory, heart failure and frailty, to prevent unnecessary admissions to hospital.
This includes the utilisation of remote technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier than may have been possible previously. The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Hospital at Home and UCRT pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of West Essex and deliver an NHS national priority.

We are pleased to be advertising for a Band 4 Associate Practitioner to work with us in our Enhanced Recovery and Support at Home Team. You will be working as part of a team consisting of Physiotherapy, Occupational Therapy, Nurses and Rehab Support workers assistants.
The service operates 8am-6pm 7 days a week and you will be expected to work on a rota basis to cover these hours. The team has a clinical supervision structure and there is access to comprehensive in-service training.
With a particular focus on rehab, falls and provision of equipment for patients at home, you will have experience in working autonomously as delegated by the OTs and Physios, be able to make your own assessments of patients' risks and needs, and undertake rehab treatment programmes.
You will receive direct line management from a registered therapist who will support you to develop within the role, both in terms of confidence as well as competence, and access further training and development opportunities to help you expand your skills and knowledge as appropriate.

About us:
Haslington Surgery is a two Partner practice, located in a small village in Cheshire. We have a clinical team including two Salaried GPs, four Nurses, one GPA, two Care Coordinators, Clinical Pharmacist, Pharmacy Technician and Social Prescriber. This is supported by a Practice Manager working with a small administration and reception team. We form part of the local SMASH PCN group of Surgeries, working together to focus on local patient care and to deliver local services.
Together we are a friendly team who work closely and meet on a regular basis to promote continuous learning and professional development for all.
We are supportive of professional development work in a supportive and friendly environment.
The Surgery delivers services to 6200 patients, including a Nursing Home. We operate from a premises that is currently being reviewed for extension, providing additional clinical rooms and meeting space.
We have an active PPG, meeting every other Month, and we deliver regular Wellbeing events, at the local village hall, focusing on patient engagement and promoting patient healthcare.

Job responsibilities
The following are the core responsibilities of the Locum GP in delivering health services.
- Work as agreed in accordance with the practice timetable
- The delivery of highly effective medical care to the entitled population
- The provision of services commensurate with the primary care contract
- Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly, referring patients to other services/agencies in a timely manner, utilising practice and any other local guidelines
- Generic prescribing adhering to local and national guidance
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Effective management of long-term conditions
- Support patients to adopt strategies that promote healthy lifestyles and apply principles of self-care
- Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks
- Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways
- Maintain accurate clinical records in conjunction with good practice, policy and guidance
- On a rotational basis, undertake telephone triage and duty doctor roles Work collaboratively, accepting an equal share of the practice workload
- Adhere to best practice recommended through clinical guidelines and the audit process
- Support the delivery of anticipatory care plans
