Main duties of the job

practice population.
Deliver

Blackhall and Peterlee Practice is a
well-established, patient-focused GP practice located in the Easington area in
County Durham. Rated 'Good' by the CQC, we provide high-quality primary care
services, operating across two locations, caring for 10,000+ patients.
We are a large training practice with
four GP partners, four salaried GPs, a nursing team of nurse practitioners,
nurses and HCAs, pharmacists, social prescriber, care coordinator, and health
and well-being coach, along with a large administrative team to support the team
and patients. We provide education and supervision for GP trainees and medical
students.
We utilise Systmone clinical system and make full use
of modern digital solutions including Accurx, eConsult, Ardens, Patient
partner and Agilio TeamNet to support efficient patient care and workflow
management.
As part of Easington Central PCN we
offer enhanced access and attached staff including physiotherapist, adult
mental health support and CAMHS to support efficient patient care and workflow

The post-holder is a Receptionist/Administrator
The Receptionist will be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. - The following are the core responsibilities of a Receptionist in delivering health services. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Job summary
An exciting opportunity has arisen to join our multidisciplinary team as a Primary Care Network Pharmacy Technician. Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and members of our Practice and PCN multidisciplinary teams.

Main duties of the job
The post holder will work autonomously within their professional scope, using professional judgement and accessing clinical supervision when required.
They will provide pharmaceutical technical support to the medicines optimisation team and programme, undertaking medicines reconciliation, supporting medication reviews, checking medicine administration and promoting effective use of medicines, including inhaler technique. They will support clinical pharmacists with Structured Medication Reviews and help patients get the best from their medicines.
The post holder will improve prescribing quality, safety and cost-effectiveness by supporting agreed medication switches in line with approved protocols, improving repeat prescribing, promoting repeat dispensing and online ordering, reducing clinical risk and minimising medicines waste.
They will undertake clinical audits across the PCN, analyse findings and provide reports to GP practices to highlight progress, identify improvements and support appropriate actions.
The role includes patient-facing work to promote safe and effective medicines use, with referral to clinical colleagues when required. The post holder will support public health and social care needs through lifestyle advice, signposting and addressing local health inequalities.
Overall, the role contributes to safer, effective and cost-effective medicines use, improved prescribing and better patient outcomes across the PCN.

Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.

The Health Care Coordinator contributes to the delivery of the Primary Care Network (PCN) Direct Enhanced Service (DES), supporting proactive, personalised care and population health management for individuals with complex needs, frailty, long-term conditions, and those at risk of unplanned admission.
The role supports the delivery of:
- Enhanced Health in Care Homes EHCH service specification
- Anticipatory care for high-risk cohorts
- Personalised Care and Support Planning PCSP
- Multidisciplinary team MDT working across primary, community, social care, and voluntary sectors
This role supports reducing health inequalities through proactive case management, care coordination, and structured clinical support within HCA/Nurse Associate competency and delegation frameworks.
The post holder works under the supervision of registered clinicians and is central to coordinating care across PCN multidisciplinary services, ensuring patients receive timely, coordinated, and person-centred care aligned to PCN DES and member practice requirements.
Clinical Governance & Delegation
The post holder undertakes delegated clinical activity in line with NHS England HCA/Nurse Associate competencies and PCN clinical governance arrangements, including:
- Basic clinical observations and structured monitoring

This role is suitable for both experienced ACPs and those who have recently qualified.
The successful candidate will deliver high-quality, patient-centred care, working autonomously to assess, diagnose, treat, and manage a broad range of acute, minor, and long-term conditions. You will manage patients presenting with undifferentiated, undiagnosed issues carrying out full clinical assessments, initiating treatment plans, prescribing, and evaluating care in line with national guidelines.

Main duties of the job
Provide expert clinical assessment and diagnosis for patients presenting with undifferentiated, undiagnosed, and complex health problems.
Initiate, evaluate, and review treatment plans, including prescribing medications and therapies within your scope of practice.
Manage a range of acute, minor, and long-term conditions independently, referring to or liaising with GPs or secondary care where appropriate.
Undertake comprehensive physical assessments, using advanced clinical skills and decision-making.
Perform and interpret relevant investigations, such as blood tests, ECGs, and other diagnostic procedures.
Promote preventative healthcare and offer patient education, self-care advice, and health promotion.
Work collaboratively with the multidisciplinary team to ensure high standards of care, continuity, and efficiency.
Contribute to clinical governance, audits, significant event analysis, and the development of local policies and protocols.
Participate in service development and innovation, supporting quality improvement initiatives across the practice.

We are a practice of around 6200 patients, with 2 GP Partners, 2 Salaried GPs, 3 Practice Nurses and a small team of Healthcare Assistants and Phlebotomists. We have an experienced and friendly admin team who support the clinical staff with the everyday running of the practice.
We are also supported by the local Primary Care Network and have access through this to Mental Health Practitioners, First Contact Practitioners and a team of Pharmacists.

Main duties of the job

We have a new, exciting opportunity to join our existing clinical team.
