Our dispensing service is fully outsourced, freeing you from dispensary commitments and allowing you to focus on clinical practice. In addition, on-call responsibilities are managed remotely, offering greater flexibility and work-life balance.
Role highlights:
Deliver high-quality, patient-focused clinical pharmacy services
Undertake medicines reconciliation, reuse checks, and ordering
Work closely with multidisciplinary teams to optimise medicines use

Main duties of the job
The post holder will contribute to the planning and delivery of specialist mental health clinical pharmacy services as part of the CWP pharmacy team. This will include working as a member of the multidisciplinary team to ensure that pharmaceutical care needs of patients are met, ensuring that medicines management standards are upheld in all areas of the trust and providing education on safe and effective use of medication to medical and nursing staff, patients and carers. The post holder will provide professional support and supervision to pre-registration and student pharmacists.

Main duties of the job
As a Pharmacy Technician, you will be responsible for providing a comprehensive ward-based pharmacy service across inpatient mental health units. Key responsibilities will include:
- Undertaking medicines reconciliation for newly admitted patients to ensure accurate medication records.
- Assessing patients' own medicines for suitability and safe re-use in accordance with Trust policies.
- Ordering, supplying and managing medicines required for individual patients and ward stock.
- Supporting patients and carers by providing information and advice regarding their medicines.
- Working collaboratively with nursing, medical and pharmacy colleagues to promote medicines optimisation and improve patient outcomes.
- Contributing to discharge planning, including assessing patients' own medicines and ensuring appropriate medication supplies are available.
- Undertaking technical accuracy checking duties where accredited and authorised.
- Supporting the safe storage, handling and administration of medicines within inpatient areas.
- Participating in service development, audit activities and quality improvement initiatives to enhance pharmacy services.
Applicants should be proactive, organised and able to work effectively as part of a multidisciplinary team. You should have post-qualification experience in a ward-based pharmacy environment and either hold, or be working towards, an accredited Accuracy Checking Pharmacy Technician (ACPT) qualification.

In 1970, the practice moved to 240 Upper Chorlton Road, but outgrew the space. In April 1998, we relocated to 171 Upper Chorlton Road. A patients research revealed the building was once called Ashville, hence the name Ashville Surgery.
Five years ago, we became the first practice in Manchester to use AskmyGP, which has transformed patient access and helped us manage modern-day pressures more effectively.
Our large common room is often used for remote consultations, encouraging informal collaboration and shared learning with plenty of coffee and conversation! We hold regular clinical and organisational meetings, involving the whole team in decision-making where possible.

Job Summary:
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities:
In accordance with the Practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including AskmyGP consultations, surgery consultations, telephone consultations and queries, triage calls, triage visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Respond to medical problems presented by temporary residents as required.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organization.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
To assist in the establishment of appropriate systems to manage common chronic medical conditions.
Completion of clinically related administrative and non clinical duties needed for the delivery of the service.
Providing counseling and health education.
Refer patients for further opinions and interventions according to Practice protocols.
Recording clear and contemporaneous consultation notes to agreed standards.
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions wherever possible).
Prescribing in accordance with the Practice prescribing formulary whenever this is clinically appropriate.
Participate in training plans of medical students as required.
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Maintaining, monitoring and updating computer database and management of medical records and practice audits
Ensure that practice clinical database is updated promptly and accurately with information relating to patient records, as required for clinical accuracy and for practice recording requirements under QOF and Enhanced Services.
Appropriate use of read codes.
Participating in the collection and collation of statistics towards the preparation of practice reports, performance monitoring and audits.

Job summary
The Cluster 3 Perinatal Community Mental Health Service sits within the GM-wide Perinatal Community Service which is made up of three Specialist Perinatal Mental Health Teams . Cluster 3 covers North Manchester, Rochdale, Oldham and Tameside. There are 2 posts available, one post is for North Manchester and the other post is for Rochdale & Oldham.
The postholder will be the clinical leader of the Perinatal CMHT supported by the Operational leads and will undertake assessments of referrals/ admissions to the service. The Postholder will have a lead role in the assessment, formulation and management of cases and will carry a caseload of patients. The postholder will provide clinical supervision for trainees in gaining core psychiatric experience and in achieving competencies in core general psychiatry. Main duties of the job
o The perinatal Consultant Psychiatrist (CP) will be the clinical leader of the Perinatal CMHT supported by the Operational leads. The CP will prioritise women depending on clinical need and ensure recommended timeframes are met. Undertake assessments of referrals/ admissions to the serviceo They will have a lead role in the assessment, formulation and management of cases and will carry a caseload of patients with complex care needs and of patients deemed to present with increased risk in the perinatal period.o Provide management in treatment resistant and complex presentationso Assessment and management of risk including of vulnerability, suicide risk, risks to health and safety and risk of harm to others.o Preparation of Court Reports, Mental Health Review Tribunal and Managers' Hearings reports and attendance at subsequent hearings.o These patients will predominantly be seen in the clinic setting but can also be seen in a variety of community settings if appropriate or necessary.o Work in a collaborative way with multidisciplinary teams including external partners across maternity, social care, health visiting and primary care.o The CP will run obstetric liaison clinics within the maternity setting and work in collaboration with the Specialist Perinatal Mental Midwife to plan pre and postnatal birth plans.

Older Adult Community Mental Health Team and Memory Service - West - Maternity Cover 12 month fixed term post: Principal Clinical Psychologist Band 8b - 1 x 1.0 wte
An exciting opportunity has arisen for a 12 month fixed term maternity cover post within the older adult CMHT and memory service covering West Cheshire (Chester, Ellesmere Port and Neston, Northwich, and rural areas), with a base in Chester. CWP community mental health services are multi-disciplinary teams aiming to provide person centred, psychologically and trauma informed models of care. The memory service works closely alongside the older adult CMHT to provide an assessment and diagnosis service for dementia. Psychology has been integrated and is valued within the multi-disciplinary team, providing specialist assessment and interventions, as well as contributing to the clinical thinking and decision making of the teams. Psychological skills are also applied to delivering training, staff wellbeing initiatives and service development projects. We would like to recruit an experienced psychologist that will join an expanding older adult psychology workforce. You will support the development of a Mental Health Wellbeing Practitioner in your team. You will meet regularly with other senior psychologists throughout the Trust and an older adult psychology specialty group to ensure equitable service delivery.
Main duties of the job
The post-holder will join a supportive multi-disciplinary team who are passionate about working with older adults. The post holder will be responsible for providing assessments (including neuropsychological if required), formulations, and therapeutic interventions (both individually and in group format) to people with complex psychological and mental health difficulties. Psychology also provides specialist neuropsychological assessment to inform MDT decisions within the memory service. The post holder will support the wider team to develop psychological understandings of people's difficulties through consultation, supervision, and training.
Based near the historic Roman city of Chester, the service has good links with the three local universities (Liverpool, Lancaster, and Manchester) providing opportunity to support trainees with research projects/audits and input into the teaching programmes.
The post holder will be expected to take a lead in service evaluations and ensure high standards of clinical effectiveness and clinical governance.
The service currently offers flexible working with a mixture of both remote and face to face encouraged.

Main duties of the job
Working as part of the team you will undertake a range of clerical tasks, including processing of referrals, scanning/photocopying and uploading documents to electronic records, answering telephone calls and forwarding/redirecting calls, arranging meetings and taking meeting notes).
Within this role you will have contact with patients, carers, families, and other health professionals. The successful candidate will have excellent communication and organisational skills, a flexible 'can do' approach, and a professional, friendly and welcoming manner.

The practice has a list size of approximately 15500 patients and also operates a walk-in service which cares for all patients in Manchester as an urgent care treatment centre.

Full preceptorship will be given to those who are newly qualified. You will be workingto workwith adults with a learning disabilityas part of an enthusiastic and committedmultidisciplinary team, taking referrals from both the local area but also specialist referrals from across the Northwest as part of the inpatient offer.Admissions are short term for people with learning disability and complex needs requiring a period of intensive assessment, treatment and or therapy within an inpatient unit.
As a key member of our Assessment and Treatment Unit, you will develop person centred plans and work closely with other member of the team to help individuals reach their potential.
Main duties of the job
The post holder will be expected to work within a multi professional framework, being named nurse for adult service users with learning disabilities and complex needs. The post holder will work within a hierarchical structure, under the supervision of the Manager/Deputy Manager.
As a Staff Nurse you will be required to take charge of the shift and offer management supervision sessions to the Clinical Support Workers who are assigned to you.
The successful candidate will be encouraged to excel within their nursing role and we offer a number of development opportunities both internally through our education department and externally though a range of colleges and universities we are associated with.
The ideal candidates must have strong and positive values, be enthusiastic, have good communication skills and able to build effective working relationships within a multi-disciplinary team.

Cheshire and Wirral Partnership NHS Foundation Trust have an exciting opportunity that has arisen for a full time (10 PA's) Consultant Psychiatrist on Juniper Ward in Chester.
Juniper Ward is a mixed category open age inpatient mental health ward caring for people aged 18 and above in West Cheshire. They provide service to patients presenting with different mental disorders including affective disorder, psychosis and personality disorder where patients can be admitted informally or detained on the Mental health Act.
The successful applicant will work alongside a large and enthusiastic service of consultant colleagues working across Cheshire and Wirral Partnership Trust. In addition, the post-holder will have an important role to play in the development and delivery of psychiatry services across the footprint of Cheshire and Wirral Partnership Trust.
The consultant will participate in the Consultant Psychiatrist on-call rota.
Main duties of the job
To manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the medical director and in accordance with the Trust's personnel policies and procedures. This may include assessing competences under the Modernising Medical Careers framework.
To undertake the administrative duties associated with the care of patients.
To record clinical activity accurately and comprehensively and submit this promptly to the Information Department.
To participate in service and business planning activity for the locality and, as appropriate, for the whole mental health service.
To participate in annual appraisal for consultants.
To attend and participate in the academic programme of the Trust, including lectures and seminars as part of the internal CPD programme.
To maintain professional registration with the General Medical Council, Mental Health Act Section 12(2) approval, and to abide by professional codes of conduct.
To participate annually in a job plan review with the clinical manager, which will include consultation with a relevant manager in order to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernisation.

You will be part of a small and supportive Integrated Care Community Team providing Therapy for patients in their own home. This will involve working with patients to remain in their own home in the community and support with rehabilitation of patients.
The team works across Neston and Willaston working alongside 3 GP practices to deliver care to the practice population
You could already be a Band 6 wishing for a change, or currently an experienced Band 5 who would like to take the next step in your career and practice in a community setting.

Main duties of the job
To work autonomously as a specialist therapy practitioner within an integrated Care Community Team to deliver a high-quality Occupational Therapy service through assessment and treatment of patients referred with complex rehabilitation needs
To work as a member of a multi-disciplinary team and communicate information to those agencies (either health or social background) involved, to meet the needs of the patient & plan effective care.
To participate in departmental In-service Training and Journal Club activities to further consolidate knowledge base and specialist clinical skills. Participate in the CPD programme to share these skills with therapy and nursing colleagues.
To provide supervision, teaching and support to senior and junior staff, assistant staff and act as a clinical educator for undergraduate students.
