Job summary
Vale of Evesham Primary Care Network is looking for an organised, motivated and detail-focused Data Care Coordinator to join our team.
This is a varied role combining data, administration and care coordination, supporting our work across neighbourhood health, population health and personalised care.
The postholder will work closely with our member GP practices and wider PCN team to help identify and monitor patient cohorts, maintain accurate trackers and reporting, support proactive patient contact and ensure actions and outcomes are recorded appropriately.
A key focus of the role will be supporting patients who may have unmet needs, experience barriers to accessing healthcare or require additional help to navigate services.
This is a support-level role and does not require advanced data analysis or specialist technical expertise. We are looking for someone who is confident working with information, enjoys keeping things organised and is equally comfortable communicating with patients and practice teams.
Main duties of the job
The role will include:
Maintaining PCN spreadsheets, trackers and monitoring systems.
Supporting routine monthly and quarterly reporting.
Assisting with the collection and collation of data from PCN services and GP practices.
Helping identify patient cohorts who may benefit from additional support.
Supporting proactive patient contact and follow-up where appropriate.
Helping patients navigate health, care, wellbeing and community services.
Recording patient contacts, interventions and outcomes accurately.

Working as part of a multidisciplinary team, the pharmacist provides expert advice on prescribing and medication queries, undertakes medicines reconciliation following transfers of care, and promotes safer prescribing practices. Main duties of the job
The post holder will undertake structured medication reviews, both face-to-face and remotely, with a particular focus on high-risk patients, including care home residents, housebound individuals and those with complex long-term conditions or polypharmacy. They will reconcile medicines following hospital discharge to ensure continuity of care, support chronic disease management clinics, and contribute to multi-morbidity reviews and QOF objectives.
The role includes leading prescription management processes, overseeing repeat prescribing, ensuring appropriate monitoring requirements are met, and supporting prescribing administration teams. The post holder will implement MHRA alerts, drug withdrawals and evidence-based prescribing changes to improve medicines safety and clinical effectiveness.

We are looking for an experienced and motivated Mental Health Practitioner to join Droitwich, Ombersley & The Rurals Primary Care Network, working as part of the team at Chaddesley Corbett Surgery.
This is an exciting opportunity to work at the interface between primary care and mental health services, providing accessible, person-centred mental health support within a general practice setting. You will work closely with GPs and the wider multidisciplinary team to assess and support adults experiencing mental health difficulties, helping patients to receive the right care and support at the right time.
