We are looking to recruit an outstanding newly-qualified Physiotherapist to join the Level Health Training Academy. The role is a static MSK Physiotherapy position which will provide an invaluable insight into a broad range of both peripheral and spinal problems.
You will work alongside a large and established team of Senior and Junior Physiotherapists in our vibrant, innovative and dynamic organisation. We have the capacity to create a flexible package to accommodate different working needs.
Level health operates a mandatory 2 year Academy Programme for all newly-qualified Junior staff. To ensure that learning and professional development is consistent, we run weekly clinical supervision/mentoring sessions and monthly in-service training meetings with the whole team, providing a well-rounded, robust, supportive and patient-centred teaching programme. In addition, we provide an annual training budget for the further specialist development of our staff.
Main duties of the job
Level Health is looking for outstanding Physiotherapists with excellent communication skills. You will be working with patients, GPs and practice staff, as well as the Level Health team, so it is critical that you are a confident and strong communicator with demonstrated IT, organisational and time management skills.

The University Medical Centre in Bath is seeking an enthusiastic and experienced Chronic Disease Practice Nurse to focus primarily on the management of asthma and diabetes care for our patients. The role may be extended to fit your skill base if you hold additional qualifications.
The role is centred around a mostly University student patient population and we welcome new members to join our existing nursing team and to help support the needs of our patient population.
Flexible working shift patterns are able to be considered and a competitive salary is being offered for the successful candidate.

Main duties of the job

This is an important role within our practice and wider Primary Care Network (PCN), supporting patients to navigate health and care services and helping them to take an active role in managing their health and wellbeing.

Main duties of the job
You will work closely with GPs, nurses and other primary care professionals, as well as Social Prescribing Link Workers, Health and Wellbeing Coaches and wider community services.
You will work proactively with our clinical team to identify patients who my benefit from care coordination and personalised support.
You will also support long-term condition management, proactive patient reviews, health promotion and wider PCN activity.

Job description:
The Clinical Services Care Coordinator is primarily responsible for supporting the Clinical Services Team with the delivery and coordination of services offered by the practice and the wider Primary Care Network (PCN).
The focus of the role is to support high-quality, personalised care for patients through effective care coordination, navigation and administrative systems, with a particular focus on patients with long-term conditions, multiple needs and those who may benefit from additional support.

Provide care navigation and coordination across health, social care and community services.
Coordinate appointments, reviews, home visits and other services where required.
Support the coordination and delivery of multidisciplinary team (MDT) meetings.
Support long-term condition management, patient recalls, health promotion and proactive care.
Undertake EMIS searches, audits and monitoring activity.

Training and Development:
- The practice will support the successful candidate to complete the training required for the ARRS Care Coordinator role where this is not already held.
- This will include, as appropriate, Personalised Care Institute-accredited Care Coordinator training, personalised care and support planning, shared decision making and relevant practice/PCN training.
- Appropriate supervision and ongoing professional development will be provided.
