Please note, the successful candidate will initially be employed by Brooklands Medical Practice pending transition into Better Health MCR.
Main duties of the job
Support with prescription related requests and queries from patients and monitor medicines usage in line with long term condition management.
Review clinical letters and hospital discharges and action appropriate medication changes and where necessary support patients with optimisation of their management.
You will undertake clinical medication reviews with particular focus on those with additional need such as polypharmacy, long-term conditions, multi-morbidity, frailty, or multi-factorial need.
You will work the organisations prescribing lead and the wider ICB medicines management team to ensure evidence based and cost-effective prescribing in line with the latest guidance and safety alerts.
You will be an experienced Pharmacist with a knowledge and commitment to working in primary care. You will understand the needs of patients from deprived and diverse backgrounds.

Westleigh Medical Practice is a well-established training practice serving approximately 4600 patients in Westleigh and Abram. We have two surgeries - Westleigh (main site) and Abram (branch site). Recently CQC inspected and awarded GOOD. We are a member of the Leigh Primary Care Network (PCN). The successful candidate will work closely with our two Partners to lead the day-to-day management and future development of the practice. Business Management

We are one of the largest oncology Trusts in the UK with an international reputation for clinical excellence, research and development. As part of a collaborative venture between The Christie NHS Trust and MCHFT to develop clinical haematology services, the Christie is pleased to be able to offer an exciting opportunity for a specialist clinical pharmacist to join the clinical haematology team This is a unique opportunity to work with us at a new peripheral centre based on the Mid Cheshire Hospitals site and contribute to the challenging and rewarding specialty of haemato-oncology pharmacy, delivering advanced clinical care to patients with haematological cancers and general haematological conditions.
We are integral members of the MDT, and we are expanding our roles in existing services such out-patient clinics and peripheral site services, which the post-holder will develop alongside the Lead Pharmacists. We are also developing links with the University of Manchester and working on collaborative projects at the main site, with scope to role this out further at peripheral sites.The postholder must possess an independent prescribing qualification to assist in the MDT outpatient clinics. They will play a key role in the future development of outpatient services within the highly regarded haematology MDT, implementing new initiatives in collaboration with the Lead Haematology Pharmacist and Lead Pharmacist for Clinical Services.
Main duties of the job
To provide a highly specialist clinical pharmacy services to outpatients within the Christie Haematology @ Leighton Service and develop a good working partnership with internal and external stakeholders to ensure a high quality SACT service is provided to patients.
The specialist pharmacist will work across the clinical haematology outpatient service at Leighton Hospital alongside a team of clinical haematology consultants, advanced nurse practitioner and clinical nurse specialists.
Pharmacist independent prescribing responsibilities support this practice. The specialist clinical pharmacist is expected to contribute to planning, education, research, and participate in clinical audit within own area of practice.To deliver and evaluate clinical pharmacy services to both malignant and non-malignant haematology patients in conjunction with the Lead Haematology Pharmacist and Clinical Haematology service manager.To support the multidisciplinary team in the safe, efficient and cost-effective use of medicine resources to the designated cancer disease group by the provision of education and training, procedures, policies and guidelines, and medicine usage information.Although the outpatient service is based at Mid Cheshire Hospitals NHS Foundation Trust site , postholders may be required to work at any location across the Greater Manchester and Cheshire Cancer Network.

Job summary
We are looking for a permanent part-time Medical Receptionist to join our team based at The Robert Darbishire Practice, the largest and busiest of the three GP practices in Better Health MCR. The post is 22.5 hours per week; Monday - Friday 08:00 - 12:30.
The postholder will be required to work flexibly and to provide cover for absence and busy periods. As part of this, they may be required to work Saturdays. There may also be a requirement for the postholder to work at our other practices from time to time.

Main duties of the job
Our reception team are the first point of contact for our patients and play an important role in understanding patients needs, resolving queries, dealing with prescription requests and either booking an appointment with the most appropriate clinician at the practice or signposting the patient to a service outside the practice.
You dont need to have worked in healthcare before as long as you have excellent people skills, a good attention to detail and you are willing and eager to learn new skills. We will provide in-house training and support.

Qualifications
Essential
- A good standard of grammar and spelling in the English language.
- General education to GCSE level or equivalent.
- You must be eligible to work in the UK. Visa sponsorship is not available for this role, and there is a significant training period. Therefore you must meet one of the following requirements: British or Irish citizen, or, indefinite leave to remain in the UK, or, a Student Visa that allows you to work the required hours and which expires no less than 18 months after the application closing date.
