North Sedgemoor PCN brings together GP practices caring for around 50,000 people across Burnham on Sea, Brent, Highbridge, Cheddar and Axbridge. We are clinically led, collaborative and community focused. Our team includes GPs, Occupational Therapists, Pharmacists, Care Coordinators, Health Coach and an experienced Paramedic who will be a peer and mentor for this role.

Job summary

We are looking for a salaried GP colleague who is passionate about providing high quality care for patients. Previous experience working as a duty doctor would be preferred, and experience of using askmyGP would be useful but not essential.

Main duties of the job

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Overview of our organisation

We have a skilled and experienced team comprising of 3 GP Partners, 7 GPs, 3 Nurse Practitioners, Paramedic Practitioner, Clinical Care Co-ordinating team and an excellent nursing team. We are also supported by Musculoskeletal Practitioners and a Mental Health Practitioner. As well as the existing team we are a training practice for GP speciality trainees and so have GP Registrars working with us from the Lincoln scheme and also host Medical Students from Lincoln Medical School. Alongside our clinical team we have a highly motivated and competent Medicines Management, Reception/ Administration and Dispensary teams.

Our GP team is a mix of ages, gender and personalities which adds to the character of our team and practice. We treat all patients and staff with dignity, independence, respect and honesty in an environment that is accessible, safe and friendly. We continually receive excellent feedback from GP speciality trainees, medical students and locums about how friendly our whole practice team is.

Our practice area is close to the Notts/Lincs border, near to the A1 and just over an hour away from London by train.

Main Responsibilities:

As a valued member of the Operose Health team, your key duties will include:

• To deliver high quality clinical care to the patients registered and attending the GP surgery and to work closely with the clinical leads and local practice team to support the delivery of the surgeries operational plan.

An exciting opportunity has arisen for a highly motivated, experienced, and enthusiastic Advanced Clinical Practitioner to become part of the clinical review team at our Christie satellite centre in Macclesfield.This is an opportunity for ACPs of all backgrounds to join this exciting team.Educated to Masters level, you will be expected to practice autonomously at an advanced level in the delivery of high quality, safe and supportive care to patients including assessing and managing radiotherapy and chemotherapy side effects and prescribing independently. The Christie NHS Foundation Trust is a highly specialised oncology treatment hospital. At The Christie at Macclesfield site we deliver both radical and palliative radiotherapy, using a range of modern techniques. The department also offers a local metastatic spinal cord compression (MSCC) service and is equipped with a CT simulator and two Linnear Accelerators.You must have the necessary skills to be able to react to and manage acute/emergency situations. You will be a key member of the clinical team which includes oncologists and other allied health professionals in both the centre and wider Christie team. You will be involved in the clinical training and development of qualified AHP's and support staff.Those without the ACP qualification may be considered for a Band 7 trainee post.

Main duties of the job

The Advanced Clinical Practitioner is a skilled practitioner who, demonstrates highly developed specialist knowledge and skills in the assessment and management of patients. They will formulate clinical decisions and complex treatment plans to manage acute and chronic illness and will respond to actual and potential health problems appropriately. Non-Medical prescribing, strategic planning, education, research and audit are key components of the role.The postholder will have a role in the clinical assessment of patients attending the hospital, working autonomously to assess the clinical situation, assessing symptoms determining a working diagnosis and appropriate supportive management. They will oversee the management of patients on outpatient treatment pathways and those attending for radiotherapy, identifying issues and altering treatment plans in line with consultant colleagues. They will order investigations, make appropriate/timely referrals and maintain relevant clinical records. They will work alongside medical colleagues maintaining good communication and collaboration between the various multidisciplinary teams. Evidence based care will be inherent in their practice, ensuring that best practice is disseminated throughout the clinical areas.
