Job summary
An exciting opportunity has arisen for a qualified PWP to join our established primary care mental health service who look after the adult population of Mount View Practice & Broadway Medical Centre.
The service offers an individualized, IAPT compliant approach to Primary Care Mental Health.
Our service endeavours to ensure that our patients experience the appropriate treatment, for a wide range of mental health issues, focusing on the use of Cognitive Behavioural based therapy, solution focused therapy and problem solving approaches; the use of self-help literature and psycho-educational groups; with an emphasis on general well-being.
The Post holder will be expected to complete some group work interventions.
Main duties of the job
To provide high quality primary care IAPT services to adults with mild to moderate mental health problems.
To work autonomously within professional guidelines and the overall framework of team policies and procedure.
To deliver IAPT services in accordance with NICE guidelines, within agreed activity levels and model, meeting required key performance indicators (KPIs).
To provide advice and consultation on clients care, to other clinicians and to non-professional carers.

To provide regular information to clients GP and/or referrer about the treatment process and outcome, initial and on-going risk assessments, making appropriate disclosure and onward referrals where necessary.

Job summary
The Mount View Practice in Fleetwood are looking for an energetic, dedicated, caring and patient focused individual with a can-do attitude, who has the desire to make a difference to the lives of the patients in our community.
Responsibilities are administrative in nature with non-clinical contact with patients and visitors.
Applicants must be able to work Monday to Friday between the hours of 0800 & 1830. Full time hours over 5 days can be discussed at interview.

Main duties of the job
We are looking to recruit a flexible, friendly, self-motivated individual to provide efficient and comprehensive support to the Practice to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.
The post holder will complete general administration tasks including Docman, answering the phone, completing spreadsheets, filing, photocopying and scanning.
Excellent communication skills are required, both verbal and written, to be able to converse with patients, relatives and other staff members in a professional manner via phone and email, taking and recording messages and acting on any requests in a timely manner.
The ability to work independently with a flexible approach to work is also essential for the role as tasks will also be allocated from the Management Team.
Previous experience of working within Healthcare or in a Medicines Management role would be preferred but full training will be given for the right candidate.

Job summary
Due to retirement, we are seeking an enthusiastic and motivated Practice Nurse to join our friendly, patient-focused primary care team. This is an excellent opportunity to work within a supportive and forward-thinking GP practice committed to delivering high-quality care to our local population.
