Job summary
We are looking for a proactive, motivated and organised Care Coordinator to join our friendly GP practice team here at The Lyndhurst Surgery. This is an exciting opportunity to play a key role in improving patient outcomes by coordinating care, supporting preventative health initiatives, and helping patients access the services they need. Working closely with GPs, clinical and administrative staff and external healthcare providers, you will support patients with long-term conditions, preventative healthcare programmes, and personalised care planning, while also providing essential administrative support to ensure the smooth running of the practice.
Main duties of the job
As a Care Coordinator, you will:
- Assisting patients by directing patients to the most appropriate clinician or service.
- Dealing with appointment, home visit and telephone requests. Booking, amending, and cancelling patient appointments.
- Contacting patients to arrange annual health and wellbeing reviews, particularly for those with long-term conditions and patients who are housebound.
- Providing day-to-day administrative support, including processing letters, updating patient records, responding to patient enquiries and Reception cover when required.
- Arranging referrals to hospitals and community services, keeping track of their progress and following up where necessary.
- Keeping patient records accurate, up to date and confidential using the practice clinical system.
- Working closely with GPs, nurses, the wider practice team and external healthcare providers to deliver joined-up, patient-centred care.
- Chasing referral appointments, reports and outcomes to help ensure patients receive the care they need without unnecessary delays.
- Following all NHS guidance, practice policies and information governance requirements to ensure safe and effective working.
About us
We are a small friendly, forward-looking practice with a practice population of approximately 5000 patients.
Our team comprise 2 Partner GP's, 1 practice manager, 3 Locum GP's, 1 Practice Nurses and 1 clinical pharmacist, together with a strong and hard-working Administrative team.
We are an accredited training practice and teach and support Medical students at different levels.

Job responsibilities
As a Care Coordinator, you will:
- Assisting patients by directing patients to the most appropriate clinician or service.

- Working closely with GPs, nurses, the wider practice team and external healthcare providers to deliver joined-up, patient-centred care.
