Job summary
The Lead Practice Nurse will provide clinical leadership and day-to-day management of the practice nursing team, including Practice Nurses and Healthcare Assistants (HCAs), ensuring the delivery of safe, effective and high-quality nursing services to our registered patients.
Working as a senior member of the practice multidisciplinary team, the post holder will provide expert clinical care alongside leadership and oversight of the nursing service. They will support the development of the nursing team, ensuring appropriate standards of clinical practice, competency, training and professional development are maintained.
The Lead Practice Nurse will take responsibility for key areas of clinical governance and service delivery, including Infection Prevention and Control (IPC), long-term condition management, health promotion and preventative care, immunisations, screening and treatment room services.
The post holder will work closely with the GP Partners and Practice Management Team to develop, implement and review clinical policies, protocols and procedures, supporting the practice to meet regulatory, contractual and quality standards.
The role will include supporting the effective management of patients with long-term conditions using evidence-based practice, including the care of older, frail and housebound patients. The post holder will also contribute to clinical audit, quality improvement and the ongoing development of nursing services within the practice and wider team.
Main duties of the job
The Lead Practice Nurse will provide clinical and professional leadership to the nursing team, including Practice Nurses and HCAs, while continuing to deliver high-quality general practice nursing care.
Key responsibilities include:
Providing day-to-day leadership, supervision and support to the nursing team.
Supporting staff development, competency assessment and training.
Coordinating nursing rotas and clinical cover to support safe service delivery.
Delivering clinical care including long-term condition management, health promotion, preventative care, immunisations, screening and treatment room services.
Supporting care for patients with complex and long-term conditions, including older, frail and housebound patients.
Leading and supporting Infection Prevention and Control (IPC).

Job summary
Martlesham Heath Surgery has an opportunity for an additional care co-ordinator to join our team, in thisgreat opportunity to work with an innovative and expanding surgery alongside current members of staff. This role could be offered on either a full-time or a part-time basis.
All shifts would be within our opening hours of 08:00 to 18:30 and would cover morning, afternoon and evening requirements. Additional overtime, sickness and annual leave cover will also be available. Full discussion of availability and working hours will be had at interview. This is a dynamic and multifaceted role that supports the clinical and administrative functions of the healthcare practice. The successful candidate will play a vital role in coordinating patient care, managing reception-based tasks, undertaking medical secretary responsibilities, performing general administrative duties, and providing support within the clinical hub as needed. This position requires excellent organisational, interpersonal, and communication skills to ensure the smooth and efficient operation of the practice.
Main duties of the job
- Reception-based work: greeting patients, answering calls, scheduling appointments, and managing patient inquiries.
- Medical secretary duties: preparing correspondence, managing referrals, handling medical documentation, and maintaining accurate records.
- Administrative support: managing patient information, generating reports, maintaining filing systems, and supporting practice operations.

The Advanced Medical Practitioner (AMP) will provide high-quality, safe, effective and person-centred clinical care to patients registered with West Heath Surgery who reside in designated care homes.
The post-holder will work as an integral member of the Practice multidisciplinary team, working collaboratively with GPs, advanced practitioners, practice nurses, clinical pharmacists, community nurses, therapists, care coordinators, social care professionals, care-home staff and other relevant health and social care professionals.
Main duties of the job
The post-holder will contribute to the delivery of proactive, personalised and coordinated healthcare for care-home residents, with an emphasis on:
- prevention of avoidable deterioration;
- early identification and management of acute illness;
- proactive management of frailty and long-term conditions;
- personalised care and support planning;
- medicines optimisation in collaboration with the appropriate prescriber/pharmacy professional;
- reducing avoidable hospital admissions;
- facilitating timely access to secondary and community care;
- supporting residents to remain in their preferred place of care wherever clinically appropriate;
- effective multidisciplinary working;
- continuity and coordination of care; and
- safe, evidence-based clinical decision-making.
The role will support the Practice in delivering its contractual requirements under the current NHS England Network Contract DES and Enhanced Health in Care Homes arrangements. The current EHCH model is designed around proactive, personalised and integrated care for people living in care homes rather than a predominantly reactive model.
The post-holder will work within their individual scope of professional practice, competence and authority. Where an activity requires GP, consultant, specialist or other professional input, the AMP will recognise this promptly and escalate appropriately.

The Mental Health Advanced Clinical Practitioner (ACP) will deliver high-quality, patient-centred mental health assessment, triage, brief therapeutic and biopsychological interventions and care navigation within a GP practice setting. The post is designed to improve timely access to mental health services within primary care through comprehensive assessment, clinical risk management, care planning, signposting and referral to appropriate statutory, community and voluntary sector services. The post holder will work collaboratively with the wider primary care team and external partner organisations to ensure patients receive coordinated, holistic and person-centred care.
Main duties of the job
The ACP will provide both face-to-face and virtual consultations, delivering evidence-based assessment and intervention across the practice population. Experience in assessing and managing the mental health needs of both adults and children is desirable.
An Independent Prescribing qualification is desirable but not essential. Where held, the post holder will be expected to prescribe safely and effectively within their professional scope of practice, in accordance with national guidance, local policies and regulatory standards.
