Job summary
The locum GP role is an essential member of the clinical team with the Extended Access Service. You will provide high quality clinical care to the patients attending the services, dealing with a diverse range of health needs. You will be part of a team providing a friendly and professional service, in a CQC rated good organisation.
This role is expected to be a mix of locum shifts on site across 5 locations. Hours of operation are:
- Monday - Friday 6:30pm - 8:30pm
- Saturday - 9am - 1pm and 1pm - 5pm
- Sunday - 9am - 1pm
The role has opportunity for personal development as well as support other clinical areas within The Confederation as needed.
Main duties of the job
Responsibilities include but are not limited to:
- Surgery and Telephone Consultations
- Administration

- Provide high quality consultations to patients in Hillingdon managing acute and chronic disease

Job summary
We are looking for an experienced part-time Physiotherapist to work in GP Practices across Long Lane PCN in Hayes as an MSK First Contact Physio for 25 hours per week on a 12 month fixed term contract. This exciting new role (Band 7) will be joining a growing team that includes Clinical Pharmacists and Social Prescribing Link Workers. Successful candidates will have a named GP Supervisor and access to peer learning developmental opportunities.
The PCN covers the following practices:

- The Pine Medical Centre
- Willow Tree Surgery
- Yeading Court
The successful candidate will have experience of initial assessments and creating treatment plans. One of our priority focus areas will be around frailty and prevention of falls. Experience in this area would be an advantage.
Main duties of the job
As a First Contact Physiotherapist, the post holder will work as part of a multidisciplinary team across the practices of a Primary Care Network (PCN), with the PCNs GPs and other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
The post holder will:
- Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, in order to establish a rapid and accurate diagnosis and management plan.

Job summary
An exciting opportunity has arisen for a full-time forward thinking and motivated experienced and IP qualified Clinical Pharmacist to join our North Connect PCN in Northwood on a 18 month fixed term contract.

Job summary
We work with general practices and other healthcare providers in Hillingdon to deliver high quality services for patients. In addition, we aim to improve care for patients by facilitating collaboration across general practice, releasing clinical time and developing the primary care workforce.
The post holder will be key in supporting the services delivered by the Confederation and will work alongside the Service Managers on aday to daybasis, helping to gather information including Key Performance Indicators and undertake enquires as and when is necessary for the Confederation Services.
This role will provide administrative and call centre support for the Confederation services, including receiving and processing referrals, utilising EMIS to enter records and create clinics and support the clinicians and managers who deliver the services.
Main duties of the job
- Provide administration support for a range of Confederation services
- Work as receptionist to meet and greet all patients arriving on site.
- Undertaking administrative duties such as photocopying, faxing and mail-out distributions.
- Participate in the induction and training of new members of staff and contribute to the Confederation development.
- Act as an ambassador for the Confederation.
- Sorting and prioritising all incoming mail and e-mail
- Ensuring all urgent and/or confidential communications are received and distributed from/to relevant parties in a timely manner.
- Covering the Patient Services telephone line- using initiative when dealing with phone calls and messages and ensuring any messages are passed onto the appropriate person.
- Booking and maintaining the patient appointment system (EMIS).
