Job summary
A vacancy has arisen for a Nursing Associate to join a community nursing team in Crawley and surrounding villages.
The Nursing Associate will provide a mixture of general and specific care to a group of patients and will be proficient and competent to work at their registered level. They will work alongside Registered Nurses and Allied Health Professionals. The post holder will have completed their Nursing Associate training and be registered with the NMC.
The Nursing Associate works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.

Main duties of the job
- The Nursing Associate will deliver care within their proven competency as achieved during their Nursing Associate Training.
- They will be supported by the Registered Nursing Team and supervised by their appointed registered nurse band 5 or above.
- Contribute to the planning, implementation and monitoring of patient care as part of the Community Nursing Team.
- This role will include supervision and teaching of trainee or newly qualified Nursing Associates and Health Care Assistants.
- Maintain at all times a kind, caring and professional attitude towards patients, relatives and colleagues within the clinical setting, taking into consideration dignity, privacy, and confidentiality, alongside cultural and religious needs.
- Recognise changes in a patient's condition and seek assistance from a Registered Nurse or GP as appropriate.
- Provide health and care information to the patient, carers and family as appropriate.
- Maintain clear concise and legible documentation adhering to standards in accordance with NMC and Trust policies.

As a Digital Research Facilitator, you will play a key role in connecting patients with commercial clinical research opportunities. Working across healthcare, data and innovation, you will support a digital pathway that enables researchers to identify eligible participants more quickly, accurately and fairly.
Using digital tools and regional datasets, you will work closely with hospital research teams and primary care partners to streamline participant identification and recruitment. By reducing manual processes and supporting faster recruitment, you will help attract commercial research investment into Sussex and improve patient access to innovative treatments and technologies.
You will also support a more inclusive approach to research participation, helping ensure opportunities reach diverse and underrepresented communities. Through collaborative working and the effective use of data, you will contribute to better health outcomes and make cutting-edge clinical research accessible to more people across Sussex.
Main duties of the job
As a Digital Research Facilitator, you will support the delivery of digital participant identification and recruitment pathways for commercial clinical trials across Sussex. Using healthcare datasets, digital systems and research tools, you will undertake feasibility assessments and identify potential research participants efficiently and ethically.
Working with clinical teams, GP practices, digital partners and research colleagues, you will help streamline recruitment processes and support the achievement of recruitment targets. You will monitor recruitment activity, maintain study records and produce reports that support performance improvement.
The role includes analysing recruitment pathways, identifying risks and implementing solutions to improve efficiency, reliability and participant engagement. You will act as a key link between research teams, primary care organisations, secure data environments and external partners to support effective study delivery.
You will champion the safe and compliant use of patient data, ensuring adherence to information governance, GDPR and research regulations. You will also support the development of guidance, training and best practice resources to encourage adoption of digital research processes.

We are looking for an experienced senior leader with a strong track record in NHS primary care, operational management and transformational change. Job summary
This service is a collaboration between Sussex Community NHS Foundation Trust SCFT and the Sussex Federations: Brighton & Hove Federation, Alliance for Better Care and South Downs Health & Care. The Federations will be delivering the GP-led aspect of the Virtual Ward at a place-level to match SCFTs Urgent Community Response service across Sussex. SCFT provides the service in West Sussex, Brighton and Hove and the High Weald, Lewes and Havens area of East Sussex.
B&HF are the lead contractor for the GP Service delivery and have sub-contracted to ABC for delivery within the federations geographical areas of Mid Sussex, Worthing and Chichester.
Since 2022, SCFT have delivered Virtual Wards Hospital at Home in line with the NHS Englands national definition that a virtual ward is a safe and efficient alternative to NHS bedded care that support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
As of August 2024, SCFT re-modelled the Virtual Ward delivery and mobilised General Virtual Wards within Urgent Community Response UCR, which supports Sussex patients within its catchment area with a higher level of acuity to be safely managed at home/chosen place of residence. This could be patients on either an admission avoidance pathway or early supported discharge pathway, where the alternative would be NHS bedded care.
Main duties of the job
The Virtual Ward activity is now prioritising admission avoidance whilst still delivering early supported discharge for patients from the Acute hospitals. It is delivered 365 days per year, 8am 8pm. UCR teams within the SCFT footprint provide urgent support to help prevent unnecessary hospital admissions and ultimately keeping patients safely at the place they call home. There is a consistent service across Sussex and is made up of multi-disciplinary teams including General Practitioners, Nurses, Pharmacists, Therapists and Carers, ensuring they can meet a range of needs quickly offering the best level of support to patients.
The multi-disciplinary team that oversees and deliver care the General Virtual Ward caseload is led by both medical a General Practitioner and clinical senior decision makers. Virtual and On Call GP cover is available seven days a week, 9am to 6pm 365 days a year, including bank holidays. This cover is linked to the respective teams local UCR teams with a virtual board round each morning
The UCR Hub Based GP will work within their assigned place based SCFT UCR base. We are recruiting for a GP for the Worthing Virtual Ward. The GP will present within the allocated UCR base working face to face with the SCFT MDT delivering GVW including supervision of ACPs and team development. Patient visits may be required. Monday, Wednesday and Friday morning
