Job summary
We are looking for a Care Navigator to join the team at Debenham Group Practice. This is a patient-facing role at the heart of how the practice runs day to day, and we are looking for someone who will bring a calm, friendly and organised approach to supporting our patients and colleagues.
Debenham Group Practice is part of Suffolk Primary Care, which gives us access to a wider range of shared expertise and organisational support, while allowing us to remain a well-regarded local practice with a strong community identity.
About us
We care for 10,000 patients across our main site in Debenham, with branch surgeries at Otley and Grundisburgh.
- A stable team, with a good mix of experience
- A strong team ethos, with people supporting each other day to day
- Use of SystmOne and askmyGP to support patient access and workflow
- Access to a broader multi-professional team through Suffolk Primary Care
We are looking for someone who:
- Is calm, warm and professional when dealing with patients and colleagues
- Listens carefully and records information accurately
- Is organised, reliable and takes their responsibilities seriously
- Works well as part of a team and is willing to support colleagues
- Has good IT skills and picks up new systems with confidence
- Has previous experience in a customer-facing or administrative role
Experience in a healthcare or NHS setting is helpful but not essential we are happy to support the right person to develop in the role.
Main duties of the job
Care Navigators are often the first voice or face a patient encounters. Whether patients contact us by phone, walk through the door, or submit a request online, you will help make sure they get the right information and reach the right place.
We use askmyGP as our primary workflow tool for clinical requests. Rather than triaging patients themselves, Care Navigators help ensure that requests are accurately recorded so that the duty GP can review and act on them promptly. You will also take turns supporting the duty GP as part of the wider Care Navigation team, and you will help signpost patients to other local services where that is the most appropriate response.
The role also covers the full range of day-to-day practice administration from reception duties and managing records, to supporting clinical and dispensary colleagues and keeping the practice running smoothly.
You will work across our sites as needed, as part of a well-established, friendly team.
We are looking to recruit for Wednesday, Thursday and Friday. Shifts fall within a working window of approximately 07:30 to 18:30 across those days. Depending on recruitment, hours may be split across more than one person.
We welcome applications from individuals who can cover all three days as well as those who are available for part of the pattern, so please do apply if mid-to-end of week working suits your availability.
About us
What we offer
- A supportive, well-established team in a friendly rural practice
- Opportunities for training and development
- Access to the wider resources and support of Suffolk Primary Care
- Access to NHS Pension
This is a role where you can make a genuine difference to patients and to the team around you. If this sounds like the sort of place youd like to work, wed be delighted to hear from you.

Job summary
You will provide high-quality, patient-centred clinical care across the practices of the Eye locality; Eye, Debenham, Mendlesham and Fressingfield. As one of a team of four paramedics working together across these sites.
Your work will be centred primarily on home visits, alongside supporting care home ward rounds, running minor illness clinics, and taking on other clinical work as needed. The purpose of the role is to extend the reach and capacity of general practice: assessing, diagnosing and treating patients who would otherwise add to GP workload, and freeing GP time for the patients who need it most.
You will work within defined clinical protocols and scope of practice, with support and guidance from GPs, but you will be expected to function as a confident, autonomous clinician in your day-to-day decision-making, particularly in the home visiting setting.
All patients requiring a home visit are triaged and clinically assessed by a GP before allocation to you, so you will always be visiting with a clear clinical picture and rationale already established, and with a GP available to you throughout the day for advice or escalation.
You will need to demonstrate safe, sound clinical decision-making and strong assessment and diagnostic skills, and to prioritise and assess patient need appropriately, including making referrals for further investigation or care where necessary.
Main duties of the job
Home Visits
Undertake home visits as your primary activity, attending patients who have been triaged and allocated to you by a GP, assessing and treating them in their place of residence across a wide range of urgent, same-day and chronic-disease-related needs. Act as an autonomous clinician in the home setting, using sound clinical judgement and escalating to a GP where a case falls outside your scope of practice. Prioritise and sequence home visits sensibly across the day, taking account of clinical urgency and geography across the four practices.
Care Home Support
Support care home ward rounds, working alongside GPs and care home staff to review and manage the needs of care home residents. Identify and proactively manage patients at risk of deterioration or hospital admission, in line with locally agreed pathways.
Minor Illness Clinics
Run minor illness clinics, providing assessment, diagnosis and treatment at first point of contact for patients presenting with minor and same-day illness. Diagnose and manage acute and chronic conditions, combining drug and non-drug treatment approaches into a coherent management plan.
