The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting,
ensuring the highest standards of care for all registered and temporary patients.
About us
Stonecot Surgery provides general practice services to approximately 10200 patients. Recently rated as outstanding by CQC and a finalist in the upcoming HSJ awards in the Primary care provider of the year category for its Long Term Condition workflow management. The practice population is in the second least deprived decile in England.The practice operates from a 3-storey converted residential premises. A small amount of car parking is available at the practice, and there is space to park in the surrounding streets. The practice has recently undergone a full refurbishment and now has 12 clinical rooms and a floor for administrative staff.
The practice team at the surgery is made up of three GP Partners, 5 Salaried GPs, 2 Clinical Pharmacists, 2 Practice nurses, 1 HCA and a physicians associate. The practice is a training practice for GPs, nurses, FY2 doctors, PAs and Pharmacists.There is a strong emphasis on support and education alongside personal development.

SPECIFIC DUTIES/RESPONSIBILITIES
Clinical Responsibilities:
In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self
available to undertake a variety of duties including surgery consultations, telephone consultations
and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with
queries, paperwork, reports and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether selfreferred or referred from other health care workers within the organisation.
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols,
developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to other care providers as
appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Collecting data for audit purposes and complying with QOF requirements.

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written
