Main duties of the job
As a member of the Practice Clinical Team you will provide primary healthcare services to patients in all required environments. You will be expected to participate in Practice activities including clinical and staff meetings, audits, and relevant CPD activities; you will also take a role in the development of the Practice. In addition, you will be expected to participate in the Practices activities relating to its function as a training practice.
Daily Clinical Responsibilities:
You will be expected to undertake consultations in all clinical settings as directed during your paid clinical time. These consultations will cover the breadth of the Practices GMS and network obligations and you will be expected to support the Partners in providing such services as outlined in the GMS Contract and any national, directed or local enhanced services agreed with the ICB or any successor body.
You will be required to share the daily responsibility of checking and signing prescription requests, dealing with patient queries and providing clinical supervision to the trainee's in place at the Practice.
You will be expected to support and input into the aims of the Practice in areas including clinical governance, best practice, clinical audits and any other improvement activity as determined by the Practice.

We are a large, well-established and thriving teaching Practice seeking to appoint a salaried GP to join our friendly team. The right candidate will be an enthusiastic and motivated doctor with an interest in enhancing our team and providing high quality, patient-centred care.
We are based in New Brighton, Wirral, close to Liverpool and Chester. We have 6 partners, 4 salaried GPs, ANP, Practice nurse team, HCAs, phlebotomist and supportive reception, admin and medicines team and we work along side other roles attached to our PCN.
Eligibility to work in the UK, full GMC registration, placement on the Medical Performers List, membership of a recognised medical indemnity organisation, two satisfactory references and clear DBS certification are essential conditions of employment.

Job summary
We are a well-established, popular and friendly practice at the heart of the local community in Hendon, London NW4 and are seeking an experienced Prescribing Pharmacist to join our forward-thinking clinical team.
We are a training and teaching practice based on a single, practice-owned site, with approximately 12,000 patients and a full range of clinical and non-clinical team members. We are a major contributor to a successful and supportive PCN, have an active group of Patient Champions, are QOF high achievers and are rated GOOD by CQC, with some outstanding points of note.
We are looking for an experienced pharmacist who is an Independent Prescriber and is confident in managing patients with long-term conditions and complex medication needs. A particular interest and experience in diabetes management would be highly desirable and would be considered a significant advantage.
The successful candidate will work closely with our GPs, nursing team and wider multidisciplinary team and will have the opportunity to play an important role in the ongoing development of medicines management and long-term condition care within the practice.

Job Summary
St. Georges Medical Centre is seeking an experienced Clinical Pharmacist who is a qualified Independent Prescriber to join our multidisciplinary primary care team.
The post holder will work closely with GPs, nurses, HCAs, pharmacists and other members of the practice team to provide high-quality, safe and effective medicines management and clinical care.
The role will include managing patients with long-term conditions, undertaking structured medication reviews, medicines optimisation and independent prescribing within the post holders clinical competence.
We are particularly interested in appointing a pharmacist with experience or a strong clinical interest in diabetes, who can contribute to the management and optimisation of treatment for patients with Type 2 diabetes and other associated long-term conditions.

Job summary
We are a well-established, popular and friendly practice at the heart of the local community in Hendon, London NW4 and are seeking an experienced Prescribing Pharmacist to join our forward-thinking clinical team.
We are a training and teaching practice based on a single, practice-owned site, with approximately 12,000 patients and a full range of clinical and non-clinical team members. We are a major contributor to a successful and supportive PCN, have an active group of Patient Champions, are QOF high achievers and are rated GOOD by CQC, with some outstanding points of note.
We are looking for an experienced pharmacist who is an Independent Prescriber and is confident in managing patients with long-term conditions and complex medication needs. A particular interest and experience in diabetes management would be highly desirable and would be considered a significant advantage.
The successful candidate will work closely with our GPs, nursing team and wider multidisciplinary team and will have the opportunity to play an important role in the ongoing development of medicines management and long-term condition care within the practice.
We offer flexible working hours and are keen to find the right experienced pharmacist to complement our existing clinical team.

Job Summary
St. Georges Medical Centre is seeking an experienced Clinical Pharmacist who is a qualified Independent Prescriber to join our multidisciplinary primary care team.
The post holder will work closely with GPs, nurses, HCAs, pharmacists and other members of the practice team to provide high-quality, safe and effective medicines management and clinical care.
The role will include managing patients with long-term conditions, undertaking structured medication reviews, medicines optimisation and independent prescribing within the post holders clinical competence.
We are particularly interested in appointing a pharmacist with experience or a strong clinical interest in diabetes, who can contribute to the management and optimisation of treatment for patients with Type 2 diabetes and other associated long-term conditions.
Main Duties and Responsibilities
The post holder will: -
- Undertake patient-facing clinical medication reviews and Structured Medication Reviews.
- Independently prescribe, amend and discontinue medication within their recognised scope of competence.
- Manage patients with long-term conditions in accordance with evidence-based guidance and agreed practice protocols.
- Provide particular support for the management of patients with diabetes, including medication reviews, treatment optimisation, monitoring and appropriate follow-up.
- Review patients taking multiple medications and identify opportunities to reduce inappropriate polypharmacy.
- Review patients with complex medication regimes and those at increased risk of medicines-related harm.

- Monitoring medication effectiveness, side effects and adherence.
- Supporting achievement of individualised HbA1c and other treatment targets.
