Job summary
PHL Group (Partnering Health Limited) is looking for an experienced Clinical Governance Manager to join our Clinical & Quality team. This is a pivotal role, providing assurance that our services are safe, compliant, and delivering high-quality care to our patients.
Reporting to the Head of Quality and Clinical Governance, you'll lead on clinical governance processes across PHL, working closely with service leadership teams on incident reporting, risk and complaints management, and quality improvement all delivered in line with our truly caring and team first values.
This is a full-time hybrid position, working 37.5 hours per week, Monday to Friday.
Main duties of the job
Key Responsibilities
Acting as a visible, approachable leader within the Clinical & Quality Team, supporting and inspiring colleagues across the organisation
Ensuring up-to-date knowledge of the clinical governance framework, including relevant legislative, regulatory and professional standards (CQC, NHS England, NICE, CAS alerts)
Producing patient safety and governance reports for services, committees and forums, escalating and actioning improvements where needed
Supporting investigations into patient safety incidents and potential litigation claims, ensuring learning is embedded and shared
Maintaining and monitoring the risk register, providing expert advice on clinical risk management
Overseeing complaints handling via Datix, ensuring timely investigation, response and learning
Researching, writing and embedding clinical and governance policies and procedures
Chairing a delegated sub-committee and supporting delivery of governance meetings across services
Supporting compliance with ISO 9001 quality management requirements

Assist the Director of Nursing and Quality in reviewing and monitoring the key strategic objectives and risk profile related to quality and safety, which are aligned to PHL priorities for quality improvement.
Ensure any safety alerts and national guidance is implemented in a timely and appropriate way, ie CQC standards, NHS England policies, CAS alerts and NICE guidelines.
Provide patient safety and governance reports for each service, relevant committees and forums, collating key quality indicators to support assurance mechanisms to demonstrate compliance with key standards and to escalate and action where improvements need to be made.
Working with the Patient Safety Specialist, service managers, clinical managers & leads, support with investigations into patient safety incidents and potential litigation claims, ensuring they are appropriately investigated and responded to in accordance with PHL policy, learning is cascaded and embedded.
Support delivery of local and national patient safety frameworks by ensuring that staff involved in incidents are supported, debriefed and offered individual feedback.
Support service managers, clinical managers and clinical leads when meeting with patients and their families and any external agencies affected by adverse incidents.
Actively support and promote a transparent learning culture just culture.
Provide feedback to staff, through various methods; newsletters, presentations and one to ones.
Working with the Service Managers and Senior Management Team ensure that the Risk Management Strategy is effectively conveyed to all staff and is translated into operational practice.
Support Senior Leadership Team and Services leaders to ensure the maintaining of the risk register accurately reflects risks and is up to date.

Job summary
PHL Group are looking for a Forensic Healthcare Professional working in custody suite covering the North Yorkshire region working alongside police officers to assess and treat individuals detained in Police Custody. Full training will be provided.
We are currently looking FHCPs to work on a part timebasis covering between 23 and 32hrs per week.
PLEASE NOTE YOU MUST BE A REGISTERED RGNOR PARAMEDIC
Could you identify if a detainee is suffering from a mental health condition? If they are uncooperative or lacking diabetes medication? If they are a substance misuser or suffering from a head injury? Our Forensic HCPS are responsible for assessing and addressing the health needs of detained persons, as well as conducting forensic medical examinations gathering evidence for use in the criminal justice system.
Main duties of the job
You will be conducting clinical assessments, identifying, and implementing appropriate interventions and collecting forensic samples. Providing advice and guidance to ensure appropriate care is maintained and keeping detailed and accurate records to ensure the health, safety and welfare of detained persons held in police custody.

Forensic Healthcare Professional Skills and Experience:
- Effectively deliver the requirements of the forensic practitioner service following the Codes of Practice of the Police and Criminal Evidence Act 1984 and locally agreed protocols.
- Keep accurate, complete and contemporaneous notes of work carried out, including the recording of injuries.
- Provide advice and support to Custody Officers in determining the health care needs of detainees and the priorities of care. Seek telephone advice and refer cases to the Consultant FME when required.
- Provide a professional, responsive and caring service to detained persons in the custody environment to support the Custody Sergeant and Officers in assessing the healthcare needs of detained persons and an effective care pathway throughout their time in custody.

Job summary

PHL Group are commissioned by Staffordshire & Stoke-on-Trent Integrated Care Board to provide out-of-hours (OOH) clinical services across the county of Staffordshire. Our service offering includes telephone triage, home visiting and patients visiting one of our seven primary care centres. These services are delivered between 18:30 08:00 Monday to Friday and from 08:00 08:00 across weekends and bank holidays. Service operating times are variable based on patient demand and access to premises.
The post holder will support the three commissioned services, ensuring they are safe, efficient and patient-focused. They will work closely with the Medical Lead, Clinical Lead and Business Delivery Manager and ensure patients receive optimal care in the correct setting within the healthcare system.
The service is staffed by a team of GPs, Advanced Nurse/Clinical Practitioners, Nurses, Paramedics and a non-clinical workforce.

Essential
- Minimum 5 year post registration experience
- 2 years experience of working as a practitioner in a primary care environment, Emergency Department, Urgent Care Centre, MAU/EAU or other first contact setting, evidence of excellent assessment skills
- Experience of having provided mentorship and/or coaching to other staff
- Experience of having participated in service improvement activates including audit and research
Qualifications
Essential
- Registered Nurse First Level Nurse or other healthcare professional qualification leading to registration
- Evidence of degree level study or equivalent
- Degree in advanced practice or equivalent (to include research an evidence to inform practice, clinical leadership, work-based learning, clinical history taking and examination skills, pathophysiology, investigations and immediate management, non-medical prescribing)
- Evidence of completion of study in minor illness
- Evidence of study in minor injury
- Evidence of completion of study in paediatric minor illness
- Evidence of study in paediatric minor injury
- Masters level study in advanced practice
- Independent/supplementary prescriber
Desirable
- Evidence of study to conduct paediatric clinical examination
- Teaching/training qualifications such as ENB998 or equivalent
