Applicants are required to have hospital/primary care or community care experience and be competent at taking blood pressure, ECGs and other observations. Knowledge of wound care is also a requirement for this post.
Main duties of the job
As a Healthcare Assistant working within the Substance Misuse Team you will assist in the delivery of high quality nursing care to patients in prison with the support of a registered nurse.
You will be providing physical care to patients according to a written care plan. This will include withdrawal observations, ECG's and wound care as well as supporting medication rounds.
You will be able to demonstrate good communication skills and be able to confidently liaise with other services to effectively support the patient throughout their time in prison setting.
You will be actively engaging with patients in the provision of holistic, needs-led care which will consider the physical, psychological, emotional, social and spiritual needs of the individual.
You will be promoting health and well-being through the delivery of health promotion activities and programmes, responding to patients needs and assisting patients to attain optimal physical health.

We are pleased to offer an exciting role to work in the Bexley COPD service within Oxleas NHS Trust. The team provide acute support and chronic disease management of COPD within a community setting. Within the team we also provide a quality assured diagnostic spirometry service for COPD and manage/ongoing review of patients on home oxygen therapy.This post is ideal for a suitably skilled & trained respiratory clinician.
This a community post requiring travel across the Borough, the post holder should hold a full driving licence & suitable vehicle for work purposes.
Main duties of the job
The role will include working as part of the team and participating in the leadership of the home oxygen service. This will include the management of a caseload and regular review of patients with reference to local key performance indicators.
You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.
We offer regular in-service training and clinical supervision as well as the opportunity to be involved in student education, training and supervision of our newly qualified staff. There is also support and development through regular appraisals, a personal development plan and regular formal supervision.

Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing, patients on the caseload. The post holder will be expected to be a competent leader and case manager and critical thinker navigating the patient through the appropriate care pathway which spans across the primary, secondary care interface. Pivotal to the care pathway is the prevention of unnecessary admissions and facilitation of early supported discharge to reduce lengths of stay.To work closely with the rest of the team to reduce patients waiting times throughout their journey by reducing unnecessary consultant outpatient attendances.
The service provides assessment to patients diagnosed with chronic lung disease registered with a Bexley GP across various community settings and in their own homes.
Key Task and Responsibilities
The Respiratory Specialist Clinician will be expected to achieve the competencies set nationally by Skills for Health 2005 (www.skillsforhealth.org.uk). These can be summarised as follows;
Utilisation of advanced clinical skills and expert knowledge to plan, implement, monitor and review individualised patient-directed programmes of care to patients with chronic lung disease, home oxygen therapy management and health promotion interventions. To undertake goal centred interventions that are evidence-based, evaluating the effectiveness of these at regular intervals with a view to improving care continuously. Develop risk management plans that will enable individuals to remain safely within their homes Use expert knowledge and skills as a nonmedical prescriber to manage patient medication within own areas of competency, developing clinical management plans in partnership with GP colleagues where necessary. Anticipate individual care needs and develop appropriate care plans, involving carers and relatives To provide individuals and their carers with the necessary education that will enable them to plan for unavoidable disease progression and self manage as far as possible. To work collaboratively with members of the multi professional team to ensure that the patients needs are met, making appropriate referrals are made where necessary. EFFORT AND ENVIRONMENT
Moderate physical effort is necessary for this post including regular periods of driving, frequent treatment and personal care and occasional movement of patients. The Mental effort required for this post includes in depth mental awareness to carry out interpretation of clinical assessment and decision making of complex clinical interventions. The emotional effort for this post involves frequent exposure to emotional circumstances, in particular care of patients and their carers at the end of life. Working conditions for this post include working in the patients own home with frequent exposure to body fluids and some exposure to containable hazards, eg need for personal alarms The mental effort required to interpret and react accordingly to patients who are acutely unwell.

Job summary
We have an exciting opportunity for two registered mental health nurses to join our Early Days in Custody Team (EDiC) at HMP Swaleside.
The EDiC team will act with the following principles:
Holistic - the EDiC team is multi-disciplinary, drawing on expertise from the other teams within healthcare to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed.
Curious - the EDiC team will engage with patients with clinical curiosity - drawing on all the information available to them to understand patient needs. The team will engage where possible with patients' families/carers to fully understand patient's needs and assist in helping maintain family relationships.
Prioritising Safety - Keep prisoners safe by identifying and addressing immediate risks for that patient including self-harm, risk of death by suicide, and unmanaged substance misuse dependencies.

Main duties of the job
As a EDiC Practitioner you will:
- Ensure that new prisoners are supported during their first 14 days in the prison to access relevant healthcare offers and manage their general health and wellbeing whilst in prison.
- Identify the wider needs of patients, through the completion of a comprehensive assessment and working in partnership with them to co-produce a written care plan that addresses their health and social care needs.
- The role will include covering some work for the mental health in-reach team. For example:
- Care planning
- Caseload reviews
- Administer antipsychotic depot medication.
- Collect and synthesize all relevant clinical history to ensure safety, continuity of care and minimal re-assessments for the patient and ensuring that the records held for patients are up to date and accurate.
- Ensure ongoing care for every person requiring it, with clear pathway management in place that is understood by the patient and their families /significant others.
- Information is shared where appropriate and consent has been given to ensure that care plans and risks for patients are widely understood by all staff supporting them, including HMPPS staff where appropriate. This will be done with full patient consent.

Oxleas Heart Failure Service is a dynamic, highly experienced & award-winning service. This is an exciting opportunity for a highly skilled & motived nurse to join our team & develop as a heart failure specialist nurse.
This is a community based role caring for residents with heart failure living in the London Borough of Bexley. Care is provided in both clinic & home environments.
This post is ideal for a suitably skilled & trained cardiac nurse who is keen to develop as a heart failure nurse. You will be supported in this post to develop skills such as advanced clinical assessment, medication reviews & optimisation, patient education & monitoring. You will also be involved in the end of life care of the heart failure patient.
You will need excellent clinical & communication skills & an ability to work with patients with diverse health & social needs. The post holder will work closely with members of the wider multi-disciplinary team in primary & acute health care settings. The post holder will also work closely with the Cardiac Rehabilitation Team to ensure patients are provided with sufficient support with their cardiac diagnosis.
This a community post requiring travel across the Borough, the post holder should hold a full driving licence & suitable vehicle for work purposes.

Main duties of the job
The Heart Failure Specialist Nurse will be a key member of the Long Term Conditions team, responsible for the delivery of expert community cardiac care and in particular, heart failure.
The post holder will have their own caseload of patients and take on direct responsibility to deliver high quality, holistic care to local residents, ensuring that patients have equal access to services that are responsive, personalized and patient-led.

The post holder will contribute to monitoring and improving standards of heart failure care in accordance with NICE and ESC guidance.
The post holder will contribute to Clinical Governance within the service by contributing to audit as part of the heart failure team.
To deliver evidence-based practice in line with clinical governance, trust policies and procedures and NMC guidance.
Contribute to audit of practice, critically evaluates research findings, and contributes to clinical practice to improve outcomes for cardiac patients.
To monitor standards through the process of clinical audit in line with the Trusts objectives, reporting all research and audit activity and audit progress of patient caseload and service development to the Directorates Management Team.
