Main duties of the job
We're looking for caring, dedicated Healthcare Assistants who hold a Level 3 Care or Lead Adult Care Worker qualification and have experience in a healthcare setting. Community experience is helpful, but you don't need previous palliative or end-of-life care experience. We'll provide the training, support and encouragement you need to develop your skills and thrive in this rewarding role.

The service operates seven days a week, with 12-hour shifts from 8am to 8pm, including a one-hour break. As we continue to grow and develop our services, including plans for future night-time community care, flexibility will be important. As this role involves travelling to patients' homes across North Devon, you'll need to be able to travel independently throughout the area.

In this role you will be responsible for:
- Providing high quality, holistic care to end of life patients in their own home in line with the care plan and risk assessment as defined by the Community Palliative Care Registered Nurse
- Working closely with the hospice team and the patients District Nurse and GP to support coordinated care
- Participating in the provision of care to facilitate patient / carer choice regarding palliative and / or end of life care

Our Hospice to Home Service is designed to be responsive and flexible to meet the needs of people who wish to remain at home. We have a great opportunity for an experienced healthcare assistant to join our team and provide expert care to people across North Devon who need our support at end of life
Main duties of the job
The Hospice to Home service is a 7-day service. You will be expected to work 12-hours shifts, 8am 8pm with an hour break. While we do not currently deliver care in the community at night, our ambition is to reintroduce a night service as part of our new 5-year strategy. You should therefore be able to be flexible to meet future service demands. The role will involve working as part of the wider hospice services and collaboratively with district nursing teams to deliver individualised care to patients in their own homes. As this post is based out in the community applicants must be able to travel across the North Devon area without reliance upon public transport.
