Job summary
The post holder will work as a Salaried General Practitioner within The Limes Medical Centre, providing high-quality, safe and patient-centred medical care to the practice population.
The successful candidate will work collaboratively with the practice team and wider multidisciplinary team to provide effective management of acute and chronic conditions, preventative healthcare and appropriate onward referral.
Main duties of the job
Clinical Duties

Job summary
Wombwell Medical Centre is looking to recruit a salaried GP to work 6-8 sessions a week.
We have a CQC rating of Good and a list size of nearly 10,000 patients. We are committed to excellent patient care and are seeking a motivated professional who shares these values.

Main duties of the job
Work as an integrated member of the practice team. You will see a wide range of primary care problems in pre-booked appointments. Duties will also include one day a week on-call during surgery hours. Equal share of non-clinical duties, such as admin and lab reports.

This is an exceptional opportunity to lead a nationally recognised Point of Care Testing service.
Berkshire and Surrey Pathology Services (BSPS) is one of the country's largest thriving NHS pathology networks, providing pathology services across six acute hospitals and associated community services. Our POCT service has been developed over many years and has established a strong national reputation for quality, innovation, clinical partnership and professional leadership.
In 2025, the BSPS POCT team won the NHS England Chief Scientific Officer Award for Outstanding Healthcare Science Service of the Year at the Advancing Healthcare Awards, recognised as a national leader in training, governance and best practice. The service delivers more than 56,000 tests each day across 1,380 devices, using a workforce of 85 staff, and was the first multi-NHS Trust POCT service to achieve ISO 15189:2022 accreditation.
Main duties of the job
As Consultant Clinical Scientist and POCT Specialty Lead, you will provide scientific, clinical and strategic leadership to a multidisciplinary POCT team of 85 staff, working across BSPS acute hospitals and community services, including three ICBs, seven community hospitals and 15 Virtual Ward and Hospital at Home services. You will also become Director of Qpoint, BSPS's established POCT External Quality Assessment Scheme, with responsibilities extending locally, nationally and internationally.
You will lead the strategic development of POCT across BSPS, and further develop POCT across acute, community, Virtual Ward and Hospital@Home services. You will develop innovative clinical pathways that bring high-quality diagnostics closer to patients, and maintain and further develop our ISO 15189:2022 accredited service. You will lead and develop a highly skilled POCT workforce, and drive research, development, audit, digital connectivity and adoption of new technologies. You will influence POCT strategy across multiple NHS organisations, and develop the national and international profile of BSPS POCT and Qpoint, working with clinicians, healthcare organisations, professional bodies and industry to advance the effective use of POCT.

Main duties of the job
The post holder will provide safe, effective and patient-centred general medical services as part of our multidisciplinary team.
The role will include managing a varied clinical workload and providing care for patients with acute, urgent and ongoing health needs. You will undertake face-to-face, telephone and other appropriate consultations, assessing, diagnosing and managing patients across the full range of general practice.
An important part of the role will be supporting same-day care. This will include seeing patients with acute problems and acting as the clinical triaging GP for Anima Total Triage during allocated sessions. You will review patient requests, assess clinical need and urgency, and ensure patients are directed to the most appropriate clinician or service.
Alongside acute care, there will be opportunities to support patients with long-term conditions and complex health needs through our New Models of Care work. You will work closely with GPs, nurses, pharmacists, paramedics, healthcare assistants and other professionals to provide coordinated, proactive and patient-centred care.

An exciting new opportunity has arisen within Central Camden Primary Care Network (PCN) for a Care Coordinator to manage patient caseloads and provide coordination and navigation of care and support for patients across health and care services.
Central Camden PCN are a group of 8 member practices who work together at scale and collaborate with others in the local health and social care system. The PCN is highly innovative and delivers services with support from the GP Federation, Camden Health Evolution (CHE) Ltd, that aim to improve the quality of care and outcomes for their patients.
This is a part-time position requiring a minimum of 15 hours and up to 22.5 hours per week, including the core hours specified below. The successful candidate will be expected to work across a range of locations, including the CHE office, PCN practices, and local community venues, in line with service requirements.

Main duties of the job

The Associate Dean (Primary Care) will support the Primary Care Dean and GP School in setting the strategic direction and development of clinical and professional education in Primary Care across the multi-professional primary care workforce within the Midlands.

Icknield PCN is looking to welcome a qualified and experienced clinical pharmacist into the PCN to play a full and active role within our pharmacy team and to also provide clinical leadership, including ongoing service development, supervision and line management.

Role Summary
The Salaried GP will provide high-quality, safe and effective medical care to the practice population, working as part of a multidisciplinary primary care team.
The role involves assessing, diagnosing and managing patients presenting with a wide range of acute, chronic and complex health conditions, while ensuring appropriate onward referral and continuity of care.
- The postholder will work collaboratively with GPs, Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants, pharmacists, reception/administrative staff and other members of the wider healthcare team.

The Practice currently cares for just under 9000 patients with a team of 3 Partners, 2 salaried GPs, an ANP, 2 Practice Nurses, a Practice Pharmacist, HCAs and a mix of 10 reception/admin staff.
We are part of East Elmbridge PCN, along with 6 neighbouring practices.
We are a training practice and delight in welcoming new students. We also hold monthly staff meetings, open to all staff, where we share information, ideas and news. We also provide the lunch!
Located in East Molesey, Surrey, with parking provided.

Clinical responsibilities:
Autonomously assess, differentially diagnose, plan, implement, and evaluate individual treatment plans, interventions, and care packages for patients, ensuring safe and appropriate management, timely referrals, and safety netting as required.
Collaborate with the multidisciplinary team (MDT) to expedite safe and timely triage consultations, interventions, and reviews.

Main duties of the job
The Salaried GP will provide high-quality general medical services to the practice population, working as part of the wider multidisciplinary team. Key responsibilities will include:
- Providing routine and urgent GP consultations, both face-to-face and remotely.
- Holding responsibility for a defined patient list, supporting continuity of care.

4Doncaster PCN is seeking an enthusiastic and motivated Clinical Pharmacist to join our expanding Primary Care Network team.
You must be a qualified Clinical Pharmacist and be registered with GPhC.

Job summary
Seacroft Primary Care Network (PCN) are hoping to expand the PCN Pharmacy team and are looking for a highly motivated and enthusiastic Prescribing clinical Pharmacist to join the team. The PCN pharmacy team currently consists of a Lead Pharmacist, 3 other pharmacists and a Pharmacy Technician. The team is extremely well integrated into the PCN and their work is highly valued and recognised by all member practices. All the PCN roles are well supported by the Clinical Director.
You will be working across Seacroft PCN which are a group of 4 well-led practices that work together to deliver excellent care to our patient population of 35,000. The practices are Foundry Lane, Park Edge, Oakwood Lane and Windmill located in LS8 and LS14.
This role will be a hybrid of remote working and being in practice. It is an exciting opportunity to use and develop your clinical knowledge and skills to contribute to all aspects of medicines optimisation and improve the lives of patients across the PCN.
Applications from all pharmacy sectors are welcome however, experience and knowledge of working in primary care would be desirable. Being a prescriber is essential. All PCN pharmacy staff are required to enrol on (or be exempted from) the 18-month CPPE Primary Care Pharmacy Education Pathway. If this is not something you have completed, you will be allocated a clinical supervisor in the PCN to complete the training. You are encouraged to get in touch to find out more about the role.
Main duties of the job
The main aim is to improve the outcomes for patients from their medicines and reduce the risk of harm. Part of this will involve improvingmedicines safety and medicines management processes across the PCN such as contributing to actioning MHRA alerts and reconciling of medicines upon transfer of care.
You will carry out structured medication reviews (SMRs) to optimise medication for complex patients, in particular the frail, polypharmacy and care home residents. This may involve home visits for the housebound which can be done jointly with the PCN care co-ordinators in the frailty team. We have a structure in place for working smoothly with the many care homes across Seacroft.
There are multiple opportunities to join established MDTs locally to deliver proactive, personalised and coordinated health and social care. Partnership working across all care sectors and effective communication will be required.
