We are looking for an enthusiastic, hardworking, team player, for 6 sessions per week. Salary negotiable dependent on experience.
We are a practice who encourage a wide skill mix across the whole clinical workforce, and expect all clinical team members to work collaboratively.

Main duties of the job
We are looking for a dynamic, professional and friendly GP to join our practice team to continue to deliver the high standard of clinical care we currently provide.
The post holder must be dedicated, responsible, and driven to contribute to the innovative and forward thinking practice direction, and be a key team player.
They must have an awareness of and compliance with all relevant practice policies/guidelines, e.g. safeguarding, prescribing, confidentiality, data protection, health and safety etc.
Excellent communication skills and a friendly attitude are essential.
Contract:
6 sessions per week
Mix of routine work, on-call
10 minutes appointments
Typical working day would aim to finish no later than 6/6.30pm
Preferable working days would be discussed on appointment.
Some evening and weekend may be required occasionally
Flexibility around start and end times essential

We are a practice close to historic Shrewsbury town centre with easy access to the motorway network. We are a forward thinking practice with 10,000 patients and are committed to providing them with high quality care.
We are a close working team which comprises of 4 GP partners, two salaried GPs, 2 Advanced Clinical Practitioners, 3 Practice Nurses, 1 General Practice Assistant, and a team of dedicated Care Navigators who support our clinical team on a daily basis. We also have a range of Primary Care Network Advanced Role staff that support us.
We have an experienced and very supportive administrative team led by our Practice and Deputy Manager

Job summary
As an Advanced Clinical/Nurse Practitioner (ACP/ANP), you will play a key role in providing advanced clinical assessment, diagnosis and treatment to our patients. With the creation of 2020/21 Network Direct Enhanced Service (DES) and the introduction of the NHS Additional Role Reimbursement scheme for new primary care roles, an opportunity has arisen for Primary Care Networks (PCNs) to bring in roles that support GP practices, Networks and elements of the Network DES.
This role will contribute to tackling healthy lifestyle issues in Shrewsbury and surrounding areas, predominantly via the delivery of the NHS Health Check Programme. The NHS Health Checks will be delivered to individuals across the Practice population in a variety of settings, offering tailored advice and encouragement to make behavioural changes accordingly, as well as onward referral to other relevant services.
Main duties of the job
The post holder may work with seldom heard communities who experience inequalities in health, such as asylum seekers, the homeless, ethnic minority groups and those living in areas of high deprivation. They will have responsibility for reaching out to, and providing lifestyle input into this cohort of patients. They will work closely with the other members of the primary care team and complex care teams in the management and decision making about care and service provision for individual patients. This will include:
- Engage with and invite individuals for the NHS Health Check programme, to the programme, offering a quality service that provides tailored advice and encouragement to make behavioural changes accordingly.
- Care planning, health coaching and delivery of systematic self-management support based on knowledge of individual's activation levels.
- Support for individuals to access appropriate community resources and services.
