Main duties of the job

• Deliver diabetes reviews and ongoing management.
• Provide patient education and lifestyle advice.
• Monitor and optimise diabetes treatment in line with national guidelines.
• Undertake chronic disease management clinics.
• Administer vaccines and immunisations.
• Perform cervical screening, wound care and health promotion activities as appropriate.
• Maintain accurate clinical records and contribute to quality improvement initiatives.

Our multidisciplinary team includes experienced GPs, Advanced Clinical Practitioners, Practice Nurses, Nurse Associates, Healthcare Assistants, Phlebotomist, Clinical Pharmacists, First Contact Practitioners, and an excellent administrative and reception team.
