Job summary
Interested in an exciting clinical prescribing role working to delivery the new Integrated Neighbourhood model of care within Lewisham Borough as part of the Lewisham Integrated Medicines Optimisation Service (LIMOS)?
We are looking for one enthusiastic senior pharmacist with strong established clinical skills to join the already established Lewisham Integrated Neighbourhood Team Programme and improve outcomes for patients with multiple long-term conditions in Lewisham.
INTs are a model of care designed to meet the holistic needs of the local population, working with professionals and as a local community to ensure people get the right care at the right time, from the right people and to tackle health inequalities. This post will be key in ensuring delivery of this new model. As the programme will focus on patients with three of more long term conditions around CVD, diabetes and AF in the first instance, experience of management of patients in these areas would be beneficial.
Main duties of the job
The Senior Clinical Pharmacist Prescriber will be responsible for providing clinical care to conduct patient facing multi-morbidity reviews and optimise medicines management for patients across one or two Neighbourhood footprints. A key part will be close working with and supervision of non-clinical members of the core INT team including caseworkers, Health Coaches and link workers. Close working and communication with these members of the core INT and wider teams across both health and social care and primary and secondary care settings to ensure effective patient management and delivery of the INT programme will be key.

Key Result Areas & Performance:

2.To work closely with colleagues and other staff involved in the INT programme to promote and embed the role of the INT into local pathways of care.
3.To provide operational and clinical support to developments around multi-morbidity and delivery of the INT model of care.
4. To support the planning and actions required to continuously improve the quality of service provided and ensure a quality improvement approach is taken to medication optimisation and any service development or re-design.

Clinical Services & Patient Care
1. To act as a clinical role model to other members of the wider MDT and provide direct supervision to non-clinical members within the core team in allocated Neighbourhood(s) to ensure improved outcomes achieved through the provision of holistic, team approach to care, which includes the safe and clinically cost-effective use of medicines.
2. To provide clinical leadership on medicines optimisation in the context of multi-morbidity across the core INT and wider setting.
3.To conduct patient facing and telephone consultations to independently review patients with complex multi-morbidity focusing on optimisation of cardiovascular disease management and improving long-term outcomes.
4. To collaborate with multidisciplinary teams (MDTs) to develop and implement individualised care plans.
5. To provide clinical advice and support to other members of the INT including complex scenarios to ensure a high standard of patient care delivered within the INT framework.
6. To manage complex and non-routine medicines management problems in patients under the INT service, making onward referrals when needed.
7.To demonstrate an intuitive grasp of situations based on knowledge, expertise and understanding and to make decisions about drug therapy with limited information.
8.To work with colleagues to facilitate robust communication and onward referral pathways to support seamless care for patients managed by the INT service where complex medicines and/or other needs are identified.
9. To ensure efficient transfer of information related to complex medicines management needs across care interfaces and reconcile information received to ensure the safe transfer of care for vulnerable patients with complex medicines management needs.
10. To work as an independent prescriber and review medication to ensure that patients medicines are optimised for safety, effectiveness and cost-efficiency in line with competency and agreed scope of practice as approved by the relevant Trust governance process.
Information & Data Management
1. To contribute to the development of quantitative and qualitative performance measures, co-ordinate their use and be involved in disseminating, acting on and sharing learning from information gathered.
2. To participate in the design and collection of clinical audit data, leading implementation in designated areas and participate in research projects to improve the quality of care and inform best practices.
3. To utilise available information from audits, national guidelines, and benchmarking data to influence and implement improvement in service delivery.
4. To work closely with PCNs ensuring seamless integration of patient data for improved care co-ordination.
Staff Management and Education and Training
1. To line manage one to two caseworkers and link workers/health coach within the INT providing supervision, guidance and support to ensure effective teamwork and delivery of goals. This will include supporting appraisals, objective setting and sickness management.

To work with the Principal Pharmacist and other leads to identify and support the design and delivery of education and training around the INT service (including medicines optimisation in multi-morbidity and holistic, cost-effective prescribing), to colleagues, and more widely across the system when needed. Training delivery will range from small communications to formal presentations.
3. To participate and contribute to clinical training programmes for trainee professionals across Lewisham where exposure to the INT model would aid learning and practice.

Following promotion of the existing postholder, an exciting vacancy has arisen for a motivated, patient centered pharmacist with strong clinical skills to work across both the interface and community setting as part of the domiciliary care arm of the Lewisham Integrated Medicines Optimisation Service.
This is a unique opportunity to work in an award winning service and gain experience in working with older people across care systems to improve independent and safe use of medicines whilst addressing challenges around overprescribing.
Responsibilities includes providing direct patient care and holistic patient centered structured medication reviews to proactively manage people with complex polypharmacy and adherence concerns, in older people and those with multiple long term conditions. The role involves visiting patients both in the hospital and home setting as an integrated approach as part of existing pathways of medicines support in Lewisham and further evolving models of Neighbourhood Care.
This post requires close partnership and team working with members of the MDT to co-ordinate medicines related aspects of care across both health and social care interfaces.
Whilst experience in primary care is not essential, an interest in older people, strong communication skills and lots of enthusiasm to working as part of multidisciplinary approach care are essential.
Main duties of the job
The award winning LIMOS service provides a formal pathway for the referral of patients with medicine-related problems for assessment, support and follow-up by a specialist pharmacy team, working across primary and secondary, and health and social care interfaces. Joint polices around medicines support across health have enabled a collaborative approach to care across the pharmacy and wider workforce.
As a patient facing role, the role offers a great opportunity to utilise and further develop strong clinical skills as an independent prescriber as part of a Neighbourhood team approach to care.
Being supported by a network of pharmacists and pharmacy technicians, this post is an exciting opportunity to work in the primary care and interface setting. but maintain strong links with pharmacy colleagues in both secondary and community settings. This close working also includes the ability to access continuing professional development via peer learning events with pharmacy professionals within both secondary care and community settings.
The postholder would work across the Borough, delivering care to an allocated neighbourhood and be based at either South Lewisham Health Centre or University hospital Lewisham.
As a patient facing role, regular home working is not included within this role. However some hybrid working may be required when working in the community setting.

Main duties of the job
This is a new post and would suit a consultant who is wishing to develop their role within a department that has a broad range of community paediatric experience. The post holder will work in all aspects of Community Paediatrics, suite to their experience, competencies and the priorities of service provision.
There is no overnight commitment. The postholders will contribute to the daytime child protection rota, but there is no expectation to contribute to Childhood Sexual Abuse (CSA) medicals.
The postholder will join a team of 9 Consultant Community Paediatricians, 2 community SAS doctors and one Speciality Doctor. Senior therapists and clinical nurse specialist also contribute to assessments and care for young

We are seeking highly motivated individuals to join our friendly Outpatient administration team to deliver first-class customer service to patients to provide a comprehensive administrative service working flexibly on rotation incorporating the Outpatients Booking Office, Call Centre and Reception areas forming a solid foundation to support the Outpatients departments, across Lewisham Hospital, Queen Elizabeth Hospital and Bexleyheath and Greenwich Community sites.
The post holder will be one of a team providing an excellent customer and administration service, delivering smooth, safe and high quality care to enable the trust to achieve key targets.

- To assist in any administrational duties required to ensure a full outpatient service is provided in the Outpatient reception, Outpatient Call centre and the Booking Teams.
- To be flexible to a wide range of duties and appropriate tasks on rotation as designated by the Outpatients Supervisor in order to ensure the smooth running of the department. Including covering for colleagues in times of sickness/absence and high workloads
- Greet patients and visitors in a pleasant and professional manner, providing a courteous reception service.
- Excellent communication skills and ability to deal with queries from patients, relatives and carers. To proactively deal with potential situations of conflict, to diffuse and prevent patient complaints. To regularly deal with patients either face to face or via telephone or email who may be emotional or challenging.
- Liaison with staff both within and outside of the department, GP surgeries and other healthcare professionals on any matters relating to patient referrals/queries
- Answer all telephone calls into the department promptly in line with the Outpatient Call centre KPIs, speaking to our patients and callers with respect and courtesy, be positive and helpful in providing excellent service and assistance.
- To participate in call reviews and call monitoring to achieve department KPIs.
- To monitor and action all incoming emails to generic outpatient team inboxes.
Ensure all patient demographics are captured accurately and any changes updated on the Trust PAS when notified.
To manage new referrals and outpatient appointment waiting lists in line with RTT national targets.
To receive and respond to appointment requests. Booking, cancelling, and rescheduling all new and follow up outpatient appointments in line with the Access Policy and agreed clinic booking rules via the Trusts PAS systems.
Liaise with relevant specialty service managers and teams to escalate capacity, waiting list and service issues that require urgent action.
To check patients in and out of their appointments in real time via the Trusts PAS systems. Complete the checking out process, recording the appropriate outcome and 18-week RTT status documented on the outcome form and booking any respective appointments, ensuring all clinics are cashed up at the end of each day.
To liaise with clinicians and services where outcomes are incomplete or missing.
To check case note availability prior to clinic, escalating to the appropriate teams for missing records.
- To keep patients informed of any delays in outpatient clinics in conjunction with the nursing team.
To maintain patient confidentiality and be aware of data protection issues when entering all patient information on all Trust administration and clinical information systems. Book interpreter services/transport as and when required and maintain and provide access to interpreter tablets
