This is an exciting opportunity to support patients throughout their colorectal cancer pathway while playing a key role in our Remote Surveillance Programme.

What you will do:

The Colorectal Specialist Nursing Team Support Worker will work as part of the ColorectalCancerCareTeam supporting registered practitioners to improve holistic care for people with cancer.

• Support the Colorectal Clinical Nurse Specialists in coordinating patient care.
• Manage the Remote Surveillance pathway for patients following colorectal cancer treatment.
• Monitor surveillance investigations and ensure patients receive timely follow-up.
• Contact patients by telephone to provide information, reassurance and appointment coordination.
• Liaise with multidisciplinary teams, GP practices and diagnostic departments.
• Maintain accurate patient records and update electronic systems.
• Support service improvement projects, audits and data collection.
• Help ensure patients receive safe, effective and person-centred care.

The Support Worker will need some prior experience working with cancer patients and excellent communication and organisational skills.

Main duties of the job

• Support the delivery of personalised care services as part of the Personalised Care Team and Colorectal Nursing Team
• Conduct Holistic Needs Assessments to identify what matters most to patients and signpost them to appropriate information, support services and self-management resources
• Provide practical and emotional support to patients throughout their cancer pathway
• Contribute to the planning and delivery of health and wellbeing events, promoting approved cancer information and support services
• Support the Colorectal Clinical Nurse Specialists by assisting with patient care coordination and administrative activities
• Liaise with multidisciplinary teams, primary care and community services to facilitate referrals and seamless patient care
• Maintain accurate patient records and ensure administrative processes are completed efficiently and in line with Trust policies
• Help improve the patient experience by ensuring timely access to information, support and onward services
