Job summary
The London Borough of Waltham Forest and Waltham Forest Federated GP Network are supporting the Primary Care Networks established across the borough to recruit Social Prescribing Link Workers.
The post will be part of a wider Integrated Care development known as the Managed Network of Care & Support.
The role is practice based and not remote.
Salary: Agenda for change - Band 5 - Pro rata
Hours: 18 hours 45 mins. Part time
Fixed Term Contract: Between 6-8 months. To be discussed during interview.
Probation period: 3 months (Extension/s can be given as per our policy and only if required).
The Managed Network focuses on prevention and early intervention. A key activity is to provide non-clinical support to vulnerable residents by employing link workers across 3 well-being pathways, Social Prescribing, well-being at Home and Local Area Coordination.
The model has been developed as part of the Waltham Forest Integrated Care Partnership and is part of the Promoting Wellbeing Programme.
Main duties of the job
Social Prescribing Link Workers empower people to take control of their health and well-being by taking a holistic approach and connecting people to practical and emotional support in the community and statutory sectors. Officers give people time to focus on their aspirations & goals and help to build active and sustainable communities by working collaboratively with local partners.
You will be practice based on a rotational basis and may be working at different practices within the PCN. This is not a remote role.
The activities undertaken by Social Prescribing Link Workers help to strengthen personal and community resilience and reduce health inequalities by addressing the wider determinants of health such as debt, poor housing and social isolation.
The service is targeted at particular groups of residents. Social Prescribing Link Workers will support residents to become more independent and enhance their well-being

5. Draw on, and increase, the strengths and capacities of local communities - enabling VCS organisations and community groups to receive referrals. Ensure groups are supported and have basic safeguarding processes in place for vulnerable individuals, so people can develop friendships, a sense of belonging and increase knowledge, skills and confidence.
6. Alongside other members of the Primary Care Network multi-disciplinary team, work collaboratively with local partners to contribute towards supporting local VCS organisations and community groups to become sustainable and that community asset are nurtured. This may mean making group leaders aware of opportunities for small grants or micro-commissioning, where available. It may also mean providing help to set up new community groups and services, where gaps in local provision are identified.
7. Social Prescribing Link Workers will have a role in educating non-clinical and clinical staff within their Primary Care Network multi-disciplinary teams on what other services are available within the community and how and when patients can access them. There is a part time MSK post available (34.75hrs a week) and is based at Leighton Hospital outpatients and various community base settings within CCICP.

Job summary
This role is to provide expertise in the specialist clinical area of Pathway 2 rehabilitation and assessment community bed base, as lead clinician and co-ordinator within CCICP. It will involve high level clinical triage and knowledge alongside strategic planning and project management to ensure all strands of Pathway 2 rehabilitation, across the CCICP footprint, is provided in a responsive, timely and equitable service delivery.
The post holder will work in conjunction with key partners across CCICP and MCHT,in the planning, development and delivery of Pathway 2 and act as a central liaison and resource for referrals requiring a timely and pathway appropriate response.
The role holder will work collaboratively with Care Community Therapy Team Leads to ensure a consistent rehab therapy led model is embedded across multiple sites. Adopting a Home First approach to ensuring patients can return to their own environment, to continue their rehab journey at the earliest opportunity.
The role holder's base will be IPOCH Hub in BIU Leighton Hospital, Crewe however they will be expected to travel to D2A bedded sites across the Central Cheshire Integrated Care Partnership (CCICP) footprint.
Main duties of the job
- To manage a highly specialist case load, demonstrating appropriate time management and organisation skills.
- To ensure effective day to day organisation and provision of service to meet service priorities across all Pathway 2 sites.
- To provide clinical leadership and support to the MDT, through case supervision, and the development and monitoring of therapy provision and safe discharge of patients in pathway 2 bedded units within CCICP's remit.
- To actively contribute innovate ideas and lead on the development and implementation of service improvements in collaboration with system colleagues and within relevant Pathway 2 groups.
- To contribute to the development and delivery of service objectives, leading on designated projects, focusing on discharge to assess pathways.

An exciting opportunity has arisen for a highly motivated nurse to Join the Alcohol Care Team within our acute care trust here at Mid Cheshire Hospitals NHS Foundation Trust.

To be successful in this venture the post holder will need to develop systems which support effective management and care of individuals who present to hospital with either primary or secondary issues relating to alcohol.
Main duties of the job
1. Lead a specialist area of practice, providing expert professional advice to others across the Trust within agreed guidelines and protocols, including medical colleagues and other professional groups when required.
2. Perform comprehensive specialist assessment of patients, plan, treat and evaluate care within agreed guidelines and protocols according to evidence-based practice and changing health needs.

Job summary
Job Purpose
To provide a specialist podiatry service for adult inpatients with diabetes and other high-risk foot conditions. The post holder will independently assess, diagnose, treat and manage patients with complex lower limb pathology, contributing to the prevention of ulceration, infection and lower limb amputation.
The post holder will work as part of the multidisciplinary team (MDT), providing expert advice to medical, nursing and allied health professionals and supporting timely patient management to improve clinical outcomes, patient flow and reduced length of stay. The role will also support outpatient diabetes services, including the Multidisciplinary Foot Service (MDFS) and Hot Foot Clinic, ensuring continuity of care across the patient pathway.
Main duties of the job
- Provide specialist assessment and treatment of hospital inpatients with diabetic foot ulcers, infection, Charcot neuroarthropathy and other complex lower limb conditions.
- Undertake comprehensive vascular, neurological, biomechanical and wound assessments.
- Develop and implement evidence-based treatment plans for patients with complex clinical needs.
- Perform conservative sharp wound debridement and other advanced podiatry interventions within scope of practice.
- Assess and manage patients requiring pressure relief and offloading using appropriate devices and techniques.
- Identify patients requiring urgent intervention and escalate concerns appropriately.
- Participate in multidisciplinary ward rounds and contribute to discharge planning.
- Provide specialist podiatry advice to medical, nursing and therapy colleagues.
- Provide timely assessment and management of patients referred from the Emergency Department, Acute Medical Unit and Same Day Emergency Care with suspected diabetic foot disease, supporting admission avoidance where clinically appropriate.
- Prioritise inpatient referrals to ensure patients with diabetic foot disease are reviewed within recommended timeframes.
