Main duties of the job
We have an exciting opportunity for a Nursing Associate to join our Primary Care Network team. The Nursing Associate will work under the supervision of the Practice Nursing Team within the allocated PCN. Leadership and support will be provided by the PCN Manager, Clinical Director, and member practices. The Nursing Associate will work within the community to support the everyday running of the PCN practices. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience. The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.

Job responsibilities
- Work within the PCN to provide and monitor care, alongside the wider PCN team, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision
- Improve safety and quality of care at every opportunity
- Undertake additional clinical training as required by the PCN to support the Network Contract/DES Services Specification
- Support registered nurses and the wider team to enable them to focus on the more complex clinical care
- To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience
- Provide support to the PCN with regards to their Network Contract DES Services Specification including CVD prevention and tackling health inequalities
- Develop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health services
- Provide support and supervision to trainee nursing associates, healthcare assistants, apprentices, and those on learning placements appropriate to level of competence
- Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates
- Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships
- Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required
- Communicate complex and sensitive information verbally and in writing. Adapt and tailor communication skills to the need of the listener
- To act as patient advocate during practice or PCN meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately

Job Description:

Speciality Doctor in Emergency Medicine
For entry to the Speciality Doctor grade, applicants must have at least four years of post-registration training, of which at least two years must be in a speciality training programme in Emergency Medicine (for example, ACCS), or must be able to demonstrate comparable skills and competencies.
The job title of the post is Speciality Doctor in Emergency Medicine. The post is paid on the Speciality Doctor scale. Applicants wishing to work part-time would be considered.
Job Plans are worked around a twenty-four-person rota. The job is 10 PA, typically comprising 1.5 SPA and 8.5 DCC.

Main duties of the job
The postholder is expected to work on the middle-grade tier rota in the department and provide the relevant expertise and experience necessary to ensure the efficient primary management of emergency cases. The postholder will be required to work with the other team members in the management of the department workload. The postholder will also be expected to provide supervision to the junior members of the team.
The postholder will be expected to undertake full clinical duties, including the immediate care of critically ill patients attending the Emergency Department and patients presenting with major and minor injuries. This will include initial resuscitation where appropriate, diagnosis, management and treatment, referral and discharge under the guidance of the Consultants. Referrals may be made to GPs, appropriate specialist teams for continued hospital care, or other relevant services.
The postholder will be expected to follow the various guidelines and protocols issued in the department. In the case of major trauma, cardiac arrests and critically ill patients attending the department, the postholder will be expected to act in a team leadership role, following appropriate life support guidelines.
Postholders would be expected to gain and maintain certification in ALS, ATLS, APLS or recognised equivalents to these courses.
In light of changing work practices, the work programme may need to be varied in the future.

As part of the Reception Team you will be the first point of contact for patients contacting the surgery, either by telephone, face to face or digitally. Job summary
This service is a collaboration between Sussex Community NHS Foundation Trust SCFT and the Sussex Federations: Brighton & Hove Federation, Alliance for Better Care and South Downs Health & Care. The Federations will be delivering the GP-led aspect of the Virtual Ward at a place-level to match SCFTs Urgent Community Response service across Sussex. SCFT provides the service in West Sussex, Brighton and Hove and the High Weald, Lewes and Havens area of East Sussex.
B&HF are the lead contractor for the GP Service delivery and have sub-contracted to ABC for delivery within the federations geographical areas of Mid Sussex, Worthing and Chichester.
Since 2022, SCFT have delivered Virtual Wards Hospital at Home in line with the NHS Englands national definition that a virtual ward is a safe and efficient alternative to NHS bedded care that support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
As of August 2024, SCFT re-modelled the Virtual Ward delivery and mobilised General Virtual Wards within Urgent Community Response UCR, which supports Sussex patients within its catchment area with a higher level of acuity to be safely managed at home/chosen place of residence. This could be patients on either an admission avoidance pathway or early supported discharge pathway, where the alternative would be NHS bedded care.
Main duties of the job
The Virtual Ward activity is now prioritising admission avoidance whilst still delivering early supported discharge for patients from the Acute hospitals. It is delivered 365 days per year, 8am 8pm. UCR teams within the SCFT footprint provide urgent support to help prevent unnecessary hospital admissions and ultimately keeping patients safely at the place they call home. There is a consistent service across Sussex and is made up of multi-disciplinary teams including General Practitioners, Nurses, Pharmacists, Therapists and Carers, ensuring they can meet a range of needs quickly offering the best level of support to patients.
The multi-disciplinary team that oversees and deliver care the General Virtual Ward caseload is led by both medical a General Practitioner and clinical senior decision makers. Virtual and On Call GP cover is available seven days a week, 9am to 6pm 365 days a year, including bank holidays. This cover is linked to the respective teams local UCR teams with a virtual board round each morning
The UCR Hub Based GP will work within their assigned place based SCFT UCR base. We are recruiting for a GP for the Worthing Virtual Ward. The GP will present within the allocated UCR base working face to face with the SCFT MDT delivering GVW including supervision of ACPs and team development. Patient visits may be required. Monday, Wednesday and Friday morning

Do you have the enthusiasm, motivation and experience to provide effective psychological care interventions for patients and carers?
As a fixed term position the successful candidate will continue to build the psychological care service across the hospice.
The expectation of the role is to ensure provision of the highest standard of psychological care for patients and their carers/families, utilising a non-pharmological, holistic approach. The successful candidate will complete face-to-face assessments of the psychological distress of patients and informal carers. This will result in the planning and evaluation of individual care programmes for those individuals.
The post holder will work as part of the multi disciplinary team providing high quality psychological support to patients living with a palliative diagnosis and their carers including some group work.
This post requires an Enhanced Disclosure and Barring Service check.
Main duties of the job
About you
You will have an appropriate professional clinical degree in addition to a relevant well-being or psychological care qualification, you will have experience of providing assessment and support of people presenting with psychological needs and experience of applying an assessment and management process.
Experience in Palliative Care/Oncology and experience of working within a hospice, hospital or similar environment is desirable but not essential.
A full driving licence and access to your own vehicle with the ability to travel throughout South Worcestershire is essential for this role.
