Practice Nurse Opportunity
We are a friendly, patient-focused GP practice serving approximately 5,300 patients across our main surgery and branch site. We are looking for a dedicated and compassionate Practice Nurse to join our team and help deliver high-quality care to our local community.
As the sole nurse based at your branch surgery, you will enjoy a varied and autonomous role, building strong relationships with patients and making a real difference to their health and wellbeing. While you will work independently day-to-day, you will have the support of an experienced nursing colleague at our sister branch, providing opportunities for collaboration, advice, and professional support.

Main duties of the job
The successful candidate will provide a high standard of nursing care to our patient population, supporting the management of long-term conditions such as diabetes, asthma, COPD, and hypertension. The role will include undertaking NHS Health Checks, performing cervical smear tests, administering childhood immunisations and adult vaccination programmes, including seasonal flu and COVID-19 vaccinations, and delivering wound care and dressings. The Practice Nurse will also play an important role in promoting health and wellbeing, supporting infection prevention and control procedures, maintaining accurate clinical records, and contributing to the delivery of safe, effective, and patient-centred care.

Monitor, interpret and evaluate patient test results and initiate appropriate treatment, or refer on if required
Make appropriate referrals to primary, community and secondary health care teams
Systematically manage chronic disease in the patient population, including asthma, diabetes and COPD
Undertake opportunistic screening when clinically required e.g. lipids, blood pressure, cervical smears, etc.
Perform venepuncture and electrocardiography
Undertake wound care / management, dressings and suture removal
Undertake leg ulcer management, dopplers and compression bandaging
Carry out ear examination and micro suction
Perform cervical screening and promote breast cancer awareness
Provide an adult and paediatric immunisation service under individualised prescriptions or Patient Group Directions, ensuring safe storage, rotation and disposal of vaccines and drugs
Provide other injections as required and within competence, both IM and SC
Provide advice on lifestyle issues, assist and support patients in changing their behaviours; refer appropriately.
Deliver the smoking cessation service
Deliver travel health care, including vaccinations and travel advice
Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and be aware of local referral procedures
Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries
Undertake first aid and management of emergencies, e.g. burns, haemorrhage, hypo/hyperglycaemic emergencies/asthma attacks
Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques
Provide contraception and family planning advice, including pill checks
Undertake new patient and NHS health checks
Support the practice in achieving quality indicators and targets of the GMS contract and any other relevant service frameworks

Main duties of the job

About us
Our Practice is within a purpose-built Primary Care Centre in the heart of Nantwich with 20 clinical rooms and 1 operating suite on the ground floor from which with provide a minor surgery provision and an onsite Research Team.
Nantwich is a charming historical market town, set beside the river Weaver, with excellent transport links with Manchester only an hour away by train, the beautiful city of Chester is a 45-minute drive away and the North Wales coast just over an hour away. Nantwich also has a number of excellent primary schools and two very reputable senior schools.
We serve a population of circa 13,000 patients and our medical team comprises of 6 GP Partners, 2 salaried GPs, 3 Advanced Nurse Practitioners, 1 Nurse Practitioner, 3 Practice Nurses, and 1 Health Care Support Worker and 2 Physiotherapists. We are part of Nantwich and Rural PCN and currently have ARRS roles in Care Co-ordinator, Nurse Associate, 2 Pharmacy Technicians, 2 Pharmacists and a social prescriber.
We also have an onsite dedicated Research Team with 3 Nurses, Admin and three of our own GP Partners, with valuable research studies constantly ongoing there is always the opportunity to become involved in our research team.
Kiltearn is also a GP training practice for F1, F2, ST Doctors and medical students. We also have Partners with an interest in coach mentoring, career conversation facilitating and as appraisers, one of our GP Partners is also the Education Lead for the PCN.

These clinics are always worked alongside a clinical colleague, helping to share workload and provide peer support.

Salus Medical Services is looking to appoint an interim chair to lead us through the next phase of our development. The main aim of the role is to support the long-term strategic direction of the organisation working alongside the CEO and the Board. This is a new role as previously the federation has operated with a clinical Chair.
The Chair will lead the Salus board providing strategic leadership, ensuring strong governance and supporting high-quality services for patients. Ealing NHS Talking Therapies was one of the first Talking Therapies services to launch in 2008, and is one of the three services provided by West London NHS Trust.
Our main base is in Ealing where we have access to over 16 clinic rooms and two open plan offices. In addition to this, we run clinics at GP surgeries across the borough, covering areas such as Southall, Acton, and Chiswick.

- To provide specialist Modality assessments of clients referred to the Ealing Talking Therapies service based upon the appropriate use, interpretation and integration of complex data from a variety of sources including psychological tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with clients, family members and others involved in the client's care.
To work with clients with a range of mild to moderate mental health problems.

The post holder may work with seldom heard communities who experience inequalities in health, such as asylum seekers, the homeless, ethnic minority groups and those living in areas of high deprivation. They will have responsibility for reaching out to, and providing lifestyle input into this cohort of patients. They will work closely with the other members of the primary care team and complex care teams in the management and decision making about care and service provision for individual patients. This will include:
- Engage with and invite individuals for the NHS Health Check programme, to the programme, offering a quality service that provides tailored advice and encouragement to make behavioural changes accordingly.

Job responsibilities
As a Receptionist within the practice, you will play a vital role in ensuring patients receive a professional, efficient, and welcoming service. You will support both clinical and administrative functions across the practice and contribute to the smooth day-to-day running of the surgery. Duties may vary in line with the needs of the service and the wider practice team.
Key Duties- Provide a friendly and professional first point of contact for patients, visitors, and external organisations.
- Manage appointment bookings, cancellations, and amendments using the practice clinical system.
- Handle patient enquiries in person, over the telephone, and electronically, ensuring all queries are dealt with promptly and appropriately.
- Maintain accurate and up-to-date patient records in line with NHS and practice procedures.
- Process patient registrations, temporary registrations, and updates to patient demographics.
- Scan, file, and manage clinical correspondence and patient documentation within electronic records.
- Support clinicians and the wider team with a range of administrative and operational tasks.
- Process incoming and outgoing correspondence, including emails, forms, and requests for information.
- Escalate safeguarding, urgent, or complex issues in accordance with practice policies and procedures.
- Assist with repeat prescription administration and other routine practice workflows.
- Promote confidentiality and ensure compliance with GDPR, NHS guidance, and practice policies at all times.
