Job summary
We are looking to recruit a salaried GP for 6-8 sessions per week to join our supportive and efficient team of doctors, nurses and administration staff. We are proud to be an equal opportunities employer.
Duties will include both surgery sessions and on call sessions. We have allocated care homes which one of our partners visits each week. Most home visits are done by the home visiting service.
Pay per session depends on experience and is negotiable.
Main duties of the job
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the PMS Contract.

We are looking for a skilled and motivated Health Care Assistant to join our small surgery team looking after patients in the Doncaster area. Our practice is currently expanding and merging with another Doncaster based practice. We are also setting up a new Primary Care Network in Doncaster.
Main duties of the job
The position will involve standard GP health care assistant duties including blood tests, ECGs, dressings, BP checks, some vaccinations, some chronic disease reviews (training required), stocking rooms. You will be part of a small nursing team of two practice nurses, one other health care assistant and a nursing associate.

The Vale Medical Practice is seeking an enthusiastic and motivated pharmacy technician to join our expanding multidisciplinary team. This role is offered on a full-time basis of 37.5 hours per week, and will be based at both our Wootton and Shortstown sites.
Reporting to the senior clinical pharmacist and senior clinical pharmacy technician, you will play a key role in supporting medicines optimisation, medication safety, repeat prescribing systems and patient care within the practice multidisciplinary team.

Main duties of the job

We are looking for a high quality, customer focused administrator to join our medical staffing department. Working within a fast paced medical Education department, you will have a keen eye for detail and the ability to juggle the demands of a busy workload. Working with all grades of doctors and medical students, you will be an excellent communicator and act as the first point of contact for all aspects of medical education. You will be present at a weekly teaching session for doctors, where you will provide support with a variety of tasks and so you will need to be able to work autonomously and think on your feet.

We are seeking an experienced and highly motivated Podiatrist specialising in the High-Risk Foot and Diabetes to join our dedicated Podiatry team. This is an exciting opportunity for an advanced practitioner with a passion for delivering high-quality care to patients with complex foot pathology.
The post holder will support the Lead Diabetes Podiatrist in the coordination, development and clinical leadership of services related to the management of patients with high-risk foot complications. SS9 South PCN has an opportunity for a care co-ordinator to join our team on a part-time basis of 25 hours a week. The role is based across Pall Mall and Highlands Surgery, in Leigh on Sea.
The post-holder will provide extra time, capacity, and expertise to our practice teams, helping us improve the care we provide to our patients.
Main duties of the job
- Support SS9-South PCN in the delivery of high-quality primary health care projects and systems.
- Support the management teams and clinical director to ensure the smooth running of the PCN operations.
- Co-ordinate the deliverance of seasonal covid vaccination programmes including ordering, arranging mutual aid, and reporting to the ICB.
- Oversee the deliverance of the Enhanced Access programme, to include setting rotas, sourcing and agreeing staff allocations. Overseeing the usage of appointments and undertaking any required reporting to the ICB.
- Support rota and allocations for the ECP teams and provide daily support for allocation of visits and route planning.
- Liaise with practices own administrators regarding PCN staff availability, ensuring PCN staff rotas are aligned and available within the practices.
- Support the SS9 South PCN and associated teams with deliverance of services to housebound patients, which may include driving and chaperoning where appropriate.

An exciting opportunity has become available for a Band 5 Well-being Practitioner to work within our Carmarthenshire Primary Mental Health Service.
Individuals will be expected to be enthusiastic and committed to provide well-being and mental health support to help address locally identified need and sustain capacity within wider Primary Care Mental Health Services. Well-being Practitioners will deliver timely interventions in the community, which will ensure early intervention, preventing many individuals from requiring secondary mental health services.

Main duties of the job
The post holder will use a range of communication methods adaptable to the needs of individuals, families/carers and partner organisations to support individuals living with symptoms of mild mental illness.
The post holder will provide support to the Service at large, to enable the best possible care outcomes for those experiencing mental health distress and connect individuals with local Mental Health and well-being services where appropriate, including liaising with partner organisations such as GP's, Primary Care, WAST, A&E, Police and Third Sector etc.
Provide general mental health advice, relating to clinical risk, Mental Health Act or Mental Capacity Act.
Prioritise own workload to ensure that all individuals receive a timely intervention.
Develop and maintain relationships with Health Board services and partner agencies such as GP's, 111 Option 2, Police, Third Sector, Local Authority and WAST etc.

Job summary
This post is for a fixed-term, full-time Consultant Psychiatrist to provide medical leadership to a community rehabilitation team.
This is a 8PA Consultant Psychiatrist post at the Southwark Community Rehabilitation Team branch 1. This is one of 3 teams that comprise the overall Southwark Assertive Outreach and Rehabilitation Team (AORT).
The rehabilitation team serves non-forensic Southwark mental health patients in supported placements and residential care. It offers specialist care for people with severe mental illness, many of whom have co-morbid conditions (substance misuse, personality disorder), with significant impairment of functioning, treatment resistance and complex needs in multiple domains requiring a high level of support; who may be difficult to engage and/or have gone through multiple episodes of inpatient treatment or protracted admissions, and who may pose risks to self and others. The service has a clear focus on recovery and maximising independence, and promoting successful step down to less supported care.
