We have a vacancy at West Leeds PCN for a prescribing Nurse, Paramedic or Nurse Practitioner. This role would particularly suit an Advanced Practitioner with good all round nursing experience as the role itself will be very broad. We need someone who is an experienced Independent Prescriber (ideally 3 years experience), particularly for the Acute Home Visiting element of this role. You would be joining a highly skilled and experienced team with a wealth of peer support available each day.
Main duties of the job
There are two elements to this role:
Annual frailty reviews - these are templated and in depth. We focus mainly on those that are severely frail but we also get the opportunity to work with the moderately frail, changing their health outcome in good time to avoid them from tipping into the severely frail Rockwood score (which you will find immensely satisfying). These are mainly done in the patient's home unless they are able to get into surgery
Acute home visits - you will spend a part of your working week doing these as this services relieves a great deal of pressure on our member practices (particularly the GPs)

Our Frailty Service is a multidisciplinary one for those over 65 years of age who demonstrate two or more of the five frailty indicators (including falls, immobility, incontinence, delirium, medication side effects susceptibility). These patients are comprehensively assessed in their own home (or care home) following which a bespoke patient centred care plan will be created. The intention is to either improve or maintain their level of health and wellbeing and also to enable them to remain as independent as possible in their own residence. The Frailty Team does not carry a caseload; each referral is classed as an intervention. Following review from the Frailty Team the patient care is handed back to their GP practice.
Each patient is assessed for falls prevalence and has their medications reviewed. An independent prescriber would be an advantage for this role but it is not essential as other members of the team can issue medication
You will attend weekly MDT meetings with a consultant geriatrician hosted by our frailty team to discuss the most complex patients who have regular home visits. You will also closely liaise with the wider PCN team as needed in order to deliver a truly holistic healthcare service. The successful candidate will need to be passionate about improving patient health outcomes.
The daily Acute Home Visiting list offers a change of pace and you will encounter such problems as swellings (legs, feet etc), UTIs, chest infections, oedema, infected wounds, skin complaints such as rashes and migraines etc. The provision of this service is vital to our member practices and frees up practice GP time while we fulfil these visits for them

Sequoia is a specialist NHS-commissioned service, co-delivered by Avon and Wiltshire Mental Health Partnership (AWP) and Rethink Mental Illness, supporting adults with mild-moderate complex emotional needs (CEN) and personality difficulties across the BNSSG area. Rooted in lived experience and recovery principles, the service provides evidence-based group interventions including Dialectical Behaviour Therapy (DBT)-informed groups, Mentalisation-Based Treatment (MBT)-informed groups, psychoeducation courses, and peer support programmes.
As a Specialist Practitioner and Group Facilitator, you will initially be co-delivering DBT informed skills groups. With the purpose of enabling service users to manage emotional intensity, build resilience, and achieve recovery goals. You will work collaboratively with NHS teams, voluntary & community partners, and service users to promote wellness, safety, and inclusion.
This role is offered at full time hours - Mon to Fri. Most service-user facing work runs 9:00am-5:00pm, though some evening sessions are required, so flexibility to work one evening weekly is essential.
This is a hybrid role with most staff being based from home with an expectation to travel for planned meetings and some group delivery, based in community settings within the BNSSG locality. Recognising the emotional impact of this work, we provide robust supervision (individual and team-based), ongoing CPD opportunities & regular chances to connect socially with colleagues.
Main duties of the job

Facilitate DBT informed groups, adapting content to meet service user needs.

Main duties of the job
The ECP Care Home GP Practice provides enhanced primary care services for over 1000 patients in nursing homes and care homes in the borough of Ealing.
The post holder is a pharmacist, who acts within their professional boundaries, working as part of a multi-disciplinary team in a patient-facing role. The post holder will assist in areas of patient care and chronic disease management within the care home practice, undertaking clinical medication reviews to manage patients especially for polypharmacy, older people, residential and nursing home residents with multiple co-morbidities.
The post holder will provide support to pharmacy team and care home general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing advice on clinical medicines issues while addressing both public and social care needs of patients in primary care.

About us
Our current clinical team consists of 6 GP Partners, 4 Salaried GPs, 2 ANPs and 2 GP Registrars.
Our surgery is housed in purpose-built accommodation located in the semi-rural town of Crook. We also serve the surrounding villages of Howden-le-Wear and Witton-le-Wear.
North House Surgery is a well-established training practice. We have a strong focus on education and we run regular in-house teaching sessions.
We have a fantastic nursing team including 4 Practice Nurses, 1 Nursing Associate, 4 Health Care Assistants, and a Phlebotomist. North House Surgery is also well supported by Pharmacists, Pharmacy Technicians, Social Prescribers and Wellbeing Coach.
Our CQC inspection was very positive with us gaining an overall score of Good. The Practice is actively involved in our local PCN and Federation.
Our Clinicians are supported by an excellent administrative team led by our experienced Practice Manager. They work hard to ensure the Practice runs smoothly.

Main duties of the job
The Patient Access Manager will be responsible for developing high quality and efficient processes that support and enhance care and service to patients.
The postholder will provide operational leadership of Patient Access Teams and associated clinical support functions across Beacon Medical Group.
The postholder will ensure the delivery of safe, effective, responsive and high-quality services that support patients to access the right care, at the right time, through the most appropriate route.
They will be responsible for the operational oversight of patient access, clinical scheduling, registrar and trainee programmes, workforce planning and service improvement initiatives. The role will support the delivery of organisational priorities through effective operational management, service transformation and continuous improvement activities.

Job summary
We are looking for a maximum of 3 Medical examiners to work 1 session per week.
Medical examiners (MEs) are appropriately trained doctors who will verify clinical information on Medical Certificates of Cause of Death (MCCDs) and ensure that the right referrals are made to the coroner for further investigation. A practising medical practitioner who has been on the general medical council register for a minimum of five years post registration can apply to be a medical examiner. It is recommended that medical examiners are consultant grade doctors from a range of disciplines, including GP principals (who will be employed as specialty doctors. Specialty Doctors and Specialist doctors with appropriate skills will also be considered.
MEs will take a consistent approach to the formulation of MCCD content, which must be clinically accurate and reflect any discussions with the next of kin/informant. MEs must not have been involved in the care of the deceased patients for deaths they scrutinise.
To engage with lead ME and lead MEO for the region.
Main duties of the job
To ensure compliance with the legal and procedural requirements associated with the current and proposed reformed processes of certification, investigation by coroners and registration of deaths.
To scrutinise the certified causes of death offered by attending doctors in a way that is proportionate, consistent and compliant with the proposed national protocol.
To discuss and explain the cause of death with next of kin/informants in a transparent, tactful and sympathetic manner. It is anticipated that such discussions will be predominately conducted through telephone conversations where barriers to understanding information may exist.
To ensure that all users of the ME system are treated with respect and are not discriminated against on the grounds of age, sex, race, religion, ethnicity, sexual orientation, gender reassignment or disability.
To maintain comprehensive records of all deaths scrutinised and undertake analysis to provide information to the National Medical Examiner's office.
To participate in relevant clinical governance activities relating to death certification including audits, mortality review processes and investigations regarding formal complaints about patient care.
To support the training of junior doctors in their understanding of death certification and promote good practice in accurate completion of MCCDs.
To work with medical examiner officers (MEOs), delegating duties as appropriate.
