Multidisciplinary team working is strong, with senior physiotherapists, occupational therapists, and social workers dedicated to elderly services. There is close liaison and collaboration with all departments, and formal links with orthopaedics, old age psychiatry, neurology, and community teams.

Main duties of the job
o To work collaboratively and pro-actively with all appropriate clinical colleagues within the Trust in developing and implementing an acceptable service model reflective of best practice
o To forge effective relationships with clinical colleagues outside the Trust to improve the coherence and integration of patient pathways
o To share responsibility with colleagues for providing evidence-based standards of care for all inpatients and outpatients
o To have continuing responsibility for the care of patients in his/her charge, including all administrative duties associated with patient care
o To liaise effectively and on a timely basis with General Practitioners and all other external agencies
o To lead and co-ordinate the dedicated multi-disciplinary team appointed to support the service models to achieve effective rehabilitation and effective discharge management
o To deliver comprehensive ward rounds and weekly multi-disciplinary meetings to co- ordinate the delivery of high-quality patient care
o To deliver multi-disciplinary outpatient clinics to provide specialist support

About us
Homerton Healthcare NHS Foundation Trust (HH) is an acute hospital located in the east London Borough of Hackney. The current Chairman is Sir John Gieve (Mary Elford will take over from 31st March 2025), and the Chief Executive is Ms Basirat Sadiq.
Homerton became one of the first ten NHS Foundation trusts in the country in April 2004. This allows the Trust to secure certain freedoms to facilitate innovation in the way services are developed and provided. As well as our Trust Board, we also have a Council of Governors comprised of governors representing the local community.
In 2011 the Trust integrated with City & Hackney Community Health Services thereby providing it with an opportunity to develop new service models and pathways that cross traditional organisational boundaries.
Employing over 3000 people, the hospital has approximately 500 beds and manages a 50-bed nursing home, Mary Seacole Nursing Home, in Shoreditch, East London. In 2022, the Trust saw over 300,000 people in outpatient clinics, and over 126,000 people were treated in the Accident and Emergency department. The Trust also had contact with over 280,00 patients and clients within the community, often in their own homes.
The Trust has a turnover in the region of £220 million, performs consistently well against national performance targets, and has an ambitious objective to increase its income to £300 million over the next few years.

Workload in clinical duties Outpatients: profiles of between 2-3 new patients, and 3-5 follow-up patients Inpatients: approximately 14 inpatients per ward round General Internal Medicine on-call 1:15 General Medicine 1:15 cover The post holder will be expected to participate in the general medicine rota initially 1:15. Existing consultants undertaking General medicine duty contribute 2 PAs / week. Acute medicine is provided by 15 General physicians. On the take day, the consultant of the day provides leadership on the acute care unit from 8am-8pm, with all other commitments cancelled There is a ward-based admission system, with longer-stay admissions being transferred from the Acute Care Unit to the appropriate speciality ward. Day 2 review is required, with Consultants arranging appropriate restructure of activities e.g Clinic/Endoscopy sessions until 10:30am on Day 2. Teaching and training The post holder will be expected to participate in teaching and training of postgraduate doctors, which is likely to include acting as an Educational and/or Clinical Supervisor to between one and two individuals, providing them with regular appraisal and support. If the postholder is educational supervisor for more than 1 trainee, SPA time will be allocated as above. It is in the mutual interest of the Trust and Barts and The London, Queen Marys School of Medicine and Dentistry to support quality medical education funded via the SIFT contract. Job summary
An exciting opportunity has arisen within Homerton Healthcare NHS Foundation Trust for an experienced band 5 or existing band 6 physiotherapist to develop their skills and knowledge within the speciality of Pelvic, Obstetric and Gynaecological Physiotherapy within a supportive team.
The pelvic health physiotherapy service is based at Homerton Hospital with a responsibility to provide inpatient and outpatient services, community clinics and group exercise and advice sessions. The service has close links with gynaecology, urology, colorectal and sexual health consultants, midwives, specialist nurses and GP services. The service is led by the pelvic health physiotherapy team lead and the wider Outpatient therapies lead. The Pelvic health team consists of a Band 7 lead and two rotational staff members, a static Band 6 member in addition to the role advertised.
Pelvic Health experience would be advantageous but an MSK physiotherapist with enthusiasm and drive to develop within the speciality of Pelvic, Obstetric and Gynaecological Physiotherapy would be considered. Potential funding is available for courses specific to development into this area. You will be encouraged and supported to develop your clinical, teaching and managerial skills and be actively involved in service development.

The award winning service is a team of dedicated healthcare professionals providing care for patients with chronic cardiorespiratory conditions within the community and hospital.

Job summary
We are seeking an experienced and highly motivated Podiatrist specialising in the High-Risk Foot and Diabetes to join our dedicated Podiatry team. This is an exciting opportunity for an advanced practitioner with a passion for delivering high-quality care to patients with complex foot pathology.
The post holder will support the Lead Diabetes Podiatrist in the coordination, development and clinical leadership of services related to the management of patients with high-risk foot complications. They will play a key role in ensuring that service delivery meets national and local standards, quality indicators and performance objectives.
As a specialist clinician, you will demonstrate expert knowledge in the assessment, treatment and management of complex diabetic foot disease and other high-risk foot conditions, contributing to improved patient outcomes and reducing the risk of ulceration, infection and amputation.
Main duties of the job
Support the Lead Diabetes Podiatrist in the clinical leadership and coordination of the High-Risk Foot Service.Provide expert assessment, diagnosis and treatment for patients with complex diabetic and high-risk foot conditions.Develop and implement evidence-based treatment plans in line with national and local clinical guidelines.Participate in multidisciplinary team working, contributing specialist advice to colleagues across healthcare settings.Monitor service performance and contribute to quality improvement initiatives.Support the development of care pathways and service innovation.Deliver education and training to podiatrists, Allied Health Professionals (AHPs), General Practitioners and other health and social care professionals.Act as a clinical role model, promoting excellence in patient-centred care and professional practice.Maintain accurate clinical records and contribute to clinical audit, governance and research activities.
