An exciting opportunity has arisen to work as a CYPMHS Practitioner in our East team, delivering a high-quality service provision that promotes and improves the mental health and wellbeing of children and young people and their families. We welcome both external and internal candidates for this 6 month contract.
We are seeking an energetic, highly motivated clinician who is willing to develop skills and knowledge in the assessment and treatment of children and young people; to deliver care, advice and support to children and young people with mental health problems.
You must be registered with the appropriate bodies and possess experience of working directly with service users. You will be responsible for demonstrating excellent clinical outcomes and safe and effective delivery of care to children and young people.
CYPMHS East is a caring, supportive and hard-working team which prides itself on working creatively, innovatively and inclusively. We have worked in partnership with young people to re design our clinic room spaces, working to better meet the needs of young people with neurodiversity who are accessing our service, and the development of the intervention offer for families of our young people.
Main duties of the job
The post holder will be responsible for assessing, formulating and providing treatment plans for children, adolescents and their families, in collaboration with other members of the multi-disciplinary team.
As a qualified practitioner the post holder will be required to demonstrate their ability to deliver the high standards of care.
The post holder will be required to prioritise attendance at all training, attend supervision and apply reflective practice continuously to enable learning needs to be identified and met.
The post holder will provide advice, guidance and consultation for professionals from other agencies and other team members.

Hertfordshire Partnership NHS Foundation Trust are transforming Adult Community Services. They are delivering a innovative Enhanced Primary Care Mental Health Service to people in Hemel Hempstead and St Alban's. Our EPMHS joins with existing primary and secondary care services, working in partnership with both, to ensure a great needs led pathway is available for people in the Hemel Hempstead and St Alban's area.

Main duties of the job
In this role you will work to build trusting and collaborative relationships with service users and their carers. Job summary
We are currently seeking an experienced GP Plus Practitioner, at Band 6 to work via a Bank Contract, based in Stevenage for our Primary Care Mental Health Service. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
The role of the GP Plus Practitioner will be to work across the interface between Primary Care and Mental Health Services to work alongside the service manager(s) to facilitate mental health assessment in primary care and support GPs about decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
The post holder will do this by providing consultation, support, education and problem-solving approaches to the Local Primary Care Teams particularly in the area of assessment and management of mental health problems in adults of working age.
HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

Main duties of the job
This will include "signposting" to other services and occasionally undertake very short-term interventions for clients with mild to moderate mental health difficulties.
The role is to strengthen access and to provide community triage for people who may need mental health services. The post holder will ensure systems are in place to promote smooth pathways between primary care and secondary mental health services.
Work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.
Work in partnership with Health and social care professionals from other agencies and organisations including Community Navigators
They will demonstrate an understanding of clinical interventions relevant to the Pathway/Protocols to include psycho education, relevant aspects of social inclusion, Recovery, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this.

Main duties of the job

Job summary
We are excited to recruit to the role of Primary Care Mental Health Practitioner (Band 7) in Hertfordshire's Primary Care Network (PCN) covering the following GP surgeries in the Hatfield area:
Potterells Medical Centre
Burvill House Surgery
Lister House Surgery
Wrafton House Surgery
The role will involve travel between the PCN sites as well as travel to HPFT sites for things such as mandatory training.
You will work alongside other clinicians within the PCN as part of a multidisciplinary team.
You will need to be able to work autonomously to triage, assess, case manage and discharge a patient without referral to a GP where appropriate.
You must be able to advise on medications, and prescribe within scope of practice, identify needs, formulate and triage risk, order investigations and refer on to secondary care services where necessary.
You will act as a first point of contact in primary care. You will accept referrals for triage via agreed routes within the PCN. You will offer rapid and autonomous 'first contact' assessment and promote early treatment by supporting people directly into appropriate mental health pathways across both the statutory and voluntary sectors and will provide short term interventions on the telephone, via video link options, at the surgery or in the person's home where appropriate.
Main duties of the job
You will provide mental health assessment and brief interventions to service users in GP surgeries.
Your tasks will include early triage, assessment, treatment, risk management, identifying and dealing with safeguarding concerns as well as contributing to case discussions and service-related meetings across the adult mental health primary care pathway ensuring good links with secondary care services.
You will provide medication advice and prescribe within specified scope of practice.
You will coordinate the time limited care of primary care-based service users delivering integrated care which takes account of social care needs. You will be someone who enjoys adapting and learning as you contribute to embedding this new role into primary care.
You will also take a leading role for service innovation and development in primary care mental health. Case find, screen, triage, assess and coordinate the development of care plans for people as part of the Primary Care Team

Main duties of the job
- To support the ACMHS in establishing and maintaining effective inter-team and inter-agency communication
- To be directly involved with our busy clozapine, depot and physical health clinics which are ran every week and will include administering medication, ordering, storing and liaising with team Psychiatrists.
- To support the ACMHS and service users by conducting First Contact assessments, acting as a duty worker on the busy duty desk and being part of the team rota.
- To hold a caseload of up to 20 service users who will have ongoing mental health and/or social care needs.
- To provide effective liaison/communication relating to referrals to and discharge from the ACMHS across in-patient, primary care and community services
- Maintain effective joint working with other teams such as EPMHS, PATH, MHSOP, CRHTT etc and other community based teams according to local circumstances.
- Ensure effective communications with relatives, visitors, voluntary organisations and the general public, thus encouraging participation in care delivery. To respond sensitively to the needs of relatives and carers, often in a highly emotive setting
- To liaise with other professionals in relation to client care, actively co-ordinating care planning and review meetings, and case conferences as appropriate.

Main duties of the job
The Community Mental Health Nurse will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a defined caseload of people with complex mental health and or social care needs; and maintaining associated records. Job responsibilities
Clinical Responsibility
01) To manage a patient caseload, identified in consultation with medical, nursing and other professions and agreed by the CMHT Team Leader.
02) To establish and maintain effective individualised nursing and social care by assessment of patients problems; planning; implementation and evaluation of agreed interventions; clear and concise documentation of individualised client care and use of electronic patient record systems.
