We are looking for a motivated and experienced Independent Prescribing, Clinical Pharmacist to join our team. You will be part of a team of five Clinical Pharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to the practice and its patients through primary care. You would be based at Farnham Practice but would be expected to travel to our other locations and our PCN SPINE partner, Kumar Medical Centre, within Slough, to be involved in the wider running of the practice, such as participating in clinical meetings and contributing to the development of new services. You will be part of the Clinical Pharmacist team ensuring prescribing medication is safe, effective, and is optimised for and with our patients. Main duties of the job
To work closely with SPINE PCN GPs and act as point of contact in the practice for all medicines related queries for health care professionals and patients.
To manage the repeat prescribing reauthorisation process and undertake medication reviews of patients, some of which may need to be undertaken in the patients home.
To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF and Locally Commissioned Services.
Manage practice formularies to improve the quality, safety, and cost effectiveness of prescribing.
Where appropriate, review pathology results for patients on known medicines.
To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing and undertake medication audits.

We are looking for a motivated enthusiastic, and hard-working Respiratory Specialist Practice Nurse to join our team. You will be part of nursing team providing expert autonomous nursing care within the practice with a specialist focus on respiratory disease to its patients through primary care. This will include leading the delivery of structured asthma reviews and the provision and interpretation of diagnostic and monitoring spirometry for patients with COPD and other respiratory conditions. You would be based at Farnham Road Practice but would be expected to travel to our other sites within Slough, to be involved in the wider running of the practice, such as contributing to the wider nursing service, working as part of the multidisciplinary team to deliver high-quality, evidence-based care and to support the practices long-term condition, QOF, and IFF respiratory objectives. We are looking for a motivated and experienced Independent Prescribing, Clinical Pharmacist to join our team. You will be part of a team of five Clinical Pharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to the practice and its patients through primary care. You would be based at Farnham Practice but would be expected to travel to our other locations and our PCN SPINE partner, Kumar Medical Centre, within Slough, to be involved in the wider running of the practice, such as participating in clinical meetings and contributing to the development of new services. You will be part of the Clinical Pharmacist team ensuring prescribing medication is safe, effective, and is optimised for and with our patients. Main duties of the job
To work closely with SPINE PCN GPs and act as point of contact in the practice for all medicines related queries for health care professionals and patients.
To manage the repeat prescribing reauthorisation process and undertake medication reviews of patients, some of which may need to be undertaken in the patients home.
To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacies.
To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF and Locally Commissioned Services.
Manage practice formularies to improve the quality, safety, and cost effectiveness of prescribing.
Where appropriate, review pathology results for patients on known medicines.
To help plan, develop and support the introduction of new working processes within the practice to optimise the quality and safety of prescribing and undertake medication audits.
