Job summary
4 - 8 Sessions per week
Esher Green Surgery, a part of Bourne Health, are looking for a proactive and enthusiastic GP to join our Practice, ideally working 4-8 sessions, on a six month maternity cover fixed term contract (with a view to extend), to work closely with our nurses and GPs to manage a busy and interesting clinical caseload.
The postholder will provide high-quality patient care across both clinical sessions and telephone triage. They will have the opportunity to contribute to the leadership of key disease areas, monitoring Quality and Outcomes Framework (QOF) trends, supporting staff training and development, and ensuring safe and effective prescribing. They will also play an active role in developing and improving healthcare services for a diverse patient population, including practice-based commissioning, patient participation and health promotion initiatives.
The postholder will be closely supported by both the clinical and management teams within the practice and will be part of a wider multi-site clinical team. While the main base will be at Esher Green Surgery, there may be opportunities to deliver triage and on-site clinical sessions at other Bourne Health GP Practices.

Main duties of the job
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake clinical sessions. A clinical session is normally composed of face-to-face appointments together with telephone appointments, followed by a variety of duties including queries, visiting patients at home, checking and signing repeat prescriptions, actioning test results, medical reports, and examinations (e.g. Insurance Companies) on behalf of the Practice. The above duties should be performed in a timely manner. Appointments will be 10 minutes duration.

You will be required to
Act upon most alerts (e.g., QOF) and reminders during a routine consultation i.e. (10 minutes).
Provide all GP services required as part of the PMS contract held between the practices and the commissioning primary care agency, including all core services such as Child Health Surveillance
Undertake Duty Doctor and Paperwork Doctor duties on a rota basis.
Undertake surgeries during extended hours in accordance with the access needs of the practice population and the contractual obligation of the practice
Provide cross cover, where necessary and practical, at the other practices
Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment, and referral where appropriate.
Provide appropriate preventative health care and advice.
Provide medical support as required to patients referred by nursing staff

Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Record clear and contemporaneous I.T. based consultation notes to agreed standards;
Collect data for audit purposes
Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Other responsibilities with the Practice
Engaging in appropriate lifelong educational and audit activity to maintain clinical competence and performance, including the completion of objectives agreed by you and the practice, in an annual Personal Development Plan. Your Personal Development Plan must be in line with the Practice Development Plan.
Actively participating in the training and development of nurses, medical students, and GP trainees in the practice.
Supporting the practice staff and responding to requests for advice and assistance from the practice reception, secretarial and nursing staff.
Attend prescribing, palliative care, clinical and practice meetings.
You will be expected to take part in clinical meetings, be prepared to chair these and present clinical topics as required for the maintenance of educational standards and high-quality care. You will receive feedback from the partners via peer review and to act upon recommendations and demonstrate change
Provide additional enhanced services to the practices that meet the needs of the practices patient population and maximise the income streams to the practice
Undertake additional training where necessary to provide these enhanced services
You are required to actively participate in Practice Based Commissioning - this will include reducing first outpatient attendances, demand management, monitoring of activity including peer review and data validation
Awareness of and compliance with all relevant practice policies/guidelines, e.g., prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of the Practice.
Meet all tight timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (e.g., Locally Commissioned Services returns, annual QOF audit etc.)

Job summary

The Administration team provides administrative support to the clinical and non-clinical teams. They support the clinical team by coding medical records as directed. Main duties of the job

Job summary
Bourne Health is a leading Primary Care NHS (National Health Service) GP (General Practitioner) Partnership. The Administration team provides administrative support to the clinical and non-clinical teams. They support the clinical team by coding medical records as directed. They support the non-clinical team by overseeing registrations and deductions and providing administrative support across all areas of Practice Administration.

Main duties of the job
As a Patient Administrator, you will be responsible for a variety of tasks including handling patient queries, managing clinical correspondence, maintaining accurate records, and supporting appointment and referral processes. You will work closely with GPs, nurses, and the reception team to help deliver efficient and compassionate primary care services.

Job responsibilities
Key Responsibilities:
Scanning
Scan and file all incoming paperwork onto patient records
Check scanning for duplicates to ensure only one copy of documents is placed on patient records
Using the agreed scanning protocol, assign documents into the relevant folders to be actioned by the appropriate team members
Electronic Document Transfer Administration
File all EDTs into the Emis patient records
Using the agreed protocol, assign EDTs into the relevant folders to be actioned by the appropriate team members
Ensure all EDTs are filed and assigned within a suitable timeframe
Registrations
Oversees all aspects of Patient Registrations
Manages the automated system of patient registrations
Inputs patient registrations from paper forms
Ensures all patient registrations are actioned in a timely manner
Ensure the appropriate safeguarding alerts are actioned
Manage any alerts that are highlighted after a patient registration
Manages GP2GP note transfers
Patient Deductions
Oversee all aspects of patient deductions
Patient Notes/Records Administration
Receiving patient notes from PCSE
Ensuring Notes are sent back to PCSE as required
Ensuring EMIS is kept up to date with the whereabouts of the patient notes
Scanning patient notes onto EMIS in a timely manner

Oversee and manage all the private medical administration

Patient Screening Programmes
Manage and oversee Patient screening programmes: Bowel, Breast, cytology and any other relevant programmes

Baby Clinics
Oversee and manage the appointment booking for new baby checks
Liaise with patients to ensure these appointments are booked in
Administration Functions
Provide administrative support as needed
Provide Administration cover when required to ensure smooth practice operations.

Assist with Quality of Framework (QoF) and other clinical recalls as appropriate
