We are seeking a dynamic and compassionate Sister/Charge Nurse to join our innovative joint post working across the Integrated Discharge Team (IDT) and Rapid Response Team (RR). This unique opportunity provides the chance to work autonomously across acute and community services, supporting patients to avoid unnecessary hospital admissions and facilitating safe, timely discharge back to their own homes
We are looking for a registered nurse with:
- Significant post-registration experience at Band 5/6 level.
- Strong assessment, clinical decision-making and care planning skills.
- Experience in community nursing, discharge planning or urgent care pathways.
- A passion for integrated working and person-centred care.
- The ability to work independently within a fast-paced environment whilst maintaining excellent team relationships.

Main duties of the job
You will contribute to the delivery of safe, effective and timely discharges for patients returning to the community from hospital.
Assess patients, work collaboratively with families, carers, the multidisciplinary team to ensure a comprehensive understanding of available community services when arranging health-funded packages of care.
Receive, assess, manage same-day referrals from Primary Care through the GP Liaison Service, using clinical judgement, decision-making to support admission avoidance and ensure patients are directed to the most appropriate care pathway.
Ensure the provision of community nursing care that responds to urgent crisis response requirements in the community by assessing, prioritising, allocating, visiting, and treating patients using up to date clinical skills and knowledge. Work with the integrated community nursing teams and wider multidisciplinary / integrated teams to provide high quality nursing care.
Work collaboratively with acute hospital, ambulance service and other providers to ensure care at home that avoids inappropriate hospital admission.
Prevent unnecessary hospital admissions and facilitate timely hospital discharge.

Job summary
Dynamic Health - Musculoskeletal Physiotherapy and Specialist Services are looking for an enthusiastic, self-motivated and dedicated individual to join our service.
We have a permanent, part-time vacancy working 18.75 hours per week, Monday - 8-4pm, with the rest of the hours negotiable for an Administrator within our administrative team.
The postholder will be required to support localities with unit wide working based at Hinchingbrooke Hospital in Huntingdon.
Dynamic Health provides specialised community service across Cambridgeshire and Peterborough, for people aged 16 years and over living with or experiencing problems from muscles, nerves and joints.
The postholder will need to be able to work effectively to support the team to achieve deadlines. The postholder will require a good knowledge of administration and possess strong organisational skills.
The postholder will be required to be proactive about collaborating with their colleagues across the localities and the Trust to problem solve and overcome issues as they arise. The postholder will be required to work using their own initiative and work on active improvement of patient pathways and wait times within the unit.

Main duties of the job
This post requires an Administrator to answer enquiries in a courteous and professional manner both by telephone and face to face, ensuring all messages and information is communicated appropriately.
This will require the post holder to make an independent judgement as to whether the message needs to be dealt with by a qualified practitioner. Judgement will also be required to ascertain whether the message warrants urgent action.
The postholder is required to be the first point of contact for patients both by phone and face to face on reception, to support patients by providing a warm and professional greeting to all and directing visitors and patients to the appropriate person or area. This will also include making sure the reception area is clean and tidy to adhere to the health and safety policy as well as infection control.

Job responsibilities
Assist in the administration of clinics including booking appointments, cancelling and re-scheduling of appointments using SystmOne, ensuring data quality at all times.
Answer telephone calls from members of the public, communicate information about the service, register the patients on SystmOne and arrange booked call backs for physiotherapy assessment in a courteous and friendly manner.
Complete the accurate registration and administration of referrals into SystmOne, clinical patient administration system.
Assist with monitoring of waiting lists and clinics for the weeks ahead and plan to ensure capacity is used appropriately. This includes sending online booking SMSs to fill routine appointments and ensuring patients that have been contacted have the process followed for contact and discharging appropriately for all relevant areas.
Administer referrals from the electronic referral system, the self-referral system and the email inbox to include:
Retrieval and attachment of clinical referral information.Registering the patients and referral opening on SystmOneAdding the patients to appropriate waiting list for triage and questionnaire sending.
Maintain the waiting lists by:

Maintain the Appointments for Booking waiting list and ensuring all the patients on the list are contacted and booked for an appointment in line with the process or time frame given.

The service works shifts from 8pm until 6pm, 7 days per week with weekend work on a rota basis. The applicant would be expected to work shifts across. A typical shift pattern for a full time member of staff may include 4 working days over a 7-day period with 3 rest days.
We have an exciting opportunity for motivated, well organised and enthusiastic individuals to join our falls team as a Part time Community Falls Support worker here in Luton. We are a small proactive team that work autonomously using your own initiative, have good IT and communication skills. Full support and training will be provided and the Trust is committed to staff development.
The team works with people who have fallen or are at risk of falling in their own homes and aims to prevent unnecessary hospital admissions and reduce the risk of falls.

Main duties of the job
- To work within the Falls Team in providing timely intervention service for people who have had a fall or are at risk of falling.
- To complete a comprehensive falls assessment and identify any equipment, care or support required to enable the patient to remain at home safely and avoid unnecessary hospital admission.
- To facilitate/support the delivery of falls strength and balance classes across Luton, therefore there will be a requirement to travel locally between community group venues, clinics and patients own homes.
- To make Falls prevention recommendations and advice and completed onward referrals to relevant stakeholders as appropriate.
- To work closely with GP suregeries, Luton integrated community services, the ambulance service and other health, social care and voluntary organisations in Luton to reduce falls.

The Integrated Diabetes Team are looking for an individual who is already a Specialist in Diabetes Care or an individual who has a keen interest to specialise in Diabetes.

Job Purpose
The post holder will work as part of the Integrated Community Diabetes Services, providing specialist diabetes advice and clinical support. Demonstrating a sound knowledge of the issues related to the identification, assessment, treatment and management of diabetes.
They will support the delivery of structured educational to people with diabetes. Give specialist advice to GPs and other health professionals about Diabetes, especially with reference to adjustment of medication and supporting the management of patients with complex needs. Working closely with General Practices and other Health Care Professionals, they will support the delivery of integrated diabetes services across the primary / secondary care interface. As well as contributing to Clinical Governance within the service by participating in audit and research as part of the team, they will be expected to champion priorities within national diabetes strategies and NICE guidance/standards for diabetes.
