Job summary
We are looking for a Registered Staff Nurse to join our dedicated and highly professional team of community nurses in North Norfolk. We have a 30 hour (4 day per week) position within the friendly and welcoming NN3 Team (Aylsham, Reepham, Drayton and Coltishall areas).
The Community Staff Nurse role is a role under the supervision of Band 6 and Clinical Lead colleagues as part of the larger integrated team, providing high quality patient centred nursing care to patients in a variety of community settings. Nursing care delivered will include catheter care, support for patients with palliative conditions including syringe driver management, management of pressure ulcers and legs ulcers as well as other chronic and surgical wounds, insulin and diabetes management and provision of holistic assessments. Candidates should have an ability to motivate and educate patients with the goal of promoting self management of their health care needs.
This is an ideal opportunity for those either wanting career progression and experience within a community setting or those with previous community experience, skills and knowledge.
You must be self-motivated, flexible able to demonstrate sound clinical reasoning skills and have good communication skills.
Main duties of the job
The service is key to the on-going development of the locality within the new Primary Care Network, and will provide the link between our operational services and the GP practices. Ensuring effective communication for those in the care of NCH&C between the G.P practice, Social Services and other service providers involved their care to ensure best outcomes.
You will demonstrate a proactive philosophy to care, committed to a "whole patient" approach, ensuring holistic care of the patient and involvement of all parties. You will be someone who believes strong and effective communication is essential and who strives to ensure quality patient care is their highest priority.
To assist you in doing this you will be clinically supported by our Community Matrons, Clinical Leads and by a management team committed to patient centered quality care. You will receive support and development through in service training, regular appraisals, a personal development plan and clinical supervision.

We are seeking a dynamic and compassionate Sister/Charge Nurse to join our innovative joint post working across the Integrated Discharge Team (IDT) and Rapid Response Team (RR). This unique opportunity provides the chance to work autonomously across acute and community services, supporting patients to avoid unnecessary hospital admissions and facilitating safe, timely discharge back to their own homes
We are looking for a registered nurse with:
- Significant post-registration experience at Band 5/6 level.
- Strong assessment, clinical decision-making and care planning skills.

Main duties of the job
You will contribute to the delivery of safe, effective and timely discharges for patients returning to the community from hospital.
Assess patients, work collaboratively with families, carers, the multidisciplinary team to ensure a comprehensive understanding of available community services when arranging health-funded packages of care.
Receive, assess, manage same-day referrals from Primary Care through the GP Liaison Service, using clinical judgement, decision-making to support admission avoidance and ensure patients are directed to the most appropriate care pathway.
Ensure the provision of community nursing care that responds to urgent crisis response requirements in the community by assessing, prioritising, allocating, visiting, and treating patients using up to date clinical skills and knowledge. Main duties of the job

East of England Community Health and Care NHS Trust is recruiting a Clinical Fellow to join the specialist palliative care team at Priscilla Bacon Lodge, Norwich - a 16-bed, consultant-led inpatient unit, purpose-built in 2023 with the Priscilla Bacon Hospice Charity.
This is a one-year, full-time fixed-term post. You'll work within a three-consultant-led multidisciplinary team, gaining direct clinical experience in symptom control and end-of-life care. You'll get a structured induction before joining the on-call rota, backed at all times by a second on-call Consultant.

- To respond quickly to all referrals for patients who have fallen or at risk of falling, arranging to visit them as soon as possible.

The Integrated Diabetes Team are looking for an individual who is already a Specialist in Diabetes Care or an individual who has a keen interest to specialise in Diabetes.
