An exciting opportunity has arisen for a Advanced Clinical Practitioner (ACP) to join our Specialist Frailty Services based in the community across North East Essex.

We are seeking a dynamic ACP to join our multidisciplinary frailty team working within our expanding Frailty Rapid Assessment Service

Bringing Frailty and Older persons expertise you will play a key role in delivering an admission avoidance service with a focus on preserving function and independence, and supporting proactive patient care.

You will also be required to support other frailty services, including the Frailty Hospital @ Home service (Virtual Ward).

Working 9.5 hour shifts covering 7 days. Weekend working will be expected.

Banding and Progression:

o Band 7 if working towards a full MSc in Advanced Clinical Practice.

o Band 8A upon completion of a full ACP MSc, with clear progression to 8A once achieved, in line with Trust policy

Main duties of the job

Uses specialist knowledge and specific training to act autonomously in assessing, planning, and implementinga comprehensive management plan for undifferentiated presentations within their defined scope of practice.

The Clinical Workforce Team has an exciting opportunity for a Registered Nurse or AHP who is passionate about clinical workforce to join as a Senior Workforce Practitioner.

We have one 32 hrs permanent position from 4th January 2027.

You will need excellent interpersonal and problem solving skills and the ability to think creatively in order to collaborate with senior clinical staff and ESNEFT recruitment and retention teams to design, implement and evaluate clinical workforce projects.

An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Colorectal Team at Ipswich Hospital as aStraight to Test - Colorectal Nurse. You will play a key role in the early colorectal cancer pathway, supporting the delivery of a safe, effective and timely Faster Diagnosis Standard (FDS) service. The role will involve clinical triage of referrals, telephone assessment, assessing fitness and suitability for investigation, identifying appropriate diagnostic tests & coordinating patients through the diagnostic pathway. We are seeking a nurse with a good clinical background & an understanding ofcolorectal disease, comorbidities, anticoagulation, frailty, functional status & overall fitness for investigation. Knowledge of the colorectal FDS pathway, suspected cancer referral criteria, clinical triage & the ability to apply guidelines & protocols is desirable. You will use electronic systems to assess referrals, request investigations & communicate with the multidisciplinary team, including colorectal consultants, CNSs, Endoscopy, Radiology, Cancer Hub and GP practices. Excellent communication skills are essential, with the ability to explain investigations clearly to patients, provide appropriate advice & recognise when senior clinical support is required. The role requires good organisation, independent working within agreed competencies & accurate documentation.Full training, competency development & clinical supervision will be provided.

Main duties of the job

The STT Colorectal Nurse will play a key role in the early assessment & management of patients referred through the colorectal suspected cancer pathway. Key responsibilities include:

• Reviewing and clinically triaging referrals in line with the colorectalFaster Diagnosis Standard pathway, referral criteria, local protocols and national guidance.
• Assessing clinical information and undertaking telephone assessment where required to establish relevant history, comorbidities, medication, anticoagulation, frailty, functional status and fitness for investigation.
• Identifying the most appropriate diagnostic investigation and arranging or requesting this through electronic systems in a timely manner.

An exciting opportunity has arisen for a fixed term Senior or Junior Community Physiotherapist (Depending on experience) to join the North East Integrated Neighbourhood Team. The role is based in Leiston. A full driving licence and access to a car for work are essential.

Community experience is preferred but not essential, as full training will be provided. Our team is motivated and forward-thinking, consisting of Therapists, Nurses, Social Workers, and Support Staff. We recently merged our Health and Social Care teams to form an Integrated Team, offering a new model of care and innovative ways of working.The primary focus of the therapy team is to provide short-term rehabilitation in the community, supporting timely hospital discharge and preventing unnecessary admissions. This is achieved through tailored exercise programmes, equipment provision, and functional interventions that promote independence. We visit patients in their own homes, care homes, and nursing homes, as well as offering rehabilitation within the gym at Aldeburgh Community Hospital. We also run clinics, such as GLINT (Geriatric clinic) and community OP clinic.
