Job summary
Practice Pharmacist Full Time/Part Time Considered
Longton Medical Centre is looking for an enthusiastic clinical pharmacist to join our expanding team. We are a well-established training practice with a growing list size of approximately 9400 patients.
Our current pharmacy team comprises of 1 pharmacist who is supported by pharmacists employed by our local Primary Care Network (PCN). We also work closely with our Medicines Management colleagues.
The post holder will work alongside other clinicians to provide primary support to practice staff with regards to prescription and medication queries. They will help manage the repeat prescription system, deal with acute prescription requests, and medicines reconciliation. They will advise on safe prescribing within the practice. They will undertake medication reviews to proactively manage patients including those with complex polypharmacy. Conduct safety audits as per CQC requirements
The successful candidate will be an independent prescriber and ideally have experience of primary care or working towards independent prescribing

Main duties of the job
To provide high quality pharmacist support to the practice team members and patients in the practice.
To deal with all suitable prescription query from community pharmacies, third party medication/supply companies and hospitals/other NHS organisations that cannot be dealt with by members of the Reception Team
To provide support, advice and guidance to members all of the practice team.
To review repeat medications and address concerns where appropriate
Reconciling medication on hospital letters to that contained in the patients medical record, raising queries with other clinical staff as necessary, ensuring patients receive appropriate medication.
Reduce inappropriate poly-pharmacy and wasteful prescribing through medication review
About us
The surgery has 2 GP partners, 4 salaried GP's, 2 Practice Nurses, Healthcare Assistant and team of ARRS staff both on and remote working. We have a team of administration staff supporting the Practice Manager.

Main duties of the job

Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse and Senior Community Nurses in the management of the health care environment.
Implement / refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries/Team Up and local acute trusts.
As this is a rapid service, there will be a need to be able to adapt care to meet the patient demand which is ever changing.

The Patient Services Supervisor(s) will be expected to provide a point of contact for patients and act as a focal point of communication between patients, doctors and other reception staff. This is an exciting opportunity to work in a large, highly supportive and enthusiastic paediatric team providing inpatient and outpatient General paediatric services across East Lancashire. The main clinical duties of this post are in general paediatrics. The Consultant Paediatricians have recently reconfigured their working patterns to participate in a 1:8.5 Consultant of the week system which is split into 2 half weeks, working a block from Monday - Thursday, and a block Friday - Sunday. During the 'hot week' there are 2 Consultants of the week who are both ward based responsible for the ward, COAU, ED emergency calls, and inpatient safeguarding. All consultants also participate in cover on the COAU from 1pm - 10pm leading a team of registrars, junior doctors and Advanced Clinical Practitioners to manage ambulatory patients and review GP and ED referrals. This is covered on a 1:17 rota. Finally all consultants partake in non resident on call. Two consultants are non resident on call at weekends, with 1 consultant on-call overnight.Overall frequency on non resident on calls is1:17. All on-calls and COAU shifts are prospectively covered.

We are a small GP Practice with a very high level of satisfaction among our patients thanks to the dedication and professionalism of all our staff.
As a Receptionist you are the first point of entry into the practice and therefore key in maintaining our reputation and our standards, whether in person or on the phone.

Main duties of the job

Depending on your shift you will be required to open or close the practice and perform the daily checks and controls associated with those moments.
