We are looking for an enthusiastic and skilled Band 6 Community Practitioner to join our dynamic Children's Neurodevelopmental assessment Pathway in Newcastle and Gateshead CYPS. This post is to cover maternity leave for 12 months, 25 hours per week, or 8 months full time.
This is an exciting opportunity to play a key role in co-ordinating high-quality autism and ADHD assessments for children and young people across Northumberland.
You will be central to the coordination and delivery of neurodevelopmental assessments, working closely with families, schools, and multi-agency partners to ensure timely, child-centred, and evidence-based care.

Job summary
Sunderland West Community Treatment Team have an opportunity for a band 6 clinician to join the service. We are looking to recruit a caring and compassionate Community Practitioner with excellent interpersonal and communication skills to work as part of our multi-disciplinary team offering assessment and treatment to clients experiencing moderate to severe mental health difficulties. Your responsibilities will include the assessment and treatment of clients experiencing mental health difficulties, both psychosis and non psychosis, working closely with families and other professionals.
The role requires an element of autonomous working and leadership skill and you must be able to demonstrate a strong commitment to the delivery of high quality, safe and effective care and treatment.
It is essential that you have a professional qualification in the field of health and/or social care and registered with the relevant professional body. We provide excellent training opportunities to allow you to deliver NICE guidance treatment. You will work closely with the multi-disciplinary team including Nurses, Occupational Therapists, Psychology/Psychological Therapists, Consultant Psychiatrists, Clinical Support Assistants and Peer Supporters. You will receive regular supervision both on an individual basis and in a group setting and will work closely with other CNTW services and local agencies.

Providing MDT assessment of clients' needs and identifying collaborative treatment plans.
Deliver various evidence based psychological therapies both individually and in a group setting.
Support the wider team in ensuring the provision of excellent standards of care.
Have a good understanding of pharmacological approaches to treatment.
To work together with clients and their families through the journey of care. You will be expected to develop your skills and the team has access to excellent training opportunities.
To maintain effective working relationships with other teams in CNTW and external agencies.
To support with the implementation of Community Transformation agenda.

Job summary
Sunderland South Community Treatment Team have an opportunity for a band 6 clinician to join the service. We are looking to recruit a caring and compassionate Community Practitioner with excellent interpersonal and communication skills to work as part of our multi-disciplinary team offering assessment and treatment to clients experiencing moderate to severe mental health difficulties. Your responsibilities will include the assessment and treatment of clients experiencing mental health difficulties, both psychosis and non psychosis, working closely with families and other professionals.
The role requires an element of autonomous working and leadership skills and you must be able to demonstrate a strong commitment to the delivery of high quality, safe and effective treatment.
It is essential that you have a professional qualification in the field of health and/or social care and are registered with the relevant professional body.We provide excellent training opportunities to allow you to deliver NICE guidance treatment. You will work closely with the multi-disciplinary team including Nurses, Occupational Therapists, Psychology/Psychological Therapists, Consultant Psychiatrists, Clinical Support Assistants and Peer Supporters. You will receive regular supervision both on an individual basis and in a group setting and will work closely with other CNTW services and local agencies.
We would welcome applicants from Nursing, Social Care and Occupational Therapy backgrounds.
Main duties of the job
Providing MDT assessment of clients' needs and identifying collaborative treatment plans.
Deliver various evidence based psychological therapies both individually and in a group setting.
Support the wider team in ensuring the provision of excellent standards of care.
Have a good understanding of pharmacological approaches to treatment.
To work together with clients and their families through the journey of care. You will be expected to develop your skills and the team has access to excellent training opportunities.
To maintain effective working relationships with other teams in CNTW and external agencies.
To support with the implementation of Community Transformation agenda.
Working collectively with the team in maintaining individual and team wellbeing.

Job summary
We are currently looking for a flexible and innovative Community Practitioner to join our experienced community neuro rehab multi-disciplinary team.
The post holder will be required to provide high quality Community Practitioner assessment and intervention skills to patients who have complex clinical needs as a result of Traumatic Brain Injury (TBI), to enable them to maximise their rehabilitation potential.
The successful candidate work in a range of settings including the patients own home and the local community, as well as providing support to clients who reside in residential and nursing care settings in Northumberland.
Service users may present with a combination of physical, cognitive, sensory, psychological, behavioural and social needs therefore a commitment to multi-disciplinary working is essential.
The successful candidate will have excellent opportunities for continuing professional development and be supported by work based learning and specialist training.
The successful candidate will work across the entire county of Northumberland and must be able to meet the mobility requirements of the post.
Main duties of the job
To develop co-produced rehab plans with patients with TBI and refer on to internal and external specialists in accordance with the patients assessed need.
Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, third sector partner agencies and the public.
To regularly review rehab plans with patients and families and amend plans according to changing need.To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
To deliver evidenced based clinical interventions.The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.Provide clinical supervision to junior staff as delegated by Long Term Conditions Lead.To provide compassionate care that is based on empathy, kindness, respect and dignity.To incorporate a client centred philosophy into practice and client centred and collaborative care.
