- Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice.

- Deal with all general enquiries, explain procedures and make new and follow-up appointments.

- Use your own judgment and communication skills.

- Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, and ensure procedures are completed.

- Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.

- Enter requests for home visits into the clinical system, ensuring to carefully record all relevant details and referring to the rota doctor where necessary.

- Enter patient information on to the computer as required.

- Open and distribute incoming mail.

- Photocopy and shred documentation.

- Open up premises at the start of the day when first to arrive, de-activate alarm and make all necessary preparations to receive patients.

- When last to leave at the end of the day, ensure that the building is totally secured, internal lights are off and the alarm activated.

- Keep the reception area, noticeboards and leaflet dispensers tidy and free from obstructions and clutter.

- Assist with the induction of new reception staff.

- Support reception team members in managing busy periods and resolving patient queries effectively.

- Process new patient registrations and action patient deductions.

Essential

• Previous experience working in a GP practice or healthcare reception environment (essential). Ability to remain calm under pressure and manage competing demands. Proficient in IT systems relevant to GP practices. Excellent communication and interpersonal skills, with the ability to interact effectively and empathetically with patients. Highly organised, with strong time management and the ability to manage multiple tasks in a fast-paced environment. Demonstrated ability to work collaboratively within a team. A flexible and proactive attitude toward change and service improvement. A genuine passion for patient care.

Key clinical responsibilities

• Assess and triage patients, including same-day and urgent presentations, and provide definitive treatment or appropriate onward referral
• Undertake face-to-face, telephone and, where appropriate, digital consultations in practice, patients homes, care homes and other community settings
• Undertake comprehensive history taking, physical examination and clinical assessment for patients with acute, chronic, complex or undifferentiated presentations
• Formulate differential diagnoses, identify red flags, assess clinical risk and determine the appropriate level and place of care.
• Initiate emergency care and escalation when required, including referral to ambulance, urgent community, secondary care or specialist services
• Treat minor illness and minor injury within competence and agreed local pathway

• Where appropriately qualified as an independent prescriber, prescribe, review and deprescribe medicines within competence and in accordance with NICE guidance, local formularies, antimicrobial stewardship principles and PCN/practice policies
• Provide patients and carers with appropriate information about medicines, including purpose, adherence, side effects, interactions and when to seek further advice.

Qualifications

Key duties and responsibilities

• Patient facing Long term condition clinics (where appropriate).
• See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma, Diabetes).
• Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation).
• Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.

Telephone or Patient facing Clinical Medication Review

• Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist team leader, nurses and/or GP on prescribing and monitoring.

Telephone or Patient facing care home medication reviews

• Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.

• To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
• Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g. those with medicine compliance aids or those in care homes).

Signposting

• Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.

Repeat prescribing

• Produce and implement a practice repeat prescribing policy.
• Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
• Ensure patients have appropriate monitoring tests in place when required.

Risk stratification

• Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.

Service development

• Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).

Information management

• Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

Medicines quality improvement

• Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team.

Medicines safety

• Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
• Implementation of local and national guidelines and formulary recommendations
• Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs).
• Assist practices in seeing and maintaining a practice formulary that is hosted on the practices computer system.
