This is a part-time post for 30 hours per week over 4 days.

Job summary
An exciting opportunity has arisen for a 6 month - fixed term, Band 4, Senior Administrator- Medical Secretary to support our North Durham Adult Learning Disabilities team based at Chester Le Street Health Centre.
Your role will consist of being responsible for ensuring the provision of a full secretarial service to consultants, junior medical staff, advanced practitioners, and other staff within the multi-disciplinary team.
The postholder will undertake non routine activities which may be delegated by line manager, consultant staff or other members of the multi-disciplinary team.
Due to the nature of the role, attendance in base is required and therefore remote working would not be suitable for this role.

Main duties of the job
The successful postholder will resolve problems which may be received directly from service users, family or carers or referred by members of the administration team. To always interact compassionately with service users and carers. Service users have a range of mental health conditions or learning disabilities.
The postholder will complete secretarial duties for medical team such as meeting minutes, diary co-ordination, setting up meetings and dictation typing.
You will be the first point of contact for service users, family and carers dealing with a range of enquiries or concerns in person or over the telephone. This may involve arranging for an interpreter.
You will provide a full range of personal assistant duties for medical staff including triaging of e-mails and post, administering annual leave and study leave applications, booking attendance at training events and conferences and liaising with designated staff to ensure accommodation and travel is booked as required.
In addition to this, the postholder will be first point of contact for managers, general practitioners and other healthcare professionals within and external to the Trust, dealing with a range of enquiries, queries or concerns on behalf of consultants and medical team, to support them in communications with all groups or individuals involved in the service.
You will cross cover team secretaries and medical secretaries' in their absence.

Job summary
The post has arisen due to the current post holder leaving the role. The post is located in the South Community Mental Health Team, based at Worsley Court, Selby.
The South CMHT provides expert secondary mental health care to adults over the age of 18 who live in the South of York and surrounding area. The locality has recently moved from GP allocated services to post code allocated services and there are further plans to align our services with the Primary Care Home model. The other parts of the city are covered by the North CMHT based in York at Huntington House; the west team based at Acomb Garth in York and the Central team, also based at Huntington house in York.
The team is multi-disciplinary, utilising the expertise of doctors, nurses, occupational therapists, psychologists, social workers and support workers. There is access to the dietetics team, physical health team and pharmacy.

Main duties of the job
You will be part of the leadership cell and will develop close working relationships with the other clinicians in the CMHT. Core work in this setting, on average, will involve 1 new and 6 follow up patients per week. You will work with other professionals as co-workers but are not expected to be lead professional.

Clinical Practice
● Perform all duties required within, or on behalf of the community pharmacy. This includes all Essential, Additional and Enhanced services which are locally or nationally commissioned. This will include but not limited to:

Main duties of the job
Mowbray Square PCN, together with Tees Esk and Wear Valleys NHS Foundation Trust, are working together to transform local community mental health services for people with mental health problems. New funding has enabled the PCN and local GP surgeries, to create a multi-disciplinary team that will work in close collaboration with a wide range of organisations.
The postholder joins the existing First Contact Mental Health Practitioner that has been extremely successful with good outcomes for patients and the PCN. They will work in partnership with practice colleagues to continue to build the service within Mowbray Square PCN. The PCN's ambition is to improve access to emotional and mental health support for everyone.
As a valued team member, you will develop close working relationships with all our friendly Clinical staff, including our existing Mental Health Practitioner. Your role will involve offering specialist mental health advice and providing clinical care forpeople with a range of mental health needs, ensuring that those with mental illness do not fall between gaps in services. You will improve access by being a 'first contact mental health worker', undertaking triage, assessments, brief interventions, and reviews, with clinical supervision provided by a B6/B7 Mental Health Practitioner. You will contribute networking and navigation expertise, enabling access to a range of appropriate mental health support across the community system.

Main duties of the job
Our Practice Nurse role would be expected to undertake a range of nursing assessments and provide appropriate care or treatment in conjunction with our clinical team and Practice Policies. The role involves working across three sites at Blackhall, Peterlee and New Seaham.
Ideally you would be able to support and deliver a high level of quality care and service based on the needs of the patient population of which would include treatment, management plans, screening, preventative care and patient education. In addition, you would also be expected to support the Practice in meeting its contractual obligations and delivery of enhanced service specifications.

We are a large training practice with four GP partners, four salaried GPs, a nursing team of nurse practitioners, nurses and HCAs, pharmacists along with a large administrative team to support the team and patients. We provide education and supervision for GP trainees and medical students.
As part of Easington Central PCN we offer enhanced access and attached staff including a physiotherapist, health and wellbeing coach, clinical pharmacist, care coordinator, adult mental health support, CAMHS and social prescribers. An exciting opportunity has arisen for a Band 4 Senior Administrator- Medical Secretary to support our Consultant's working in Adult Learning Disabilities across Tees Valley and Darlington.
Your role will consist of being responsible for ensuring the provision of a full secretarial service to consultants, junior medical staff, advanced practitioners, and other staff within the multi-disciplinary team.
To undertake non routine activities which may be delegated by line manager, consultant staff or other members of the multi-disciplinary team.
Due to the nature of the role and cross covering sites in the Tees Valley and Darlington area, Wessex House / Flatts Lane Centre, attendance in base is required and therefore remote working would not be suitable for this role. We currently have a vacancy for a G.P Practice Operations Coordinator to facilitate the smooth running of the practice, working closely with the practice partners and the management team.

Part time hours can be worked within core opening hours of the GP practice - preferred shift pattern is 15 hours per week, Tuesday and Friday.

This will contribute to the safe and effective day to day operation of the PCN and the efficient and timely management of clinical enquiries and decision-making.

Main duties of the job
The successful post-holder will become part of the PCNs clinical leadership team, joining an ambitious locality network that is committed to developing a strong, caring and compassionate multidisciplinary workforce.
As a valued team member, you will develop close working relationships with all our friendly Clinical staff, including our existing Mental Health Practitioner. Your role will involve offering specialist mental health advice and providing expert clinical care for people with a range of mental health needs, ensuring that those with complex mental illness do not fall between gaps in services.
In addition to this, the postholder will contribute to safe and effective transitions in care between Adult Mental Health services, where appropriate. You will improve access by being an ' Advanced first contact mental health worker', undertaking triage, assessments, brief interventions, and reviews, with managerial supervision provided by a B7 Mental Health Practitioner.
You will contribute to the clinical supervision structure within the PCN and will contribute networking and navigation expertise, enabling access to a range of appropriate mental health support across the community system.
