Job summary
Band 6 - Alcohol Care Team
An exciting opportunity has arisen for a highly motivated nurse to Join the Alcohol Care Team within our acute care trust here at Mid Cheshire Hospitals NHS Foundation Trust.
We are seeking an inspirational nurse; you must possess strong leadership qualities, have the ability to implement new initiatives and the courage to drive forward future developments.
To be successful in this venture the post holder will need to develop systems which support effective management and care of individuals who present to hospital with either primary or secondary issues relating to alcohol.
Main duties of the job
1. Lead a specialist area of practice, providing expert professional advice to others across the Trust within agreed guidelines and protocols, including medical colleagues and other professional groups when required.
2. Perform comprehensive specialist assessment of patients, plan, treat and evaluate care within agreed guidelines and protocols according to evidence-based practice and changing health needs.
3. Communicate complex specialist information to medical colleagues and others. Refer patients to appropriate clinicians, in the Trust or externally, within agreed guidelines and protocols, according to assessment of patient need and evidence based decision.
4. Prioritise patient care, supervising and allocating one's own workload, monitoring and evaluating effectiveness of care, including discharge of patients, or referring on, within agreed guidelines and protocols.
5. Provide a comprehensive specialist service, accepting accountability and responsibility for the management and effectiveness of the service on a day-to-day basis.
6. Collect, collate, evaluate and report on information, including communicating detailed information to clinical colleagues and others, maintaining accurate patient records and documentation.
7. Promote patient and public involvement in the improvement of services, and involve patients and carers/relative in the planning and delivery of their care.

About us
MCHFT provides a full range of local hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester boroughs (population 357K). The Trust provides high quality planned and unplanned/emergency care, cardiac and critical care as well as child health, maternity services, and intermediate care across our sites at Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford.

Job summary
This is a new additional role to the East Primary Care Network (PCN) to join well established band 7 and band 6 practitioners.
A PCN is a group of GP Practices coming together to share their experience, workforce and services with the aim of improving access and meeting the needs of local patients and communities. The East PCN covering areas including Armthorpe, Dunsville, Hatfield, Stainforth, Thorne and Moorends.
The role is 24 hours per week, working over 3 or 4 days top include Monday/ Friday.
Main duties of the job
- Provide a role in supporting Primary Care Networks (PCN) in triaging mental health concerns and providing specialist advice, signposting and forward referral as appropriate.
- Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system
- To act as a point of contact for GPs to gain advice and support regarding people's mental health.
- To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.
- To provide a comprehensive Primary Mental Health Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery.

Job summary
The role of the Nurse or Paramedic Non-Medical Prescriber will be responsible for the assessment, treatment and discharge of scheduled and unscheduled cases with an urgent nature and assessment initial treatment and referral of emergency cases in patients of all ages.The post holder will use their non-medical prescribing qualification to support patient care according to the local formulary. As a Mental Health Nurse working at Enhanced Level Practice (level 7), you'll play a central role in delivering the PCN's mental health offer. You will be embedded within GP practices, acting as a first point of contact for patients with mental health issues in primary care.
Main duties of the job
This role operates within a busy primary care environment, where timely access and early intervention are key. Clinicians work within structured clinic templates, supporting a consistent and manageable flow of patients.
Up to approximately 80 appointment slots per week will be available within your clinic template. As is typical in primary care, not all patients attend, and not all slots will be filled. This generally equates to delivering around 13-14 patient contacts per day across a mix of new assessments and follow-ups.
You'll be supported to deliver high-quality, structured consultations that prioritise safe, effective, and person-centred care, while helping patients access the right support at the right time.

Job summary
We are excited to recruit to the role of Primary Care Mental Health Practitioner (Band 7) in Hertfordshire's Primary Care Network (PCN) covering the following GP surgeries in the Hatfield area:
Potterells Medical Centre
Burvill House Surgery
Lister House Surgery
Wrafton House Surgery
The role will involve travel between the PCN sites as well as travel to HPFT sites for things such as mandatory training.
You will work alongside other clinicians within the PCN as part of a multidisciplinary team.
You will need to be able to work autonomously to triage, assess, case manage and discharge a patient without referral to a GP where appropriate.
You must be able to advise on medications, and prescribe within scope of practice, identify needs, formulate and triage risk, order investigations and refer on to secondary care services where necessary.
You will act as a first point of contact in primary care. You will accept referrals for triage via agreed routes within the PCN. You will offer rapid and autonomous 'first contact' assessment and promote early treatment by supporting people directly into appropriate mental health pathways across both the statutory and voluntary sectors and will provide short term interventions on the telephone, via video link options, at the surgery or in the person's home where appropriate.
Main duties of the job
You will provide mental health assessment and brief interventions to service users in GP surgeries.
Your tasks will include early triage, assessment, treatment, risk management, identifying and dealing with safeguarding concerns as well as contributing to case discussions and service-related meetings across the adult mental health primary care pathway ensuring good links with secondary care services.
You will provide medication advice and prescribe within specified scope of practice.
You will coordinate the time limited care of primary care-based service users delivering integrated care which takes account of social care needs. You will be someone who enjoys adapting and learning as you contribute to embedding this new role into primary care.
You will also take a leading role for service innovation and development in primary care mental health. This could include projects with other junior staff to ensure delivery of safe and effective primary care mental health services and making a significant contribution to the development and leadership of mental health services within Primary Care Networks. You may be expected to conduct audits to help understand the efficacy of these roles. You will also take part in training other colleagues in subjects related to your field of expertise.

Job summary
The post holder will be responsible for the supporting the delivery of the High Intensity Use service in the community. The role will require engagement with clinical staff to coordinate care for individuals referred and identified by the service. The aim of the service will be to provide positive health and well-being outcomes, through addressing non-medical/social issues in their life, such as housing, debt or isolation.
The post holder is responsible for facilitating discussions between Primary Care, hospital and community services. The post holder will be required to manage patient caseloads and advise on improvements, which will be used to deliver measurable outcomes for the benefit of patients, staff and the public. The role requires extensive liaison with statutory and non-statutory services, to review and support the flow of referrals into the service. Work with the High Intensity Use workforce and stakeholders across the healthcare system to implement referral and feedback processes.
Develop comprehensive knowledge of wider support services for people with high support needs, including social isolation, wellbeing, housing, unemployment and welfare benefits.
Develop the directory of prescribed services and maintain systems for accurate HIU service delivery records.
Liaise with health professionals and build relationships with existing services to increase project visibility and ensure information remains current and relevant.
Support robust patient flow from referral through to outcomes for the HIU cohort.
Create tools to enable effective referral and client engagement, and implement a process for feedback to referrers.
Report risks and implement actions as directed by the line manager / NWL HIU Programme.
Work with healthcare IT systems to ensure appropriate referral and ongoing client management processes are established.

Main duties of the job

Job summary
Providing clinical expertise, education & support for patients approaching End stage Renal disease (ESRD) to enable informed decision making when choosing a treatment option as their kidneys fail.To work in partnership with the Advanced Kidney care Matron & team, Frailty & AKCC Consultant and allied healthcare professionals in identifying patients who may benefit from the support offered. To provide educational support to ensure that all patients have a frailty score when approaching ESRD in order to identify the patients who will benefit most from this service.
To work in partnership with primary care (palliative, General practice) to ensure good links that enable smooth referral where patients choose a supportive care pathway.
To work in partnership with the Trust palliative care teams.
To collect relevant data agreed with the Medical team & Programme Lead to demonstrate the outcomes of the intervention and provide monthly updates to the Divisional Tri that can be communicated to the ICB & NHSE.
Main duties of the job
The post holder will manage a caseload of multi co-morbid patients, with advance chronic kidneydisease providing education, support and coordinating the patient care pathways for in and outpatientsas appropriate.They will also provide specialist advance care planning to patients and their families, and act as aresource for other professionals. By utilising expert knowledge, skills and experience, the post holderwill enable and support the process of delivering high-quality care to individuals reaching the end of lifeon dialysis with advanced care pathway needs.To participate as part of a multi-disciplinary team, working collaboratively with colleagues, patients,carers, other health and social care professionals, statutory, voluntary and independent organisationsto ensure optimum patient management.The post holder will play a key role in ongoing service developments to ensure the delivery of evidence based best practice and assist in meeting Trust targets/objectives
