Main duties of the job
Our reception teams are responsible for busy back of house call centres. A significant part of this role will include working as part of the reception team in the sites back of house reception call centre.
- Acting as the first point of contact for patients and visitors, ensuring a professional and friendly welcome.
- Managing appointment bookings, cancellations, and patient registrations.
- Handling phone calls and directing enquiries to the right team members.
- Supporting clinicians by handling medical reports and administration.
- Maintaining a well-organized reception area.

Job summary

You will be enrolled on the Level 2 Customer Service Practitioner apprenticeship, where youll learn and apply your new skills whilst working for the charity in this role. Youll do your training during your working hours and we will provide you with the support you need.

Provide a welcoming and professional service for both Sheffield Futures and tenants, including notifications of arrival.
Act as a first point of contact for those currently using our service, or those wishing to.
Answer telephone calls promptly and direct enquiries appropriately.
Respond to general email and in person enquiries and pass messages to relevant colleagues or signpost as appropriate.
Ensure good working practices are maintained in the building, ensuring all visitors have signed in and are provided with appropriate access while bearing in mind security and GDPR issues.

Job summary
We are looking for a Locum Community Mental Health Practitioner to cover an assignment with the Ministry of Defence (MoD), at Bulford DCMH, in Bulford, Wiltshire.
This is a locum role (37.5 hours per week) and the successful candidate will deliver an exceptional standard of care to the MoD service personnel.
Pay rates:
PAYE - £24.59 (£21.94 + £2.65 holiday pay) per hour
Umbrella - £28.28 per hour
Main duties of the job
- To provide an outpatient occupationally focused community mental health service within their area of responsibility (AOR) of the Department of Community Mental Health (DCMH)/liaison role. This will include helping to organise and deliver therapeutic interventions, educational training and mental health promotion initiatives in collaboration with the multi-disciplinary mental health team.
- To carry out initial mental health assessments and subsequent reviews for all personnel referred to the DCMH.
- To be responsible for an identified case load by reviewing client needs with ongoing development and evaluation of individual treatment plans.
- To maintain accurate contemporaneous electronic records and clinical notes of all client contact.
- To provide reports to and maintain effective liaison with referring Medical Officers (MOs), General Practitioners and other relevant professional agencies following initial assessment, on discharge or whenever appropriate.

Main duties of the job
The post-holder will provide general medical services, managing a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered patients.
You will work with the practice team to support the achievement of outcomes for Quality Outcomes Framework (QOF); Directed Enhanced Services (DES); Local Incentive Schemes (LIS) and Prescribing Incentive Schemes (PIS).
We are a paper-light practice and looking to further streamline processes.
Sessions involve a mixture of face-to-face and telephone appointments, there is a home visit rota and prescriptions duties are shared.

The South Kerrier Integrated Neighbourhood Team (INT) are seeking a forward thinking GP !
This role would suit a GP who is interested in supporting people with frailty to live and die well in the South Kerrier community and those who live in care homes. The post holder will work alongside the INT Clinical Lead and INT Frailty Lead to provide GP input into the frailty model that is being collaborated designed in the South Kerrier area. The role will sit across the South Kerrier area and will therefore be required to work collaboratively with the 4 GP practices in the area, as well as NHS partners, Cornwall Council, voluntary sector providers and independent sector colleagues.
We will consider flexible working arrangements including sessions ranging from 4 sessions to 6 sessions per week.
Main duties of the job
The Frailty General Practitioner will be responsible for the following (there may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels):
- Attend and contribute to frailty multi-disciplinary (MDT) meetings
- Assist the frailty lead to co-design and develop a comprehensive frailty service for the South Kerrier population
- Act as the clinical conduit between primary, community and secondary care
- Assist practices in the management of complex frail patients, linking with the named accountable GP for the practice
- Commence and lead work on the hospital pull list reviewing previous day admission data to consider a prompt and effective discharge wherever possible
- provide a clinical lead role to facilitate collaborative working with hospital geriatricians and colleagues developing key relationships and understanding of patients need to support an integrated approach to care planning

This is an exciting new role working across primary care, community services and the voluntary sector to help residents experiencing financial pressures, debt, deprivation, social isolation and the wider impacts of the cost-of-living crisis.
Working closely with Social Prescribers, GP practices, local authorities and community organisations, you will provide practical support that helps people stabilise immediate financial difficulties and build longer-term financial resilience. The role directly supports the ambitions of the Worcestershire Crisis and Resilience Fund, which focuses on crisis prevention, financial resilience and stronger community support networks.

You'll be working 38.5 hours a week, (day shift 7.30am - 8pm and night shift 7.30pm - 8am) making a positive difference to the lives of the people in our care at Cygnet Hospital Derby.
Shift patterns are 2 weeks of working day shifts and 2 weeks of working night shifts (rota includes week 1 x 33 hours per week and week 2 x 44 hours per week).

Our integrated male care pathway includes Litchurch Ward, our low secure ward, and the Wyvern Unit, our mental health rehabilitation service.
